Great video. I am an AP in an end of life care team in the community and due to start my adult nursing in September. There is not enough health care professionals who know how to fill these forms in and we get so many unsafe discharges. We also get so many patients delayed in hospital, whose preferred place of death is home and sadly they die in hospital because the forms weren't filled out correctly and without the relevant information needed. We work quite closely with CHC and know how tight things have to be but it is so important in end of life care to get things right and sadly its not always happening. I think if the education was there for health care professionals around this topic it might go a long way in solving this. I will definitely be sharing this with my colleagues.

Tee Kruger I think there needs to wider training on this maybe. So that every trust and healthcare professional is more aware about it all. It’s such a shame that end of life patients should go through that. Their forms should be top priority/ fast tracked through to CHC to have everything put into place for them too 💔💙 hopefully the message gets around to as many people as possible :) Thank you so much. Your role must be amazing! Very emotional but so rewarding I think. Xx

My Mother with Vascular Dementia and is deaf was hospitalised with Sepsis and Pneumonia. She was released to me after one day at the war instead of them monitoring her medication. She came home and refused to take medication so phoned the Social Services. Now I can't take her out due to her needs. The home she left and that I've lived in for 20 years will be used for the cost of care. I'm livid.

It's all one big Rip Off! Carer turns up to a house across from me. Carer sits in her car for 15 to 45 minutes and then goes in to see patient. After 10 to 15 minutes she is out and gone. About 4 hours later she comes back and does exactly the same again And the patient pays for this diabolical treatment.

It's vital to understand that in many cases the CHC NA doesn't apply the regulations and CHC isn't deemed eligible unlawfully.

Thank so much Claire for taking the time to highlight this issue! this video is a real eye opener for me As a student nurse, I have learned a great deal of things I didn't know about package of care especially the importance of getting it right from the outset to avoid all the implications. Nurses need to be educated more on this.

Thank you, this video is of great help. Discharge team issues…I can’t begin to explain my experience with them. They may as well talk a different language.

It's so frustrating :( and such a shame for patients when they get wrongly discharged hom and a hospital re-admission as a result.

Leon Matthews it depends what you want to do? :) both very different fields! Maybe go out for a day with the paramedics near you? And see what it all entails? X

I’m in my 20,s ventilator dependent & oxygen plus hoist & electric wheelchair user and type 1 diabetes just to mention a few… I’m stuck in my local hospital waiting for CHC 😢😢😢😢so depressed

I am a CHC assessor , we don’t assess people for NHS CHC funding while they’re in hospital. You are discussing discharge planning , (not CHC assessments)
