Mantle Cell Lymphoma - Aggressive B-Cell Neoplasm - Non-Hodgkin’s Lymphoma (NHL) - Hematology
Hematological malignancies can be divided into leukemias, lymphomas, and multiple myeloma.
Lymphomas can be divided into Hodgkin's lymphoma and non-Hodgkin's lymphoma.
Non-Hodgkin's lymphoma can be subdivided into aggressive lymphoma and indolent lymphoma.
Aggressive non-Hodgkin's lymphoma includes:
*Diffuse large B-cell lymphoma​ (DLBCL)
*Burkitt's lymphoma.
*Mantle cell lymphoma.
*Precursor T-cell lymphoblastic lymphoma.
*B-lymphoblastic lymphoma.
Indolent non-Hodgkin's lymphomas include:
*Follicular lymphoma
*Marginal zone B-cell lymphoma (MALToma)
*Hairy cell leukemia/ lymphoma.
*Waldenstrom macroglobulinemia.
*Mycosis fungoides/ Sezary syndrome.
Mantle cell lymphoma is a rare, aggressive B-cell non-Hodgkin's lymphoma that affects people around 63 years of age...More common in females than males...t(11;14) translocation is common. Overexpression of BCL-1 "cyclin D1" leads to increased proliferation of cancer cells.

My father-in-law was diagnosed with this 9 days ago, on a Thursday. He died 2 days later. He was 90 years of age and did not want treatment. He went very peacefully, much respect to him and all the hospital staff.

Thanks for this video. It has been 4 yrs since my diagnosis of mantle cell. I have watched and read a number of articles but yours is very simple and easy to understand. I had the treatment you described and a stem cell transplant. I was 60 & female. I’m now 66 and have never been healthier. I am proactive re diet and exercise. I took control of my own health.

@@starrysky1588 I work hard at being as healthy as I can. I focus on food as my main source of healthcare and keeping my organs working well. My liver and kidneys have improved greatly. My main struggle is digesting food and I struggle to be able to be active. I am just happy for each day I get with my family. Thank you for asking

@@myenchantedlife5262 I am glad to hear that your kidney and liver are improving. You will heal more just be patient. Being active is after all, not the primary goal of life. You can rest and read books and do many other things. The main concern is health. I wish you a complete recovery! And indeed, every day we all get to spend with family is a blessing for all of us!

I have recently been diagnosed with Mantle B Cell Lymphoma, I start my chemo next week, but great to understand where the mantle sits within the lympthnode, i am 55 was fit and healthy, and literally started to lose weight in the last few months, and my spleen is massively enlarged, if all goes well in the next 6 months, i will be having a stem cell transplant, anyone out there who has this horrific disease, STAY POSITIVE x.

Hi Macmad, I am 5 yrs from completion of a successful treatment for mantle cell. Stem cell transplant wasn't pleasant but it has certainly done the trick. The problem I had wasn't the treatment as it is a well documented process and procedure that many people have been through with great outcomes but the after care in my case was left wanting. More information needs to be given re recovery back to normal healthy life. It has taken me 5 yrs to work out the best ways to help my self return to normal health state. I now follow a diet that includes fermented foods which have helped return my microbiome to a good state. All the antibiotics and toxic drugs destroyed my gut. My advice to you is be proactive about your after treatment health. It is as important as you treatment. Do your research about gut health. It wouldn't have taken me 5 yrs to get to where I am if I had been told this straight after my treatment finished. I am now 66 I have taken control of my health, I eat lots of fermented foods, kefir, sauerkraut, youghurt made at home high in benificial microbes . I exercise multiple times a week and I have a supportive husband, family and friends by the way I'm in Australia ,, Good luck with your treatment I know you will be ok.

@@darrelleporteous2780 Hi Darrelle, Thanks for taking the time to reach out and give me your own experiences, i am sorry to hear you found the after care treatment inadequate, in your case, as surely that's the most important part after SCT, we all need to be monitored, i know gut health is an exptremely important factor in keeping a suitable bio balance, i have a great team supprt team around me, said to be one of the best private facilities currently in London, I have heard the SCT is the toughest, and the biggest low you could even imagine, i will be in Hospital for 3 weeks in April, once my stem cell has been harvested and transplanted, i am under no illusion what's ahead of me, but know with the love of my wife,daughter, family & friends, i have the best support network available, outside of the most amazing staff who care for me currently every 3 weeks, my current treatment consisting of R-Chop and CYTARabine, which is administered and alternated every 3 weeks, ( 2 sessions) per day, with a 12 hour gap between another (2 sessions), i have been having my first sessions at 18:00 hrs, followed by next next session at 06:00 hrs, i am then left for another 12 hours, before my last 2 session will begin, again at 18:00hrs and again at 06:00hrs. This is done as a impatient over 3 day hospital stay. I don't appear to be suffering any noticable side effects, to either R-Chop or CYTARabine, i did have suffer a small infection, which was diagnosed as pleurisy and strong antibiotics, resovled the issue. I am a very positive guy, and this is just a mild set back, i am hopefull to pull through this and come out the other side, Wishing you all best for the future, Darren (Macmad)

@macmad6959 My father died of mantle cell lymphoma😭 the doctors here in Pakistan couldn't do much about it, he was 45 and got very week and within 9months after diagnosis he died and his spleen also got enlarged. I wish we could save him but all wishes to you get well soon

All I can think of is becoming a good doctor and provide early diagnosis plus accurate treatment, I wish our medical system here in pakistan to be this much effecient like in other developed countries...

The Nordic protocol worked for me. Alternating rounds of high does R-CHOP and in hospital high dose of cytarabine. Now I’m on Rituximab maintenance which is chemo but my hair doesn’t fall out. The maintenance regime is like an insurance policy. I’m three and a half years on from diagnosis and I’m OK.

So happy to hear good news. My father diagnosed with MCL last week. The chemo plan is 3 cycle R-CHOP and 3 cycle R-DHAP and the otolog ctem cell trans. Then rtx maintance therapy. Can you givi more detail about your journey? Will be very appreciated

I've been in "complete remission" for four years now. I gather that the outcome depends on the nature of the mutations in the MCL cells. It looks like one group of patients will relapse within two years and the other group will go for up to ten years or more. The good news is that when I do relapse, there are drugs like Ibrutinab that "pushes the pause button" on the MCL cell division process. And now there is CAR-T therapy that might even be "curative." CAR-T has not been in use long enough to say just how long it is effective.

Amazing explanation. I was diagnosed with MCL in 2016 at 46 Went into remission in 2017 at 47 and relapsed in 2019 at 49 Started my 2nd treatment today of lenalidomide pill and start rituximab in 2weeks for 5weeks

I have Mantle Cell Lymphoma non-hodgkin's & was diagnosed in Stage four in 2011. I was 50 years old & a woman. Did all the chemo & radiation but the cancer was too strong & within a couple weeks symptoms returned with swollen lymph nodes & tumors on my arms & legs. I was offered a stem cell transplant & so far I'm still here in Jan 2020, almost eight years later! They tried to gather my own healthy stem cells because there's no risk of them being rejected but I didn't have any. The next best thing is collecting them from your blood family. I have three sisters from the same mom & dad but they didn't match me. They found a German woman living in Germany that matched mine! I'll be 60 in April. I hope I've gave you some hope. Only God knows when it's your time to leave this world. Hang on. Don't give up! Sending prayers up!

Have they offered you a stem cell transplant? I'm not surprised at all that your MCL came back so fast. Chemo & radiation won't cure it. It just keeps it at bay for a short time. They gave me every chemo meds available & even some experimental drugs. I went to the Mayo Clinic. They gave me all the chemo my body could handle! Try a stem cell transplant if you haven't already. I’m only at the beginning of this video but I’m a 28 year survivor of mantle cell lymphoma I am a female and I was 38 years old at the time of onset but I’ve been re-diagnosed with a new cancer but I haven’t received the results yet but it’s in a different place so I don’t know what is going on as of this moment…..

I am confused about the cycles For example If patient will be treated with RCHOP RDHAP protocol 6 cycles every cycle is 21 days , is that mean that the total duration of treatment is 126 days? approximately 4 months? Am I right!

Why do you need to do a bone marrow biopsy if there is cytopenia again? (is it just to rule out leukemia?)

Because the cause of the cytopenia is usually bone marrow disease. The question is “which one”?! This help us make an accurate diagnosis on which treatment depends.

Thanks for information!! I diagnosed with non Hodgkin Lymphoma b cell consist with mental cell i start getting chemotherapy R maxi chop and Retuximab and next cycle cytarabine and retuximab after 4 cycle pet scan was negative but i complete 6 cycle now dr suggest me to do autologous bone marrow, i want your suggestions for bone marrow its must needed ?

It’s not a must, but it can be of huge benefit depending on your situation...I can’t give you any info about that...I am not an expert on that issue...Talk to your doctor, try to get a piece of paper and write down your pros and cons of it, and hopefully you will see the big picture and can make a better decision...You can always get a second opinion. Wish you the best!

My dad was diagnosed with diffuse large b cell lymphoma and mantle cell lymphoma about 2 months ago. He's been fortunate enough to be able to take his chemo at home although he still has to go to ohio state james cancer hospital to get chemo shots in his spine. I pray that they get yours into remission. It takes a very strong person to be able to go thru this.

Yes do the bone marrow transplant. Chemo drugs & radiation only last between doses but won't cure it. I have MCL non-hodgkin's. When they found mine I was in stage four. After all the chemo & radiation my body could stand they gave me a stem cell transplant. That was almost eight years ago. Btw, I was 51 when diagnosed & a woman. Many blessings to you.

The autologous stem cell transplant is like dropping an H-bomb on your immune system. The drugs they give you kill everything and the infusion of your own bone marrow cells rescues you. It’s the way to go if you don’t have co-morbidities like maybe diabetes or whatever.

My father has been diagnosed MCL . the doctor says it is in stage 3 after PET scan and he started chemo therapy. My question is is it curable in its stage 3?

I was in Stage four MCL non-hodgkin's & had a stem cell transplant. That was eight years ago. I'm a woman. That would depend on how old & how healthy he is too.

Stage 1 or 4 doesn’t matter. MCL is not like a solid cell cancer that can be surgically removed if you identify it early.

It has been told by the doctors that i may be suffering with side effects such as vomiting, hair falling, fever, digestive problems etc...during chemotherapy for mamtle cell lymphoma. I finished 3 cycles. I didnt have any of the side effect. What does it mean? Is it positive or negative?
