Sure, the modifier definitions found in the CPT appendix A : Modifier 59 should be used when coding for a different session, different procedure or surgery, different site or organ system, separate incision/excision, separate lesion (noncontiguous lesions in different anatomic regions of the same organ), or separate injury.

My question is concerning the 13-year-old with the left tibia fracture. The documentation states that the radiographs were ordered, performed and billed in the office already. @@tanushreeagrahari3224 These should be very simple because all you have to do is locate the code in the correct section: eg, New Office, Established Office, Consultation, etc, then find the code with the language that says Low Medical Decision-making.

@@mohammadhameed6459 a medically appropriate History and OR exam must also be documented. Make sure you read the guidelines for yourself, that way you will know it for yourself.
