It is for safety. I am assuming the indoor-town style is a memory care that is locked from residents exiting to avoid them from wandering out of the facility (the reason why locked designated memory care locations exist). They likely have their own reserved outdoor space, but the indoor set up is a unique way to avoid the clinical/hotel feel of most memory care locations. I think it's a great set up!
