Question number 14 CXR shows that the device is an AICD. In AICD lead two thick portions are seen so called DC shock coils which are clearly seen in CXR

Can anyone please explain why we can give ace inhibitors in u/l RAS ? As it will cause decrease in gfr

Normally there is vasoconstriction of efferent arteriole but Ace inhibitors cause vasodilation of efferent arteriole that's why ace inhibitors increase gfr in unilateral ras...

@@amantayal5998 but vasodilation of efferent will cause decrease in gfr , not increase in gfr.. vasodilation of afferent will cause increase in gfr

@@n.s.studytime i agree vasodilation of efferent arteriole cause decrease in gfr but in ras there is too much decrease in gfr beacuse blood is not going towards parenchyma of kidney...so there is only one method to increase in gfr of ras PATIENT to vasodilation of efferent arteriole... Bro do comparison .. between the ras patient and ace inhibitor use in ras patient..

U/L ras..main problem is hyper-reninemic hypertension.. hypertension is due to increased renin only..no other way to treat this htn other than ACE OR ARB If you compare it with b/l ras..there is no pressure natiuresis.. leading to increased salt and water retention ultimately blocking renin release. ace contraindicated there..to prevent renal shutdown.
