One of my best campaign elements was a kingdom whose young king had just come of age and taken over for the regent, but he was a selfish, self-indulgent little jerk who would rather drink, gamble, and chase women with his friends rather than rule properly. Try to tell him he might want to do things differently, and he'd punish you severely. This led to a succession conflict. It definitely got my players more invested in a fantasy kingdom than I've ever seen before.
