It works best DIRECTLY ON THE EYEBALL..so yu hod the ip wide pen before gently rubbing a thick cating of vicks into the pen eye ball. Then let go so can cose their eye abd ceanse THE BACK OF THEIR EYELIDS as well..shuld be washing the back of eye ids with thick suds of soa eveey bath and let soak few minutes before rinse at least 5 days a week any way.
