Health and well-being begin at home. Tommie lived in a boarding house where his resources and support were limited. With assistance from Trillium’s Transitions to Community Living (TCL), Tommie got a single unit with the accommodations and the services he needed to live independently. TCL promotes recovery by providing long-term supportive housing, community-based services, supported employment, and community integration. With the right accommodations and support, home is the fertile ground where a healthy, fulfilled life can grow.
