@ariannao6441 oh my 😩 so close!!! And studying the whole book is key I learned I have read the whole book because many of my colleagues did the same and werent successful. I'm going over all her videos and lifelong videos and doing questions on the AACN and trying to currently find a good resource on the synergy model I am scheduled for the 30th. Good luck u will kill it next time! Study everything! ❤️😩

At 45:50, the slide says reverse Trendelenburg, but then said, "you certainly don't want to have the HOB elevated." I think I'm the thick-headed one here, but aren't those opposite? Or, maybe you mean place the patient in reverse Trendelenburg, but not extreme. Help! 😊 Even as an experienced nurse, I've really enjoyed your videos. Thank you!

No worries at all! Placing the patient in reverse trendelenburg would be most ideal for this patient as their body would be straight but their legs would be lower than their head, meaning that gravity would allow the stomach some extra room to breathe and wouldn’t increase intra abdominal pressure by being “pushed up” like it would if you were sitting in bed. If the patient was sitting upright in bed, their body would be folded at the hips, which would cause increased pressure buildup in the abdomen. Hope this makes sense! Thank you for your kind words ☺️

These answers and questions are straight forward, the questions at the back of the Barton’s pissed me off and had me screaming at them, scenarios with minimal info or vague eg elevation in lead 11,111,aVf,pt hypotensive with clear lungs, then they intentionally left out elevation in leads V3R /V4R,then said which Mi the pt is having. Listed inferior ,which I picked, and listed Right ventricular failure which was the answer! So,yes we can have a right ventricular failure after getting an inferior MI,but wouldn’t there also be changes in the V3R and V4R leads, why left those out of the scenario and only list symptoms as clear lung sounds and hypotension for right ventricular infarction, then went ahead and listed inferior infarction as one of the options,so obviously I chose inferior infarction which was wrong 🥵👹👹👹

I think i see the question you're referring to, is it question #150 in the second practice test? If it is --the one important part of that question that excludes inferior infarction being the correct answer is that the rest of that answer, "requires afterload reduction" is not correct. Afterload reduction means lowering SVR which means vasoDILATION, and that will open your blood vessels and drop the blood pressure even further!! So while you are correct, it can be an inferior infarction, the treatment will not include afterload reduction! You're totally on the right track! I hope that helps :)

@@NurseLifeAcademy no it was not that question,it was the one to identify which MI and the answer was Right ventricular infarction and not inferior MI,but they intentionally left out V3R and V4R elevation and listed elevation in 1 avl in the answer option, then put inferior Mi as one option and the answer end up being Right vent failure.Ill find the exact question for you. Thank you for all you do

@@NurseLifeAcademy yes you are right,I went back and look,it was question 150,and yes after load reduction would further compromise an already low BP,thank you!

No worries, you've got it! Not every answer will be textbook but they're really not trying to trick you, I promise! They want to see if you truly understand the critical care topics and can make appropriate clinical connections ☺️
