Obsessive-Compulsive Disorder (OCD) can seem like a strange and bizarre mental health condition that doesn’t make a lot of sense. However, once you understand what is going in in OCD and how obsessions and compulsions work together to drive OCD, then it actually makes a lot of sense. This video introduces and explains a behavioral model of understanding OCD, describing how obsessions lead to anxiety and distress, which is then reduced (temporarily) by engaging in the compulsion. The compulsion briefly leads to a reduction in anxiety, thereby reinforcing engaging in the compulsion. However, the reduction in anxiety is only short term and by doing the compulsion, it reinforces the obsession and maintains the anxiety in the long term. Thus, OCD becomes a self-fuelling engine that drives itself forward. It is critical to understand this model of Obsessive-Compulsive Disorder (OCD) because the treatment of OCD (something called exposure and response prevention or ERP) comes directly from this understanding of OCD. I will explain exposure and response prevention and how ERP works in an upcoming video.

A very clear explanation of OCD. It made me think of Andreou's concept of “intransitive preferences” because in regard with OCD the “intransitive preferences” theory would state that between engaging in a reassurance-seeking spree and not engaging in one, people with OCD would tend to prefer the first alternative because it momentarily feels better. However, between a life of OCD and one OCD-free, most people would prefer the second alternative because it’s overall more satisfying even if they are unlucky and their worst fears materialize. What I think is the underlying cause in most of OCD cases is the fear of uncertainty in life or the mindset of desire that everything in life should go well ,there should never be any problem that brings shame or guilt or pain . As a result even if we tell someone with OCD or depression that you don't have to live with this fear or pain ,they kind of ask or seek assurance nothing bad will happen in their life and that's where things get screwed up . It's of paramount importance to understand that whatever happens ,just happens ,nobody has any control . This is hard to articulate and accept . As long as you live in ignorance it's easy but otherwise it's ok for those who can accept that life can be unpredictable they can deal with their fearful thoughts easily but for those who can not it's a pain.

I did something morally bad(sexually) in my 16 after 7 year ....the thought came in my mind and i feel fear about that thought.....it repeated everyday... everyday feel like hell........ 🥺

Love the video. very clear. By cuting compulsions, we should expect some period of pain that will last and then calm down and with practice this is how we rewire the brain?

Correct - if a person can stop engaging in the compulsions, it cuts off part of the fuel supply that is driving the obsessions.

Engaging in compulsions can maintain the OCD and, with time, it's not uncommon for a person with OCD to pick up new obsessions/compulsions.

Can you have anxiety disorder and have normal levels of cortisol. I only mildly get palpations occasionaly, have trouble sleeping though. Basically im aware of my breathing all day everyday but im not hyperventilating. Can you have ocd symptoms but no flight or fight response. Can it all just be cognitive or emotional anxiety

Hi Colin - thanks for your question. Yes, you can experience anxiety and not have a lot of physical symptoms of anxiety. Anxiety can be experienced primarily at a cognitive and emotional level, without the same degree of physical symptoms. Hope that makes sense.

So it,s not something physicaly wrong with the brain? I don't have ocd i have generalised anxiety.

It's not that there is something "wrong" with the person's brain in OCD, but more a pattern of behavior and thought that drives the OCD process. These behaviors and thoughts can be altered through therapy. I'll be posting more videos about this in the future.
