"You look your best when you're up and dressed"... Meet our Therapy Discharge to Assess team - Steph, Nicola, Sandra, Ameera, and Laura!
When a person is an inpatient in Rotherham Hospital, they will be assessed and treated medically. When they are identified as being ‘community ready’, they can be referred to the Therapy Discharge To Assess team, so the patient can be discharged, and assessed at home.
This is a true patient centred care journey and recognises that each journey is individualised to the patient. The option of being assessed at home allows us to adopt the ‘home first’ approach, meeting an individual’s needs in an environment that is familiar to them.
Do you have a loved one staying in hospital that you think would benefit from this pathway? Why not mention the referral service to the Nurse in Charge or Consultant on your next visit, so they can assess whether it would be appropriate!
