Otosclerosis and Otospongiosis are caused by bony formations around the Stapes middle ear bone that connects to the cochlea. This impedes the conduction of sound from the eardrum to the cochlea. This typically results in a Conductive Hearing Loss.
Approximately 60% of cases are genetic. It occurs in females twice as often as in males, and in Caucasians more than any other racial group. The onset of Otosclerosis and Otospongiosis is usually around ages 15-45.
Otosclerosis and Otospongiosis are generally diagnosed by a hearing evaluation which indicates a conductive hearing loss, abnormal tympanometry, and often absent reflexes. Once referred to an ENT physician, a CT scan may be completed to confirm the diagnosis.
There are two typical treatment options for these conditions, Amplification with hearing aids and Surgery. Depending on the severity, surgery may be recommended to correct the conductive hearing loss.
The two surgical procedures are a Stapedectomy which involves removing the Stapes and the Stapes footplate and replacing them with a prosthetic, or a Stapedotomy which involves removing the Stapes and drilling a hole into the Stapes footplate and replacing it with a prosthetic.
If the surgical procedures return hearing thresholds to normal, no additional treatment is needed. However, if there is still a hearing loss after surgery, hearing aids may still be needed.
Overall, Otosclerosis and Otospongiosis are very treatable. So if you are on the younger side and your hearing has declined, make sure you find an audiologist to get your hearing tested. The sooner you identify the problem, the sooner you can fix it.

I was diagnosed with Otosclerosis four years ago. My ear started ringing one night and it didn't disappear after a couple of seconds like it normally did. I went to an Audiologist and they noticed the ear drum wasn't responding properly and that's when I was referred to an ENT who had me do a CT Scan which confirmed Otosclerosis. My hearing has been stable these last four years, the hearing loss I have was deemed to be mild according to the hearing test. The hearing loss wasn't severe enough to warrant an operation so they gave me hearing aids and we're doing a "wait and see" approach. To those suffering with tinnitus, it does get better as habituation takes hold. I rarely use my hearing aids as I've become fairly accustomed to the ringing and don't notice it most day.

Similar situation here! Only my right ear is affected and diagnosed since 2020. I hear less in my right ear. Tinnitus is annoying but I am used to it now. Main problem would be listening to people speak in crowded area. It isn’t really difficult, but if they are speaking softly it is.

Thank you very much for this informative short video. Can the otospongiosis make you go deaf? Do you have to do the surgery as soon as you're diagnosed with it? Is it reversible?! My mother had surgeries for both ears 10 years ago and now she has a hearing loss on the left ear and doctors said she has to do the surgery again!

I have the Oticon Opn1 and they are working great for me. I have an MRI scheduled for Oct 3rd. So many possible side effects with this surgery and they only do one ear at a time. It’s terrifying! This is a great video explaining everything. Not a lot of good info around about it. Thanks for making it.

My father(60) had a problem of water leakage from the ears for a long but he just used to take home remedies to heal that. But today when he started feeling pain inside his ears we took him to the doctor, he said that there is a damage in the bones of his ears belive me sir this news left us with deep sorrow my mother got extremely emotional.😭 Sir, Does this bone break problem have any solution? I just had my surgery one week ago. I feel the tinnitus became very loud, especially when everything is quiet, my tongue taste like metal. I have dizziness which prevent me from driving, even walking where there is a car's noise. After my research, it shows that i should feel the difference after 3 weeks or more, hopefully my hearing get better. I feel some strange sensations on left side of my head. At sleeping time, I feel some liquid inside of my ear (mid ear) moving back and forth when I move my head. Every sound even the normal ones sounds very loud for me and make me nervous. My walking is very slow because I try to find a balance. So, this is all about my First Week After Surgery. I will let you know how I will feel the second week. Inchallah it will be much better. please Doctor after reading this, let me feed back and tell me if this feelings are normal or no ? May God heal every single person who struggle from sickness. amine.

Hi Billy. Others have these reported side effects following surgery. You need to give your body time to heal, but most of these cases have positive outcomes.

I had this to. Is very normal in the 3/6 months you Will ear the sounds very loud. Then with time you Will recovery. Remember that our ear is very sensitive it controls your balance. So try not to do fast movements 😊 I had mine in 2012 and was a sucess a gain Almost everthing.

I had this surgery almost two years ago. What you're describing is NORMAL. Most of it should improve. For me, I still have a strange sensation on my tongue, but I've gotten used to it. I have occasional tinnitus, but again I've gotten used to it. Overall, I'm very glad I had the surgery. Thinking about having the other ear done soon. It's crazy how little I can hear with what I once considered my "good" ear!

@@widM_ it's something I had my whole life in my left ear, but I didn't realize it wasn't normal. I just thought I was "right-eared" when I was a kid. By the time I got it diagnosed, it was not great, then it took me about 5 or 6 years to muster the courage to get the surgery, so the hearing got worse in that time as well

There are always risks associated with any surgery, but these surgeries are generally lower risk. I can't blame you for wanting to avoid it though.

Always tell them you want medication for nausea direct after surgery (before waking up) . Especially for this surgery. First surgery more issues. 2nd just over a week ago only slightly dizzyness when moving my head to fast. Minor pain. Mostly the cut and sometimes a bit earpain. Simply medication is enough.

CANNOT thank you enough doctor. After 7 months I know now that my dizziness is caused by the existing otosclerosis because it is connected with the inner ear. Waiting time here in Ireland is 2 years and I am constantly dizzy. Any cure until they call me for stapedectomy! Please do answer. Thanks

@@sumeradavid8458 I am 50 now and I was cured for dizziness by a physio through some balancing exercises few days after this comment. The name of disease I cannot remember but it was the imbalance due to sinusitis. It gets better until I accidently move my eyes to the extreme corner and the dizziness comes back. Again I am given exercises. I suspect this is incurable disease .

I was diagnosed with Otosclerosis 6 years ago. That’s on top of the sensorineural HL I already had, in same ear. This ended up with mixed HL in that eat. Otoneurologist refused to do the surgery, because of my age. I am now 67 years old. He thought that surgery was too risky for someone over age 60. Do you agree? I am a speech pathologist. I wear bilateral hearing aids. The mixed HL has resulted in much frustration, because the hearing on that worse side, is still very poor, even with the hearing aid.

Hey Ronnie. It's hard to say. I've had several patients who didn't have the surgery due to the potential associate risks. I can't directly speak to the recommendation you received from the Otoneurologist, but it may be worth a second opinion.

50 years ago my wife, who is now 76, - she has bilateral otoselerosis, as her mother did - had a stapedectomy in one ear and it was a failure. She was told then never to try it on the other ear, which has slowly declined to the point where it now has 115 DB loss. Her latest hearing test: her audiologist recommended she inquire about cochlear implants and bone anchored hearing aids but she doesn't want to chance another surgery. Maybe in five years . . . any surgery is taking a chance.

I have had 2 surgeries and now left with nothing to attach..per my Doctor in UW Madison. Will a hearing aid work? I was told no in 2013, the last surgery caused meniere's. I went to the ENT for vertigo last week and he told me a hearing aid would help me now.. that they have improved.

I'm diagnosed with moderately severe mixed hearing loss, with a loss of 51dbhl left and 46 dbhl right. And also the neural paths have decreased . What are the options available for this because my doctor told that if there is nerve weakness involved surgery cannot be done.

@@mukeshp-k9t hi.. It's very normal to feel clogged after the surgery.. For me it took a week to start feeling the difference . Once they remove the cotton and other packings hearing will be back to normal

@@sumeradavid8458 33....the after surgery gonna be worse than the surgery itself.... depending on how you react to the anesthesia...i had it march 2nd and it's still healing but the hearing in my left ear is amazing...so far..worlds apart...I'll do my right ear after this

I was afraid of the reason that I might lose hearing altogether. Now the tinnitus seems to get louder and louder and hearing is declining too. What are the chances that tinnitus will go away if surgery is successful. I am hesitant about surgery as my CT scan came normal.

If a hearing loss gets corrected, there is a chance that the tinnitus subsides.

Not to contradict the doctor, but tinnitus is tricky. They don't really know what causes it. You may get lucky and have it improve, but that shouldn't be the main reason to have the surgery in my opinion. It ought to be that your hearing loss is severely impacting your quality of life. The hearing improvement is really great, but I still have tinnitus. Maybe when I get my second ear corrected, but I'm not counting on it.

Whine time of a serious note: I'm 49 and had a stapedectomy on my right ear that was at about 40% since I was a teenager- but no improvement. Between age 47 and 49 my left ear mysteriously diminished to about 40% and since I met my deductable I had that ear done also. So with what was supposed to be about an 80% chance of improved hearing you would think one of them would have taken but the left ear actually ended up worse. So if I was a gambling man I'd be pretty upset. Well despite not being a gambling man, that double failure just doesn't make sense to me. It's depressing and it's now been over a year since both surgeries so I can't keep waiting for them to heal more- they are what they are. What should I be doing at this point? I have Phonak hearing aids and Bose hearphones but is there anything else that can be done surgically for my situation? Losing your hearing stinks and I'm not saying that I'm young but I'm by no means in the target audience to have hearing aids. Thanks

@@DrCliffAuD My right was diagnosed as conductive. my left was diagnosed as conductive but after surgery and 6 months of follow ups it was changed to sensorineural. not sure about the final diagnosis of the right ear

This happened to me in my left ear after surgery it got worse. I had surgery when I was 21. I decided not to do my right ear because what was the point for me. Im now in my 50’s and can only hear if you scream into my ear. 😮 I have gone to several ENT’s and all say there’s nothing they can do.

Hello Doctor, greetings from India. I'm experiencing hearing loss from past 4 yrs. I can hear in calm place but can't hear in noisy place and even can't understand some words after asking twice or thrice that what they said. I had sinus prblm earlier and used spectacles for headache. Now my headache has gone. I cant hear when some one call me from behind. But I can purely understand when they talk in front of me. I had typhoid 3 times and chicken pox once. That was 10 yrs back Is this affecting my ear ? Or do I really have ear prblm Please let me know why I'm facing this prblm . I have Consulted many ENT but none of them telling what's hindering my hearing .

Yes, that symptom is why most people see an audiologist.

Dear doc....my mom has an unilateral hearing loss for nearly 13 years...her age is 50+...in 2017 we visited an ENT..He confirmed after tympanometry it is otosclerosis bt did not perform any MRI..my mom used to feel as if her ear was watery and water will drain out from her ear...she is also suffering from headaches in the back of head...is there something to worry about??...plzz inform me..

I can only really comment about the hearing loss aspect of Otosclerosis. Any issues with headaches should be consulted with her doctor.

Dear Doctor I am 69 years old having otosclerosis and acute hearing loss in my left year .I had a stapedectomy in the same year about 25 years ago which improved my hearing and enabled me to work actively .Now at 69 since I am quite healthy and active wanted to know if I can opt for another stapedectomy
