Passenger dead and 15 in hospital after train collision in Wales 

One passenger has died and 15 others have been taken to hospital after two trains collided in Mid Wales on Monday evening.
The incident involved a service from Shrewsbury to Aberystwyth and a service from Machynlleth to Shrewsbury.
