Medway CCG is leading the way with many primary care initiatives but ensure they are clinically-led. We are supporting patients with mental health needs in the community by having a mental health nurse integrated within general practice.
Having a mental health nurse in GP practices saw a 26% increase in annual reviews for patients with dementia, and a 46% increase in physical health checks for patients with Serious Mental Illness.
