My love affair with this video began years ago. There are a couple of versions on YT, some purporting to be taped on a Japanese program, and at least one ( • Little Feat - Long Dis... ) ---not widely seen---video claiming it taped for that most lovely of programs, the Old Grey Whistle Test (of course, you likely wouldn't find this out unless you already knew to search the two phrases together).
So I tried to figure out where it was from, repeatedly, with no real success until a very good internet detective on /r/tipofmytongue took my scattered clues ( / tomt_music_video_looki... ) and found a source purporting to be from the OGWT.
I still harbor a few doubts--Lowell's lack of beard in this take is confusing, but who knows how long between this take and the famous OGWT Fat Man In The Bathtub ~ Rock and Roll Doctor, maybe enough for for some stubble to come in.
In any case, there are enough strong clues to point to OGWT (and it was sold as a OGWT unreleased take...) that i'm comfortable calling it.
Edit 03/12/18: Further excellent detective work from user Big Kahuna, and a hint from none other than Forrest George himself points to this video being recorded in a rehearsal session at The Alley Studios:

Alright, so upon further investigation -- I am able to conclude that it is in fact from the "Basement" rehearsal room at the Alley on Lankershim Blvd. in North Hollywood. If you freeze the frame at 15 seconds, you will see the various signatures on the wall, behind Sam Clayton. Dean Parker was, in fact, correct about the location. What threw me off is that the various wall hangings (burlap sacks) were not in place when I worked there. Bill Elkins, the owner of "The Alley" (R.I.P) was a seamstress, among other things. He made rock and roll t shirt quilts for some of the wall space in the larger room that was dedicated to Lowell George. There was also a banner from the "Waiting For Columbus" tour on the stage for the backdrop. Both ShiloH (Bill's wife) and he have passed on -- however, I am told that "The Alley" is soon to re-open. I have not been by there in over a decade.

Thanks to the fact that the band dark-humoredly reproduced pages from the hospital bills on the back of the album's cover, it's clear that Hayward, admitted on July 7, was discharged on the afternoon of July 12. Soon after he was playing again. And a couple months later he was rehearsing with the band at Alley
