I live in Saskatchewan, the first province in Canada to implement a single-payer medicare for all system. When I see a doctor -- by appointment, dropping into the walk-in clinic or at emergency -- all I have to do is present my health card to the receptionist and confirm my address and phone number. I get examined, treated, referred -- whatever necessary -- and then the doctor or hospital sends the government a bill based on the agreed upon schedule of payments for procedures for prompt payment. One person can take care of all the billing for a practice of 8 doctors here. It would take 3 or 4 in the US, because they're dealing with different insurers, several different sets of codes, and a gamut of people at the insurers trying to find every excuse not to pay or insisting on a cheaper treatment. And here in Canada, drug companies, hospitals and doctors aren't allowed to waste money advertising directly to potential patients. Nor do we have people getting sicker from stress or trying to heal themselves with snake oil.

Which means people are dying of pneumonia, not covid19, on paper. The U.S. mortality rate of covid19 is hovering around 10%, whereas the international rate is 2-3%. That means a whole lot of people in the U.S. are positive/ aren't being tested.
