I don't think your positioning was at fault, the players ran across your line of sight and coincided in position at the exact moment the tackle was made, there was little you could have done about that - but you did make an important decision about something you didn't actually see, possibly because you constructed it in your mind as what probably happened. This was one of those occasion where a self video referral could have been used - that said the player penalised just accepted the penalty, which in the circumstances was quite odd but gave you no reason to doubt your decision at the time.

Yep, a penalty stroke awarded for nothing and yet, in the last incident on the video, no penalty at all when a penalty stroke should have been awarded.
