I had this exact same condition and reconstruction in Baltimore at Johns Hopkins when I was between ages 14 and 21 in 1985 - 1991. Ameloblastoma in my left lower mandible, roughly 6cm. The tumor was removed with all molars up to and including the condial. Free fibular reconstruction with bone chips and marrow from both hips. Dental implants inserted from the outside during one of the procedures. Only one implant could be used which resulted in a 3 tooth bridge. The other implant was drilled out and I had to get a soft tissue graft to build up the vestibule. Soft tissue harvest was one of the worst parts of the entire ordeal. My left side is weaker and I predominantly chew on my right side but the left is fully functional with minor reduced ability to open my jaw and a somewhat crooked angle when I open wide.

I'm happy to hear you had a favorable outcome, this is a very difficult tumor to treat. Thank you for watching and sharing your story!

Was the lesion excised with the overlying mucosa and attached gingivea due to perforation of the bony plates ?
@OpenReductionOralSurgery
@OpenReductionOralSurgery Год назад
Good question! We removed the overlying mucosa along with the tumor in the areas where it was perforating the bone, and we maintained the overlying mucosa elsewhere. The mucosa we removed was replaced with soft tissue from the free flap.

I have an ameloblastoma in my upper jaw and had it removed twice and is back. What would need to be done for the paper jaw?
@OpenReductionOralSurgery
@OpenReductionOralSurgery 3 месяца назад
I am sorry to hear this! The upper jaw is treated in a similar fashion to the lower jaw with a resection and reconstruction. I would advise an evaluation with an Oral and Maxillofacial Surgeon who treats this lesion. Usually this is a surgeon who works at a hospital. Thanks for the comment and best of luck!

Great question! He was able to walk normally after the procedure. Thankfully a portion of the fibula can be removed and it does not affect the patient's ability to walk. Thanks for watching!
