Im 47,I've been using finasteride for 25 years with minimal results. I'm now on Dutasteride 0.5mg three times a week with finasteride every other day, Minoxidil 2 times a day,ketoconazole shampoo every day,and dema rolling once a week.After 20 odd years my hair line is starting to respond.with New hairs and thickening. Dutasteride 0.5mg has minimal side effects,well tolerated and its been far more effective that finasteride.👍

25 years on fin with minimal results?? If you still have hair at 47 id say you had good results! Although its good to know that you have noticed an improvment when adding Dut after that long.

Dutasteride every other day? There's absolutely no point for you to continue taking finasteride then. Your just making it harder on you liver without providing any benefit or mixing two medicines that usually are recommended no to mix.

Why would u take dut for hair loss i do not get it as just the enzyme 5alpha reductase type 2 is responsible for hair loss, not type 1. Dut reduces both while Fin just type 2.

What a load of bull crap. I hate people like you feeding out misinformation. You’d think 25 years of taking finasteride with minimal results you would try to change something up after idk, maybe 2-3 years? No one sticks to something that does nothing for them for 25 YEARS. Just another maniacal exaggerator so people come flocking their story. If you really have been using finasteride for 25 years that means you noticed significant hair loss at 22 years old and have maintained your hair ever since from it. That’s a bit more than “minimal” if you ask me, it’s not common to regrow hair from finasteride that’s more on the uncommon side. It is however common and expected to maintain the hair you already do have.

Hello Doctor and thank you for the helpful videos about hair loss. Even though Finasteride and Dutasteride taken orally reduces serum DHT levels by a large amount, there will still be DHT leftover on the follicular level. Some people are just more sensitive to DHT and keep thinning on oral Finasteride or Dutasteride. One way to adress this could be a) Oral Dutasteride with a higher dose b) 0.1% topical Dutasteride as an adjunct treatment once or twice weekly c) Topical Anti Androgens. I don't think Minoxidil can adress this issue in the long term. Also i know that your options as a certified hair surgeon are limited to mostly FDA approved drugs.

Thank you so much for your educational content! I would like to know 1. What is a good way to self-assess at home if Finasteride is working (i.e is there a standard accurate way to measure the hairline to make sure it’s not receding further with time with Finasteride use)? 2. I just started Finasteride 1mg x 3 times a week. If it doesn’t stabilise the hair loss after 6 months, should I increase it to 1mg everyday? Or should I go for Avodart?

first step is to take it every day, reevaluate after a few months. Then you can try adding dutasteride 1/week.

Does anemia or Crohn's disease affect the body's ability to introduce Finasteride into your system effectively? Either through poor absorption or circulatory issues.

Dr Vikram, can you explain in detail at 2.43 why DHT could not be the total answer as to why androgenic alopecia is taking place and what if the person has also had a blood test to rule out any other problems? I think it would be helpful.

Yes, as bulk guy said. All androgens on the scalp cause hair follicle miniaturization, it's just that DHT is the most potent at it. So for most lowering DHT is enough to combat hair loss, but for those who's hair follicles are very sensitive towards other androgens finasteride will have less effect.

He doesn’t know, he is just hypothesizing, and no, testosterone does not cause hair loss. Look a the people with no 5AR activity due to congenital defects, they never go bald.

​@@mrtree4871 I have observed some hair regrowth in the initial stages of taking finasteride, although it is not particularly noticeable. My only regret is that I did not begin using finasteride at the earliest signs of hair loss. Best regards,

Same I'm also losing hair, on fin and minoxidil for 5 months experiencing so much shedding. But I see new baby hairs in temples and my hair became so thick. I wash my hair daily because of dandruff. With a natural shampoo and a ketonoconazole shampoo. I have an extreme oil scalp. So the question I'm I losing hair because of the dandruff or shedding phase or due to washing my hair daily?

I wouldn’t say it would accelerate hair loss Per se, but being that DHT is a byproduct from testosterone and you would be adding more into your body, there than would be more testosterone available to be converted to DHT regardless of the amount of trt you take. You could have the dose of finasteride increase to a level that you’re body tolerates but also keeps any side effects at bay. It’s kinda tricky to find that exact dosage that gives you the effect we’re looking for but also keeps side effects low if any at all. I hope that makes sense

@@GokuPlaysUK DHT is a byproduct of testosterone, the question that I posted my answer to stated that he was having trt done which would in theory be a catalyst to make more DHT. I’m not a doctor, just applying my knowledge into the science aspect of it

I Switch from finasterid to dutasterid tomorrow. Finasterid slowed my hair loss a bit but nowhere near good results. If somebody has experience i welcome everyone to Talk about it since its a Big topic in my life

A mature hairline will look like you have a higher hair line than as a child. You may also have a slight M shape on your hairline. Balding will be a further step from this where that M shape is pronounced, see pictures of Jimmy car before his hair transplant for what that looks like. Alternatively you may be loosing it at the crown which is an easy sign to spot. You could also be suffering from diffuse thinning all over although its not as common. Its a lot more difficult to spot this.

For someone who (for a year) has been on oral Dutasteride at .5mg/day and oral Minoxidil at 5mg/day, and still losing ground fast with normal bloodwork, would you suggest 1.) Switching to topical Fin? 2.) Switching to topical Dut? 3.) switching to oral Propecia (brand name) at 1mg/day (brand Avodart is too expensive)?

Try ketocanazole shampoo, my scalp itches like hell because of persistent dandruff for years and minoxidil is making it worse. I recently add keto in my routine and it actually helps to reduce the itchiness.

Hi Doctor. I will make 3 questions, and separate, so its not so confusing. 1 Question - Hair transplant can induce premature tellogen effluvium of surrounding weak hair. ( Can be named as Shock Loss, correct ? .......... 2 Question - Now if a person have hair, and need to make HT only on temples and crown. Could the " healthy " and non miniaturized hairs be " lost " in the process of HT? I do know that sometimes even weaker hairs can grow back. But is possible that the healthier ones couldnt grow back? Or it only happens with the miniaturized ones? .......... 3 Question - If a patient do HT only in temples area. Will the " shock loss " happen in ALL head, or only in the surrounding area of where the operation was made. In this case would the shock loss occur in all head or only like 1 or 2 centimetres of the temples. Thank you.

And what if st first dutasteride and minoxidil worked wonders but then lose almost 50% of my hair in 3 months with an insane scalp pain. I guess you're talking about people who doesn't respond since the beginning of the treatment, but what happens with people like me?

when you say you add in dutasteride to current patients on finasteride what doses are we talking? i have been taking 0.5mg of dut for 2 years and it's kept my crown at bay now all of a sudden it's shedding like crazy and i don't know what to do

Dr Vikram and Dr Knudson. I absolutely adore your hair loss channel. Incredible and honest information in an industry that is full of scams and corruption. Can I ask what you recommend as a starting dose for Oral Minoxidil. It's not widely available here in the UK unless on Private prescription. I have just found out I might be able to get it because the topical one is just a nightmare to keep using consistently. I just wondered with all your years of experience what you suggest to your patients as s starting dose with the 1mg finasteride.

@@kylekeenan3485 Oh gee, wow, I never thought about that. Really? How very smart of you. Firstly, my GP is not a hairloss expert as oral Minoxidil is not prescribed by them so you need to be referred to a specialist. Secondly, its useful to get an opinion from someone who is a specialist in that area and who has many years experience with other patients using the same drug. Thirdly, whilst I hoped Dr Vikram would respond whilst I waiting for a confirmed appointment with a dermatologist surgeon I could then take the info from Dr Vikram and present it to my surgeon as useful information in case he/she was unfamiliar with what the starting dose should be as there is no exact data on it. That's why we are all here, to she and collaborate with each other. And to update you from when I made that post to the present day, my surgeon, when I got my appointment was really delighted I presented him with what Dr Vikram advised and is actually going to contact him and collaborate with him. So well done Einstein for a very well thought out comment which i never even thought of. It never crossed my mind to ask my Dr. You get an A+ douchebag. Now go back and play on your computer games.

Great vid, Did you ever had patient that lost hair despite using dutasteride and finasteride? If someone is losing hair while on finasteride and dutasteride and stabilizes at norwood 2 or 3 after a year or two, is he eligible to do hair transplant, i know some hair is weaker than other, so lets say that temples are really weak and other area is not that weak so finasteride can keep hair in that area, would you fill in the temples on that patient despite him losing temple hair on dutasteride and fin

Would you recommend 0,5mg Dutasteride (Off-label) once a week althought you have been on Finasteride 1mg/day for 2 years with stabilization of the hairline and no further hair loss? Just to prevent "potential" further hair loss down the line.

@@johncrow2852 The heart thing was an unsubstantiated claim(on reddit?) that there was heart damage. Pretty much most people dismissed it, but it spread everywhere. Still, I wouldn't mess with the stuff since it all originates from shady chinese suppliers. One popular seller's stuff was tested and it came back under 50% pure with a claim of 99%

Hi! I've been taking finasteride 1.25mg every day for 7 years, and about two months ago I added dutasteride 0.5mg every day in addition to the finasteride. Since then I've had heavy shedding and my hairline looks like it's suddenly getting worse very quickly. I fear I may have messed with my hormones and caused either telogen effluvium, or reflex hyperandrogenecity. Or, the increase in scalp testosterone could be the culprit. I'm regretting taking dutasteride but even if I stop taking it now it won't leave my body for a long time due to the half-life. I feel stuck and desperately need some advice. Please help!

@@relampio yeah it's really weird and hard to pin down with so much conflicting information. Some say an initial shed means you're responding to the drug, promising regrowth in the future... at some point, whereas others who've stuck it out, well, I've read a lot of bad experiences at this point and quite a bit about how dut also inhibits important neurosteroids by blocking type I 5a, potentially causing cognitive side effects. What the long-term effects of this could be, frankly, unnerves me to think about. I'm seriously regretting even bothering with dut. I should have just remained content with fin and thought a bit harder about it. At this point, self-acceptance is the key. I'm not going to find happiness with myself by screwing around with my hormones and brain chemistry to maybe keep a bit more of my hair. What about you? What's your experience with the whole thing been like?

@@Jazzzzzzzzzzzzzzzzzzzzzzzzz i understand your point and think it is the best idea. i faced gyneco related symptons after 2 months usage of duta, then i panicked and used aromasin for a month, of course quited duta. even after that i couldnt use fin due to the symptons that werent fully resolved. so after 3 months pos duta im starting to use fin daily again because the symptons are gone, the hormonal rollercoaster that i caused probably is the reason why im facing the worst hair condition ever. ill stick to fin til the end because it always was safe to me, recommend you to do the same, cheers.

I have done transplant in 2021 march front 4 grade approx 3900 graft on crown somewhat baby hair I had so left that area after HT I started Finasteride and minoxidil and first 6-8 month take 4 prp also . After one year lots of hair fall without reason even on Finasteride and minoxidil I knew it might shedding but not stop and continue fall my crown visual so my doctor recommended Dutastride for daily 0.5mg so my regime 1 time minoxidil and daily 0.5 mg Dutastride and 3 times kito 1% with zpto combinations shampoo in week 6 month were good but after November start again slowly falling 10-20 day same continue last month but now fall increase without reason same routine 0.5duta one time Minox and kito zpto shampoo 3 week and other day mild shampoo my hair on crown like small hair and visual clear even in middle all long hair fall only front I have long but somewhat density also reduce from front and thin What should i do ?

Was on finasteride for about a year and a half and it seemed to only work for the 4th-7th months and then stopped after the 7th. Switched to duetasteride and it doesn’t seem to help much. Some times it seems like I’ll go a week or two without shed but then sometimes my hair is rapidly falling out. Been on duetasteride a year and a half and have only lost progress. Oh yeah and I’ve been taking minoxidil the whole time with both drugs. Not sure what to do at this point.

It's not only about dht blocking,, but also about inflammation ,,your scalp gets more sensitized by alcohol which is present in your minioxidil,, you can try fin 1mg alternate day and minox 5 times and in weekends (sat and sunday) you can try anti inflammatory topical corticosteroid lotion to reduce inflammation such as mometasone fuorate 0.1 (dht affects hair loss but it is more often affects by inflammation which may be induced by stress ,dht and many other etiology ) you should implement my advice ,,hopefully it helps

Im 3 months post accutane i definitely see thinning shed is still existent..as for dutasteride i started 3 times per week (Sunday tuesday Thursday) it took like one month maybe a bit more to start seeinv improvment but its definitely a noticeable difference from finasteride, drug didnt do anything

No. They're all myth. Only side effect possible are low libido and enlarged breasts. To get feminine features, you would need to increase estrogens in body which is possible via harmone replacement therapy

If you have gyno it means your estrogen levels are naturally high and reduction of androgens is causing those andogens to increase fat near your breasts. Maybe introduce micro-needling or topical anti androgen like RU58841. If you are still experiencing hairloss then you have to accept it and hair transplant probably not good idea as you will have to cut short anyway due to further hairloss behind the hair transplant

Finasteride is a blocker, if it works for you, it'll stop the hair loss process, you're likely to see that happen after taking it consistently for at least 3 to 6 months. Finasteride doesn't guarantee your hair will grow back and get thicker, because that depends on other factors like your health, nutritions, status of your hair follicles, stimulations and such, but in my own experience and the understanding of it, once the hair loss and miniturization process stop, naturally your body will try to heal your damaged hair follicles, I certainly got a lot of my hair back after taking finasteride for 9 months, and I know lots of other people do too. Give it time, if it works for you, you'll see it.

My understanding is that, since DHT level isn't high enough to harm the follicles anymore due to finasteride, the body is healing itself. Finasteride in itself doesn't really promote hair growth.

Yes mate, readily available from pharmacy’s online. I’ve been using “the independent pharmacy” it’s a real pharmacy, real doctors, prescribers etc. fill in a brief online consultation & they will write you the prescription in order for you to purchase from them. There is also an option for them to inform your regular GP about the medication you are going to start, so everyone is in the loop. I’ve been thinning all over for around 5 years and noticeable shedding, since being on dutasteride for 15 months, I’ve noticed my crown has filled in but most Importantly I have zero noticeable shedding. So what I have now, I will keep for as long as the treatment is continued.

@Stephen7 Well i was on fin 1mg daily initially and around 4 months in my eyes started going funny. A haze, intermittent blurred vision. Stopped that, took a bit of a break. Thought I'd give dut a go. 0.5 mg daily. Same again upto 4 months in my vision was so ridiculously blurry. More so in the morning's. I could hardly see what i was doing in the bathroom mirror. Can vouch I've had no issues with vision prior to starting these medications.

Hi love the channel. My hairloss started about 10 years ago. Was balding quick (have the photos) I jumped on treatments very quickly. The usual stack! That we all know. I changed up a year or so ago taking finasteride every day to 3 days a week. Did a bit of research including this channel. It didn't need to be taking every day as it can stay in the system etc and also saving me money in the long run. So I'm taking it Monday Wednesday Friday and nothing at the weekends. I take hair supplements everyday. Hairmax laser Monday Wednesday Friday. Minoxidil foam in the morning Minoxidil liquid at night. Dederminator (derma roller) nizoral shampoo leave in for 10 mins. But for the last year or so I've noticed my crown area has started to really thin out again! I tried PrP treatment but because of covid the three sessions I brought got completely wasted! Any thoughts??? Am I doing too much?? Should I add dutasteride with my finasteride? Go back finasteride every day?

Hey! I'm curious, what would you advise? I'm 22 and I'm taking finasteride 1mg, 3x a week, for 5 months Additionally, I'm applying topical minoxidil once a day, for the last 5 months. No results at all, the hair loss is still getting worse, but there are also 0 side effects. I thought about increasing the finasteride to x7 a week, and do it for another 6 months. What would you advise? Thanks!

The same with me bro, 5 months on finasteride Only, 3x a week. No result. I'll try maximum dose (1 mg a day) and see if it works. Have you tried maximum dose on finasteride? Is that work?

@@fajri6721 Hey Man, Yes, a month ago I started taking 1m finasteride every day, so far no results (and no side-effects as well), but it has only been a month. I'll give it another 5 months, and hopefully I'll see an improvement ^

Many people (including myself) have a hard times getting results even from Finasteride, which is a pretty powerful drug. Saw Palmetto is weak af, I wouldnt put my eggs on that basket.

@Samir Thamer Indeed Brother; nothing beats the Big 3: Finasteride (Dutasteride if Finas dont work for you after 1-2 years), Minoxidil (both topical and oral, when possible), and Ketoconazol (just gives a little help/push to finas and minox). All the other stuff is BS and money-trap. The only last alternative is shaving or hair transplant.

even on dutasteride, there is still scalp testosterone present. There would actually be a spike in scalp testosterone due to it not being able to 5 alpha reduce. If you look at trans women taking agressive anti androgens that inhibit everything, they have way better results than men on dut and fin.
