Hi ,one doubt ,do we need to verbalise all points in A-E assessment eg ., in breathing your breath is normal ,you are not using your accessory muscles etc., like that or simply take the vitals and mark it down and ask about ADLs question right?

Yes, you have to verbalise in full for A to E.

Mam they have already mentioned some recommendations..do we need to verbalize that based on the score of the patient

Hii. Do we have to ask each and every question of PHQ to patient .If we skip any of them will be failed or?

Your briefing will be that questions have been already asked and scored, you need to add the score and address how will you support your patient regarding this.

Hello mum, are there possibilities that the phq - 9 score isn't filled on the assessment day and you are supposed to assess and fill it before calculating the total score?
