Doc , i am a dental surgeon practising few year i would like to know if these upper thirds in some older patients with dense bone splayed roots and gross decayed crowns is it more effecient to rise a flap and remove some buccal bone to make the procedure simple ....i would also like to ask if acess permits is it okay to make a small trough in the distal aspect so that extraction becomes easy

Yes, some amount of buccal bone can be removed to make the procedure easier.

Yes lidocaine gel can be used before injection to nullify the needle pain.

Covering eyes and face is a part of painting and draping procedure for any surgery. It is important to keep talking to the patient to make sure he is fine. Also, i always ask my patient to raise his/her left hand if they are not feeling well.

Yes. As long as the upper third molar is not going to be used as a pontic, it is advisable to be removed. If not extracted, the upper third molar will supra-erupt and impinge on the soft tissue of the mandible leading to ulceration and pain.

When I perform non surgical (closed) extraction, I prefer standing position. For surgical, I prefer sitting position.

Please don't extract your healthy teeth. They extract teeth only because to create space how funny it is. I wish I knew the consequences before extraction. My face is completely destroyed by these things.

I disagree with the claims you make. We do not extract teeth "just to create space". The aim of every dentist is preserving the natural teeth. We extract teeth only when we deem it necessary. Can you give me the details of how your face got "destroyed" by a dental extraction???

@@drzibranomfsevery friend and relatives of mine have said that my face looks ugly after this treatment. Even I can notice that as i Know my face Better than anyone. I won't get my good looking face back by arguing with you. I want to save few people from becoming victim for this treatment.

Again, I disagree. You can't claim such things without any clear evidence. In my 10 years of surgical experience, I've never seen such a thing in any of my patient. In most of my surgical videos, I even add picture of my patient after the surgery. None of them have such complication.
