The church of San Biagio, also called the temple of San Biagio for its monumentality, was built by Antonio da Sangallo the Elder, who had as an immediate model the basilica of Santa Maria delle Carceri in Prato, designed a generation earlier by his brother Giuliano from Sangallo. An example of 16th century Tuscan Renaissance architecture, it rises just outside the historic center of Montepulciano.
The origin of the church is linked to a miraculous event that took place on April 23, 1518, when two maidservants, Antilia and Camilla, and a peasant named Toto, passing in front of the fresco depicting the Madonna with Child and St. Francis, saw that the Virgin's eyes moved as if she were alive. Toto, of whom a statue remains in the sacristy, will be busy begging all his life to build the Church.
The church of San Biagio has a Greek cross plan and is covered by the dome, 13 meters in diameter, which was built between 1536 and 1544. Close to the back wall of the apse is the high altar, characterized by rich marble dossal, the work of Giannozzo and Lisandro di Pietro Albertini who built it in 1584. In the center, between two Corinthian columns, there is the fourteenth-century fresco, believed to be miraculous, depicting the Madonna enthroned with Child, known as the Madonna di San Biagio; on the sides there are four niches, each of which contains a marble statue of Ottaviano Lazzeri; the sculptures date back to 1617. The Temple is on the outside, as on the inside, entirely clad in travertine slabs that show an amber color. The stained glass window was commissioned, in 1550, to Michelangelo Urbani from Cortona. It depicts an allegory of the Conception of the Virgin, surrounded by Angels and Saints, while at her feet there are Adam and Eve who commit original sin. In front of the church is the harmonious building of the Rectory, with a double loggia, also designed by Sangallo and built around 1550. The well in front of the Rectory was built between 1550 and 1551 and originally had two columns with travertine lintel.
