Hello Jay... I want to ask whether 5 minutes allotted for case notes study is just the time we can spent or even more, irrespective of the test taken on paper or computer??? Because the total time is 45 minutes... and I am like the time spent on case notes study, organise and transform take around 15minutes and so remaining 30minutes can be spent on writing. Thank you Jay ! l have 2 questions though , - Is diet a relevant data for the nurse who already knows Jane ? -If family history was given , would it be relevant to both physiotherapists or the one who knows Jane can remember that ?
Dina Khamis Hi, if the receiver already handled the same patient, might as well he / she already knows the personal info. If its a new info, then include it if its necessary and relevant to the patients new diagnosis or medical condition.
I'm completely confused with the example which you have given in the last regarding the aids which is required to her 1 extra hour a week... Whether to think different if you get these kind of questions and take risk or just write the letter as per task given as completely normal which you have explained earlier?

Somehow, I feel like this effort of selecting the appropriate case notes is scenario dependent, as you change the scenario, you actually change the way of the writing, hence, we might not be able to make a consistent plan with every scenario in order to attempt it.
As a nurse I should know everything about patient under name (nursing process). Assessment content history that is mean everything about the history of the patient ,some how it difficult to exclude part of the history (irrelevant). how is that ? Thanks for lesson.
i know this is way late but correc my comment,,,isnt it should depend on the task whether the case is relevant or not? not if the nurse or perdon will receive knows the patient though thee a little connection though hehehehe..
