I had three round of accutane over 5 years, starting with 40mg per day for 6 month, after a year acne came back, 2O mg daily for over a year, after 4 month occasional breakouts started, now for last 2 years I am taking 20 mg twice a week and my skin is porcelain clean and perfect. Zero side effects. Low dose accutane is game changer, all the struggle with my skin would have ended easily if my dermatologist gave me this medicine on time. Low dose I started myself, bcs no-one will tell you that it does work.

Thanks for posting this! I'm about to go on low dose Accutane. I've been suffering with acne for decades now (I'm 37). I swear I think the skincare industry tries to hide the success of this by scaring people away from Accutane. They act like it's far more dangerous than it actually is.

@samarthpawale4178 That's something you need to experiment yourself. For me, I have access to 20mg tablets without prescription in Europe, so I found 40mg total per week works well. Oil production is under control, yet I have no dryness and side effects. I never break out even during my periods, I don't have acne in any way anymore, meaning completely clear skin all the time.

I made an appointment with my dermatologist, and had to do a blood test to figure out what exactly causes excessive seborrhea and breakouts. Turns out I'm genetically predisposed to having oily skin, I had a very indepth conversation with my derm about how it affects me and causes emotional distress and was approved for low dose accutane. He said to take 10 mg twice a week as a staring point. I would like to thank you Dr. Jay for making these videos about low dose accutane, and for those who are interested in low dose system, please discuss with your dermatologist and do a blood test prior to make sure that what you are dealing with is in fact acne vulgaris and not something else. @@user-mg7ug9sh3c low dose accutane isn't actually that great :( I have a very oily t zone and normal / slightly dry cheeks. accutane made my already dryish cheeks even dryer and a lot more sensitive and prone to irritation, while my t zone was only slightly less oily. Overall I must say that it did help with breakouts but only if I used a topical antibacterial treatment in combination ( I used azelaic acid 15 %) accutane alone wasn't enough for my pimples and wasn't working as expected.

This works great for seb derm and rosacea as well. For those that are on this journey, when you are clear, if you are getting any side effects like dry lips, etc, play around and find your perfect dose. Keep increasing the time between doses by 1 day and wait 2 to 3 weeks to see what happens. Keep doing this until you have 0 side effects while still staying totally clear. I've been doing this for years and I can go at least 11 days in between pills without getting any bumps, however, I can tell the oil is returning because my face and scalp gets itchy. I can eat whatever I want to as well and skip washing my face for days and it won't break me out anymore. Eat clean though, or you'll have worse issues than acne to worry about, but I ate like trash for over a month one time just for the sake of science, and not a single pimple. I found 2.5 mg a day keeps my oil levels low with barely any side effects. Don't hesitate, be safe about it, but do this now! Stop wasting time and money on garbage that doesn't work! I recently spoke to a very nice dermatologist and she was happy to prescribe me 10 mg Accutane three times a week. The only thing is I want to be able to convince her to keep giving me scripts for life so that I don’t need to keep dealing with it hopefully she will let me 🤞

@@camiririri yeah I’m still on it and currently 2.5 months in. I don’t really know there’s a purge but I am still experiencing very minor breakouts but not as bad as when I was taking 40mg daily. I’ll keep you up to date if anything changes or for when it begins really working.

@@Dan24.Yes, mostly. I still experience very tiny pimples occasionally (mostly ingrowns, which Accutane doesn't always resolve), but overall, it's been quite effective. It's important to note that it took around 8 months for me to really notice a difference due to the lower dosage. So, consistency is key, but you should start seeing results if you stick with it. 👌

I have been on low dose accutane for 2 years. I have found the dose of 10mg every other day keeps my oily skin, acne and sebaceous hyperplasia at bay. I am over 50 and was still dealing with these things. My doctor and I have managed to find this is the solution that works. My question is anti aging skin care. I see some anti aging benefits from accutane and I can even manage .05 tretinoin a few days per week. Could you go over your basic skin care regime while on the low dose therapy? Thanks for this video, I remember you made one like this a while back as well.

Hi there! That's great that you and your doctor have found a solution that works for you! I'm glad to hear that the low dose therapy is helping with your oily skin, acne, and sebaceous hyperplasia. My basic skin care routine while on low dose therapy is to use a gentle cleanser and moisturizer, and to use a sunscreen of at least SPF 30 every day. I would also suggest using retinol or other anti-aging products in addition to the tretinoin you are already using. Do you have any other questions about this?

@@DrJayFeldman I just wanted to make sure I was doing as much for anti aging topical skincare as a could while also on low dose accutane. I've read accutane can also increase collagen but if I can handle a bit of tretinon in conjunction, that is the way to go. Thanks for your response.

​@@parkerb1275 Hey there🙂 I'm not a doctor but there was a discussion about combination treatments with low dose accutane, in the study topical tretinoin was used and the conclusion was that combination is better than monotherapy, however in my opinion its better to use azelaic acid instead of tretinoin. Azelaic acid has been proven to be as effective as tretinoin in the long run, but its naturally present in our skin which makes it safer ( im not saying that tretinoin isn't safe, but maybe getting it internally and topically might be too much in the long run) again I'm not a doctor and this is my personal opinion, just saying that if you wont be able to get your hands on tretinoin, you might check out azelaic acid 🙂

Doc I have 2 questions and I request you to answer them please. 1) right now I am using tretinoin gel should I switch to a low dose of accutane as the gel is irritating my skin. if yes then I should take 5mg a day or 10mg. 2) what about taking natural vitamin a supplement instead of accutane?

Hey, First of all, great video. And here is my story: 10 months ago I took antibiotics (I took the antibiotics for 3 months) along with Epido (adapalene 0.1, benzoyl peroxide 2.5) and Im still using Epido it helped a little, but I still have skin blemishes like blackheads, mild acne, and sometimes even cystic acne. And in a few more days I'm going to my dermatologist to try something else because it didn't work that well, and I'm taking the option of low dose accutane seriously. Do you think he will agree with me that I need accutane?

Hey there! I am in the EXACT same situation as you (but I am finushing my three month course of antibiotics by the end of August, so I am on my second month right now). I am taking Azitromicin (Zitromax) and using a Benzoyl peroxide + Clyandamicin gel (Indoxyl) as topical treatment. I have had acne for eight years now and I have trued everything. Birth control only made my acne worse while the antibiotics don't seem to help. Did Accutane do the trick for you? Wishing you luck

Great channel👍👍 I’ve found a doctor who’s willing to start a low dose therapy, but he also has no experience with that kind of treatment. I’m very sporty and currently I prepare for a half marathon. Im very scared about an rhabdomyolysis of the cardiac muscle. Do you think the risk is low with this moderate dose?

My derm said that young actors and models especially those who have oily skin are often on low dose accutane for a very long time ( probably until they are old, and their skin has stopped producing so much oil). Since its a retinoid, you can't develop resistance to it, but they key is to be on a very low dose, to avoid all the side effects. Also accutane is used for chemo prevention and has been proven to show a significant anti aging benefit. There are definitely possible risks with it, but again low dose is key. ---- I have a couple of questions for you doc; 1) Do you take it at the same time each time or it doesn't really matter? and whats your dosage per week at the moment? 2) Do you use any topical actives like azelaic acid or benzoyl peroxide? 3) May I please know the brand name of your moisturizer and sunscreen

Yes, the intensity decreased a lot. Even my milk spots( milium) disappeared. In my opinion, low dose accutane is the best treatment for adults with oily skin 👍

Anytime! I’m actually on my fourth round of lower dose accutane. It’s been an effective and life changing drug for me, along with an excellent routine. I’m on .3 mg/ kg per day for my weight, indefinitely thanks to an amazing derm.

I had mild acne most of my life, overdosing on B5 (pantothenic acid) worked for me for years. Then i stopped that and everything seemed fine. Years later in 2020 i started getting breakouts again and i jumped on the B5 and this time it had no effect. I knew thaf if i went to a dermatologist they would not give me accutane and i had no money and had no insurance. So i just ordered the accutane from an indian site. I started taking 10mg every two days and my acne cleared up.

Hi I’m considering a low dose accurate treatment for my 14 year old son. We went to his Dr. Yesterday and did his blood work. After I got home I started googling and freaked out to find it could stunt growth. Would this be true on 10 mg a day? I’m so worried I couldn't sleep all night. Please let me know your thoughts.

Accutane doesn't cause hair thinning. The only thing that causes hair thinning is DHT, which in your case, there's no problem because you are using finasteride. The only thing that accutane does is that it increases your skin turnover rate, so as your hair is also part of your skin , they will fall more, but the hair follicle will be still there and producing hair. No worries. Also, when you finish taking accutane that will stop, so I'll recommend you doing it , if you have acne . I'm on accutane for rosacea and acne and it's getting better everyday

@@Dan24. Yes, you can, see my response. It won't cause hair thinning, cause the follicule will still be there, it just makes your hair and skin shed at a faster rate. But after the accutane course, that effect which is really rare, will stop and reverse.

hey there! i am a woman and planning on staying on a maintenance dose (10mg once a week, and then lower it down to every 2 weeks, etc…) for as long as I can. it honestly isn’t hard as long you are on contraception. i.e, i don’t want to take hormones, so copper IUD is amazing!

I am being considered for maintenance dose Accutane for scalp folliculitis. I’ve had a standard course and had complete clearance during however it’s returned. I’m a female 33 years old. Is it safe to be on for years?Im scared of reaching old age and having systemic health issues!

@@DrJayFeldman thanks so much for your swift reply! I’ve just finished 4 weeks of Flucoxacillin, I’ve had Doxycycline in the past for 3 months. Between that and topical Ketoconazole, ciclopirox olamine, and topical steroids I’ve had no relief :( My main concerns would be hyperostosis, hairloss and I’ve seen some forums citing sexual dysfunction… are these significant concerns or more non causative correlations??

I have a question, my doctor perscribed 40mg per day (I weight 54 kilos aprox) for 1 week per month for a 6 month period. I will be taking the pills during 7 days and then resting for 21 days and so on for 6 complete cycles. I honestly have very few acne and spots I want to cure but it is not severe at all. Do you think this dose will be effective?

My question is if you go that route of 10mg every other day or 2. Is this something long term you do for years like rest of your life or treat like a normal accutane session? If normal session how long will it take? I assume years?

Hy i have extremely oily skin i have been on Accutane 5-6 times for 6-8 months regular I got rid of My acne but my oily skin is just unmanageable,it makes me look dull ,Accutane helped me a lot before for getting me rid of my oily skin ,i want to ask that is the side effects is temporary aur just permanent? Or can I Accutane for longer period of time like 10 mg in 2 days to manage only my oily skin ! Please reply it will be really helpful

@@DrJayFeldman i have tried many products to get rid of this but nothing worked for me, my oily skin is just unmanageable so he recommended me Accutane to make it less oily and it worked for me ,I got a regular check up and the report were also good .So I want to start it again as u recommended 10mg in 2 days, I think taking so low dose Accutane will not show that much side effects also and i can manage my oily skin also .

hey doc, im on a low dose accutane too. Just a random thought that crossed my mind; is it possible to develop resistance to accutane? like will it all of a sudden stop suppressing oil production after years of usage ?

It's possible that over time the effectiveness of Accutane could decrease, though I'm not aware of any research that suggests that users can develop resistance to it. It's definitely worth talking to your doctor about. Have you had any experiences with Accutane that made you wonder this?

@@DrJayFeldman Personally no, i was doing some research on how acne related to antibiotic resistance is treated with oral isotretinoin, and wondered if its possible to develop resistance to accutane, but couldn't find any studies. Will have to speak to my derm and see what he thinks

@@DrJayFeldman Hey Doc, so I found out that you can only get resistant to antibiotics and not retinoids. Since oral isotretinoin is an retinoid, resistance to it is not gonna happen. I have also found out a post on reddit, where a woman was using low dose accutane for about 7 years with great results, and after stopping her skin stayed the same ( just a little oilier). As she stated " I think that I've been using isotretinoin for so long that my skin has changed its chemistry and I no longer need it, but if I'll notice first signs of acne I'll definitely be back on it". Low dose accutane has changed her life for the better without any serious side effects.

From my 7 years experience - no, you don't develop resistance to it. Your complexion will be perfect as long, as you're using it, provided that you use it for seborrhea. Because if you stop, oiliness will come vack in about 2 weeks. I heard it is different, whenYou take it for acne. Then effect is more permanent.

That being said, it is possible to use 10mg of Accutane in combination with 0.05% tretinoin in the evening, however the dosage and frequency of use should be determined by your doctor. @@DrJayFeldman Doctor I read that you use accutane for oily skin. I started with 20 and then 40 mg and have now gone to another doctor, who now advises me 10 mg per day. I was just curious because you also take for oily skin. I have had more inflammation under the skin after using accutane, which does not heal quickly and therefore leaves a scar. Did you have that also? Can it be true that accutane slows down the healing of an under the skin, causing scars? My doc says no, but I’m not sure I never had scarring before only pores. And could advise something to stop becoming a scar/ice pick. Looking forward to hear from you many thanks 🙏🏻

I finished the course for acne on the body, but 2 weeks after the course I got sebaceous glands on my forehead. So I started taking at first 1 tablet a day (20 mg), now I take 3 days, then a break of 1 day. I also have seborrheic dermatitis on my face (manifested by red nose, forehead) and an oily T-zone (no accutane). It's going well, but I can see and feel that the skin on my forehead is not completely cured. I think a large dose of Accutane has solved this problem (for the duration of this dose), but can tretinoin help? I have read that even 30 minute contact therapies have improved the skin. I tried azelaic acid once, but it just made my face red and that was it. What about tretinoin?

Great video Doctor! I'm planning on discussing low dose accutane ( 10 mg twice a week ) with a dermatologist. I did manage to get some results using topical treatments, but my sebum production is driving me crazy. May I ask you how often do you do blood tests?

I am seeking advice on alternative treatments for my GRZYBOWSKI Syndrome. I am an extreme case because the cancers ITCH deep into my soul almost & deep tissue burning. I've had Mohs most often... the layering & scraping with a curette. Some spots take as long as 6 months to heal & close up.. I have had to have some removed twice... I keep asking doctors here on YT & Instagram for ideas & alternative treatment... I am awaiting an appointment with a skin cancer specialist dermatologist. I like to participate in my health care & try to educate myself so WE ( doctor & Me) can discuss options and together work on a battle plan...

Hi Doctor Jay , first of all thanks for the video. My son is a 17 year old started taking accutane 10 mg / day for the first month and now after the blood test and approval of Dermatologist he continues with 20 mg per day for one month. Do you think that he could still keep the 10 mg per day for the same results ? Skin looks better after the first month but I’m worried for his liver or side effects that can appear.

I hear sexual dysfunction is now considered a side effect and it decreases testosterone. Is this a permanent side effect? Does permanently alter gene expression ?

@@DrJayFeldman thanks for your reply! I have done a low dose long term course (20mg/day) 1.5 years, but may need long term maintenance possibly 20/30mg per week. I am unsure if I experienced SD upon cessation as I had other stressful factors at play although it is a concern as I have read about permanent cases. Should my side effect profile on maintenance be less than my standard dose regime? I didn’t experience hairloss first time but worried it may occur this time
