Very good, however, you didn't mention restitution of the baby once it crowns. As a student midwife, we only assist the baby turn if the restitution doesn't occur by itself; posing the risk of shoulder dystocia. Also, the third stage of labour can take up to an hour is left to happen physiologically. Otherwise, we can give women an injection of Syntocinon (man made oxytocin) called Active delivery, which speeds up the delivery of the placenta.

Currently revising for my Midwifery VIVA. I have been reading books about the physiology of the first stage but I needed a visual aid to remember by heart this all process. In my opinion this video is very helpful, I am very grateful for the breakdown of sequences and rationale given throughout the video. Thanks a lot. Another video including the principles of fundal dominance, gap junctions etc would be very helpful :-) thanks again.

Anyone who's studying physio apparently has passed the anatomy class so please focus less on the anatomical terminology and more on the physio aspects of it,,, like hormones induce/inhibit what functions,,,etc...

Prostaglandin is a hormone-like subs which promote smooth muscle contraction. I guess we cannot say it is a hormone...

Could you break down the actual mechanism of the 3 layers of uterus and how the multi directional muscle layers work in partnership. The word contraction is actually not the best descriptive although it’s a most commonly used term.

The baby turns by itself it s not the birth assistant that does this its called restitution

Thank you for the amazing information. I send these on to my doula clients as educational tools. It would be great it ya'll could update your language to be more inclusive, using terms like "birthing person" rather than "mom". Thanks for being open to the suggestion! : )

why do we cut or clamp the umbilical cord?

+gen med NOT EVERYONE does it. Some people practice lotus birth in which the cord detaches itself. Other than that in a physiological management the cord sort of "clamps" itself after it stopped pulsating so if we are to cut it that should be the moment. Delayed Cord Clamping.

I suppose that the idea is that all the baby's blood could rush back to placenta if it was held up and thus the baby would bleed out.

This birth is explained as a medical event and not a physiological event. Mother is lying down, the health care provider tilting the baby once crowned, and the umbilical cord clamped right away...? these practices were never the correct one to begin with and are now outdated in most hospitals. Why does the healthcare team does not encourage the mother to do breech exercises in advance once the ultrasound has confirmed the breech presentation.. why planned caesarian? Although baby can be born breeched as well if the healthcare providers are trained to do so. This is not a Physiology of Reproductive System .... :(
