If the lung degrades the serotonin why is carcinoid syndrome occurring in liver metastasis?

So the lung heart carcinoid tumor was on my practice NBME today and I was like WHAT IS THIS lol. So this is high yield

why doesn't a carcinoid tumor in the lung affect left side of heart? if the tumor was in the lung, wouldn't serotonin directly go to pulmonary vein and get to left side of heart?

This just don't make any sense. Adenocarcinoma is strongly connected with smoking, a lot of other research says this. The only difference is that a large percentage of non-smokers are associated with this type of cancer, more than any other type of lung cancer.
