You had swapped out your shield for a small one in the last part. Damage reduction is a must when engaging in close-range combat. Your build currently relies on Elusive for durability but that really needs continuous movement and movement speed bonuses to be effective. Thankfully, it will feel 1000% different once you invest points in the skill: "The Old Ways". This will give you enough durability to get in close without getting pummeled too badly. You replaced your shield during one of the cuts, but keep an eye out for shields that have "Absorb" or "Adaptive" mods. Those are tanky. Typically, past level 21 you should have a shield that is 700+. Other ways to pump up survivability are: 1. Investing in Constitution for more max health and ward. 2. Using dark magic spells and weapons. This will send streams of healing back to you. 3. Always leave at least one weak enemy alive for the Death Save. Don't hesitate to tactically retreat to cover if that is advantageous to your situation. That goblin boss from the obelisk had a nasty shock AoE that is easier to avoid (or block with cover) from long range. It will chase you, so you can lead it to an area where you can break line of sight easily to avoid its attacks.
