Because the footage you see is only of pyongyang, and it's what they want you to see. The rare footage is the forbidden side of north korea very few films show...the starving people being tortured in labor camps.

They may not have a choice. These things are staged and planned. Not to mention that all tourists will have to go with a guide that will take you to places that you will have to go to and you cannot go anywhere else.

"All those people are actors" Yes and no, they are the most loyal to the North Korean Government and so they get benefits like a modern apartment with modern technology and the freedom to travel outside of North Korea if they choose along with getting cash.
