That arm was in pretty good looking condition considering what it had just been through. I was expecting a compound fracture but it appeared to just be a big flesh wound. He was very lucky.

@@NealB123 No, the arm was completely crushed from mid-biceps down. We can’t save a limb with a massive crush injury like that. It’s never the bones, it’s the blood vessels, nerves and tissues we can’t repair if the injury is too severe and that looked like an arm needing an amputation. They’ll get him to the ER, do a Doppler ultrasound and if no blood flow and the surgeon says no, his arm comes off. That looked like a lost arm to me.

It all depends on how bad the crush injury has damaged the circulatory system doesn't it? And how long he was trapped? It didn't look good for his arm, the dramatic step shape of the arm (the difference between above the injury and the crushed area) makes me think the injury is more severe than being just skin and flesh. In my experience patients with this type of injury sometimes don't do very well because of the combination of blood loss, shock, and damage to bones, muscles, etc. I couldn't see if they managed to get a tourniquet above the injury. It would be around the very top of his arm to help with stopping the bleeding until they make the decision to keep or remove it. I imagine that it would need a long surgery to save it and then more to reconstruct it with skin grafts etc. And then healing and rehabilitation. Poor guy made a few stupid decisions, paid the price and will continue to pay the rest of his life and with nerve damage. Blessings from South Eastern Australia, Dot

@@dotcassilles1488 A tourniquet is not needed for a crush injury. That’s the point of what I was saying. Once a limb has been crushed, everything - bones, interstitial tissue, blood vessels, and everything are crushed. There will be no blood flow to the arm because well….. it’s been crushed. Depending entirely upon what the surgeon finds on exam and testing, he stands a very good chance of losing the arm. I didn’t work in the ER but I am a retired ICU nurse.

His arm was under the door of the car, he was not getting out of that by himself. His arm was crushed and looked like there was skin, flesh and muscle missing as well. Lifting the car off him too fast could have killed him because of crush injury, where the built up toxins race to the heart and stop it beating when you release the pressure. They needed access to the arm to put a tourniquet on the very top part of it, almost in his armpit. That way he wouldn't bleed out when they lifted the car. If you were worried about the car then most likely it would be totalled because the frame could have been weakened as a result of the roll-over. Most insurance companies would write it off as soon as they heard the mechanism of the crash. Leaving it intact and having to work around it would have been very hard as anyone putting weight on the car could be adding pressure to the injured arm and making the injury worse. That's why you could hear him screaming when they moved anything, cut the A pillars and when the firey climbed in from the back to remove stuff and cut the C pillar that was resting on the road. Pretty sure he will have to live with nerve pain or phantom limb pain (if he loses the arm) for the rest of his life. Blessings, Dot

@dotcassilles1488 you are 💯 correct. The saying "in a pinch" comes from the train yards of old and the hooked the train cars together by hand. A worker would get crushed between the cars and hence, "in a pinch." They'd go and get his family to come say "goodbye" because once they uncoupled the cars, the worker would die. Hard to imagine, but it happened. My comment was satirical and not very nice. Based on your reply, I imagine you being a super kind and caring person, so you missed the meaning of my comment. Not defending drunk drivers (if that's what happened) but have you never made a couple of really stupid decisions and had to live with the consequences? Don't worry we all make mistakes. If you haven't already then it will be your turn eventually. I hope when you do make mistakes that you won't face as painful consequences as this man. He will live with chronic nerve pain the rest of his life if he survives. Doesn't matter if he keeps his arm or loses it, the nerves are already damaged. Lifting the car might make his injury worse if it put more pressure on his arm. They also needed to control how quickly they released the pressure from his arm... How that is done depends on how long he is trapped for. Most likely he has crush injury. Crush injury is where the circulation is stopped and all the toxins start building up in the area where there is no circulation. Lifting the heavy object could release the toxins into the rest of the body and damage more areas. If the toxins rush up to the heart it can cause your heart to stop and you die. It damages lungs and brain and other organs like kidneys. Being the rescuer in this situation is complicated because you have to balance getting the bleeding stopped, getting him out and to a hospital but not rushing to release him and risking more widespread injury. He could still die once you release him, even hours or days later from the affect of the toxins. In my experience people can get crush injury in their arms or legs or sometimes across their chest or one side of their body. Most often caused by accidents involving machinery, vehicles or objects like sheets of metal, gas cylinders or anything heavy. They may not have open wounds but have massive internal bruising/bleeding. A similar but less scary experience of this is when you get pins and needles, a numb foot or butt from being in one position too long or people experience it when they fall asleep the wrong way. A quick release of the pressure could kill him from toxic shock and crush injury depending on how long he has been trapped. They need access to stop the bleeding and prevent further injury with a tourniquet, give him pain relief, etc. Any movement or pressure made him scream. I kind of wondered that myself. I use to drive tow truck and have been asked to render such aid. Since we were on the police list we did have certain coverage.

He screams when they put any extra weight on the car, like when they were cutting the pillars of the car or when the firey got in to remove stuff from the back so they can't just work around the roof. He wouldn't be able to get his arm out from under the door and crawl or walk out either so it takes 4 to 6 people to carry an injured person. Rescuers will be balancing the risk of going slow to reduce how widespread the damage is versus rushing to get him out, risking killing him or doing damage to his body. She clipped a semi coming the other way and became wasted. At least she didn't kill anyone else. Her drunk freind in the passenger seat survived. Whole drivers side of a Montana van got erased
