My MRI and X-ray were normal, so what is causing my pain? The answer is often seen in motion imaging

I was diagnosed with ankylosing spondylitis 20 years. Positive HLA-B27. My X-rays when I was 30 years old (20 years ago) showed calcification/ degeneration in the right SI joint. I am now 50 and in severe pain in my hips/SI joints and spine.i just had X-rays of hips/spine etc. and they told me my SI joint looks normal. 😳 What??!! How? I am beside myself because I feel awful😢

Thanks for sharing Mhizz Lee- We can definitely appreciate your frustration, and that's why we made the video. It's really common that all hope is put into an MRI or CT to find something, but it ultimately didn't lead to answers as to what treatment would be ideal. If you would like our team to potentially see you here, we'd be happy to help. We wish you all the best in healing!

Can a person have their case reviewed by your team from a distance if I'm able to acquire whatever tests are needed? How much does the review cost? Or do they need to be seen in person? I live in upstate NY and have been dealing with a crazy amount of the symptoms (for over 15 years) without any relief or found cause despite excessive medications and various treatments. I now see that majority of my symptoms are related to this diagnosis... and most others probably are due to having incorrect diagnoses for so long. Problem is... I'm at a point where I'm not sure I trust any doctors in my area to properly diagnose/treat. But I maybe if I get an official diagnosis & info about treatment recommendations for my specific case... I can get my insurance to cover the treatment with someone local. I'd love to have treatment done with your team... but sadly I don't even know if I can afford one trip (for a review) -- let alone many trips for treatment. Either way, your videos have given me hope... after being made to feel like I'm crazy for so long. Any insight is greatly appreciated.

If you could give me an answer you would help me tremendously. I injured my leg months ago, I couldent walk without experiencing severe amounts of pain. It hurt so bad I ended up with minor injuries in multiple other joints from Hauling this leg around. It’s been about 4 months now of basically no movement and everytime I start walking on this leg I get like a day in and I can’t walk anymore. The most amount of steps ive taken in months is 2000 in a day, that was today and the front of my knee feels like it got hit with a baseball bat. The terrible part is that my doctor got me an MRI at a cancer center out here, and it came up with nothing. Which actually made me feel even more hopeless cause I’d rather deal with surgery too get rid of this pain. I can’t work I can’t cook, I can barely get up and go too the bathroom and take a bath. It hurts so bad sometimes I fall asleep cause my body can’t take the pain. My doctor makes it out as atrophy, but I just can’t see this being normal at all. I’m atrophied forsure, I can’t squat at all, but the front of my knee is the only place I experience pain on my knee. Please if you could give me some form of hope that I’ll be able too walk again it would help so much. I’m so hopeless right now I really don’t know what too do anymore

I’ve had a cadaver cartilage graft for my articulate cartilage on the lateral side of my knee 6 years ago. I’m only 28 and I’m starting to have pretty significant arthritis on that side of the knee. I’ve had MRIs show close to full-thickness loss of cartilage. 1. Can prolotherapy help my condition (which is due to trauma, as opposed to degeneration)? 2. Can prolotherapy help heal cadaver tissue? I’ve heard it’s not as helpful of it’s not your own tissue.
@CaringmedicalProlotherapy
@CaringmedicalProlotherapy 3 года назад
Hi Bobby, Great questions and glad you are looking into the possibilities of regenerative therapies to help you avoid further surgery. Prolotherapy is used in both cases of degeneration over long periods of time as well as traumatic injuries. While the initial injury prior to surgery may have been traumatic, continuing to have pain and joint/cartilage breakdown would indicate that the joint is in a degenerative state. After knee surgery, the joint is more unstable, and therefore accelerated degeneration of the tissue continues, even after a cadaver or other implant. This is why so many patients often end up doing surgery after surgery yet still have pain and are unable to return to full function. We see patients post-knee surgery in our office very frequently and help many of them avoid additional surgeries, including replacement. Prolotherapy is not to treat the cadaver tissue, rather to treat your own ligament attachments that need to be strong in order to stabilize the knee and slow the aggressive degeneration that occurs with joint instability. We would ultimately need to review your case in more detail and see you in person to examine your knee under ultrasound and other physical exams and watch you walk, bend, etc, as well as discuss your current activities and athletic goals going forward. We'd be happy to help! You're welcome to reach out to us about the details regarding your case anytime. www.caringmedical.com/conditions/prolotherapy-knee-pain/#get-help-now There are also a lot of articles on our site that go into much greater detail about Prolotherapy for post-surgical knee pain that you may find helpful. Wish you all the best in healing, Bobby. Take care.

Here in the uk knee mri are carried out laying down, it’s the height of stupidity as it needs to be done under load, I question this and was fed a load of bs, I work on the railways for 30 years and know the track can look perfect when not under load but actually be very unstable when a train passes over, obvious to me but Doctor here are a joke.
@CaringmedicalProlotherapy
@CaringmedicalProlotherapy Год назад
Stevie- that is a perfect analogy with the railway track. Thanks for sharing that. It is the same here in the US- MRIs are almost always performed while the patient is laying down and then left with no answers as to why they have pain. There are a few specialists who will do motion imaging, and we are one of those places. Thus, many patients finally get the answers they were searching for and can lead to more understandable treatment options that are trying to get to the root cause of the problem. Wish you all the best in finding a great doctor close by. If you ever travel to the US, we'd be happy to help you here. We treat patients from the UK, although we know it is not feasible for everyone to travel for medical care. Thanks again for sharing that great analogy.

Like how in the video you guys explain that motion imaging may catch the actual pain source. Might it be possible for motion imaging to capture a damaged muscle? Years ago, I strained myself to the point where I immediately thought I tore the top row of my abdominal muscles, but all of the tests have shown no indication of a tear. 2 ct scans, ultrasound(laying completely still) blood work and an mri.

I just had my x rays get back got injured in February 2nd of this most recent year for a lower back injury bending forward lifting well over 100 lbs and twisting g repetitively doing that lotion lord then 35 times. I weight 105 so I was lifting well over body weight. Been through physical therapy and that made it worse. I got diagnosed for tronchantric bilateral bursistis which is affecting my hips at night and walking. It’s hard to walk up stairs I can’t stand for longer then 30 minutes without needing to change my position of my spine because it hurts. I don’t get good sleep. I have memory problems. My balence is off and I’m very weak all the time. I have to walk up stairs and that is super painful to do and hard - often I feel the other hips compensating the other. I’m not working because I can’t maintain a normal job because of pain. I get back spasms I’ve had an MRI that showed degenerative discs and bulging discs that they told me were due to age ? And mild facet hypertrophy. I sleep every night and can’t help my body to not turn on its sides and man do each of my hips hurt like really bad when I lay of them. Just had an x ray come back and it was normal ? Which upsets me because I deal with this existent pain every night. I’m wondering if it might be possible to get your medical team to evaluate my information or look. I have Kaiser so they should be the best at finding out what is going on but so far I’ve been sent to acupuncture asked to get injections on my lower back which I haven’t done because the needles are long. I’ve gotten injections to both my hips though because of severe pain. It’s hard to walk I’m on the waiting list for disability So any professional medical advice you may think or add for me may help me in this case. I can’t dance anymore like I use to or stand for a long time go hiking on uneven hills and not care Or go to the gym with my husband. I’d really love somebody who knows and CARES what they are doing to help. Thanks for sharing such a informative video

Unfortunately if only it was that easy to say pain can be seen on an MRI.. However the research doesn't support that claim at all. Pain is multivariate and dictated by physical, psychological & environmental factors to name just a few. Yet mechanical abnormalities are seen in nearly every MRI - for patients with pain or without pain. To that point 75% of athletes over 18 that play football have hip cartilage tears with no pain. To say that they need motion imaging is outdated and basically says everyone pain is mechanical by nature. Which is false.

In this case, how would pain that isn't being seen on imaging be explained? What can we do when we are told there's nothing showing that explains our pain/injury?

Hi Robert, The closest option is Candlewood Suites which is right behind the office. A lot of patients stay there because they have kitchenettes and within walking distance. May be something to consider.
