Phantom pain may be the cause of the 'tight' feeling in my left foot right now. I certainly remember the 'phantom pain' from my stress fractures in my college years. Those phantom pains from stress fractures in college were ten years ago, and I frankly did forget about the sensation/feeling. Stress fractures are no joke for serious runners out there. Any bone injury for a runner can cause a lot of anxiety because you are forced to slow down and rest, an uncommon notion for most long-distance runners. At the end of the day, I think this may be a phantom pain that I am dealing with right now.

I also feel that as runners we are incredibly in tune with our bodies since we tend to put ourselves through "pain" all the time when we run, and we build up such a tolerance to it. What you feel might just be something that a non-runner wouldn't even notice and is like you said just a finalizing of the healing process.

Phantom! QD: I experienced PP for about a month after I resumed running, most notably on my first long run. I felt it, was definitely not the same type of pain, more like a diffuse pain that gently washed over the top of my foot. It caused me to stop and walk for a moment because it scared me, not because it hurt. I slowed down and finished the run -with no increase in pain, and it wasn't continual. This would happen about every other run for a while, the duration of that feeling became shorter and shorter and less frequent. I totally understand the PTSD. I don't think about it anymore, but it was a struggle there for a minute. Great Vlog today!

Jason Aberly you hit on things here! Eventually it goes/fades away from memory... once we get to the point where it doesn’t hurt we aren’t absorbed with it and we move on without realizing it at first.... then suddenly hey! It’s gone!

Seth, I've been watching your videos to follow your recovery process. Not a doctor, but a runner here sharing my experience. I had a bone bruise (Xray showed no fracture 3 weeks after injury) on my tibia at the bony protrusion near the knee (near the lateral condyle) from a slip and fall. I waited 8 weeks before staring to running again and still encountered a slight "twinge". At the end of it, it took about 12 weeks to be fully pain free and my injury was probably at a low load bearing site. Keep in mind that your body sees up to 3x your body weight during a running gait cycle. Since your injury is in your foot, you'll probably want to be conservative in timing your comeback. Keep monitoring your phantom pain and if the un-localized pain becomes more and more focal, back off it. Again, not a doctor, just sharing my experience with my recent bone bruise.

Thank you so much for the video!! It still helps runners after 4 years, wow! It has been three months after my cuboid stress fracture. As a newbie runner, it kinda sucks and destroys my confidence into running. And it’s soooo frustrating especially with this phantom pain during back to running. Love this video. Hope it can all go away soon 🙏💪

I had an overuse injury in high school related to football training, and even after I healed the pain in my foot persisted for months afterward. Our team doctor told me there was nothing wrong with me, and I got it x-rayed and everything. I'm convinced it was 100% a mental thing brought on not by the fear of re-injuring my foot, but by an intense desire to never play football again. I quit the team not too long after that. Sorry dad.

After my bought with severe IT Band syndrome, I had phantom pain regularly. I was afraid to do hard efforts or longer runs in fear of the return of the injury. Definitely bailed on some runs thinking I was re-injuring. Finally, the PT assured me enough that I just ran. Haven't looked back since! I continue my strength building exercises even today.

It is all about my first injury PF I did it one week before my first half marathon and I was thinking about it so much when I was preparing for same run year later that belive or not I start to feel same pain in same place exactly one week before same half marathon but one year after sound impossible but that true . Hope that make sense 😁. I mean some injury it is only in our heads. So head up Seth we are all with you .

Hi Seth, interesting video today! I had a stress fracture almost 2 years ago, and after removal of the plaster cast (I walked around with the plaster casted foot since it left my ankle free, and I could also cycle with it), I had no pain on the foot when I re-started the running. I had pain in the foot on the spot which I was pressuring to the plaster cast while walking. This was actually exactly on the spot where I am normally landing when I am running. When I am walking it's more the transition zone from heel to toe. It took a couple of weeks and then it improved but it was like it was bruised or something. Good luck with the cross training, I was thinking about it during my run today that I didn't see you swimming for some time, but there you are again! And of course I hope you won't have any pain or other issues while returning back to your running form from a couple of months ago!

The article you reference claims that there is a link between stress fractures and treadmill running because of impact forces hitting virtually the same areas all the time. Whether or not there are studies to back this claim up, I believe it, because when I experienced a stress reaction a while back, I was running high intensity intervals on a treadmill in Brooks Ghost 11's (high drop shoes). This leads to a question: where or how do you run in the winter if you don't want to run on a treadmill, and you also want to avoid running on pavement or frozen fields? Run in the snow? Start snowshoeing? Other? Could make for a show topic.

The stress fracture I got was most likely caused by running on a treadmill as well. I only use one, when there's no other option left. In winter I run also outside (but I am lucky while I am in the Netherlands ;) , with just a little bit of snow in winter, or none at all). When there's snow a trail shoe can help and when there's ice of ice under the snow it's possible to use the shoe's with metal studs, like the Icebug shoes for instance. Furthermore Seth posted some vlogs on using screws as 'anti slip' feature.

You will be just fine! Just take it easy because most of these injuries heal with about 08 to 10 weeks! So if you still are insecure with your foot I really recommend another image exam just to make sure everything is good! Best wishes

QD. So I have been "recovering "from a stress reaction in my tibia. I have recently started to get some miles going again in the early stages of training block for NYC in November. I too am wondering if I am dealing with the same thing. Not dealing with pain like I did when I was initially injured however like a dull feeling in my shin area. I also have been very nervous to reinjure my leg. So now hearing this is definitely a boost in my confidence that I am not crazy!

I don't want to get too technical, but there's nothing phantom about aches and pains we feel as runners. They are real LOL But I know what you mean. One day the top of your foot feels weird, the next day your left hip is a bit wacky or your lower back feels a bit tight. But magically they go away in a day or two all on its own. I'd love to know what actually causes this but I imagine it has to do with imbalances and/or weaknesses in our bodies. Another theory I've heard, especially when in taper mode, is that when we taper 'too much', meaning we drastically decrease the activities we have been doing for weeks on end, subconsciously the body feels like there is no more stress coming and thus begins to heal itself. Some of these healing can manifest in the form of swelling or inflammation. But who knows really...I'm nowhere near an expert but I can tell you, I too, have felt these phantom pains 😊

​@@SethJamesDeMoor I had a horrible experience with tapering the day before the LA Marathon in 2018. I had gone from averaging ~45 miles per week, down to 15 miles the week leading to the race. The night before the race, walking back from dinner, I could barely walk. The top of my right foot felt so bad it almost brought me down to tears. I put on some Tiger Balm, wrapped my foot for the night and the following woke up with no pain at all. It turned out to be the best marathon to that date. PR by 54 minutes from my previous LA Marathon, 2017 😁

I had Phantom pain after a 5th metatarsal break 5 years ago. I want so freaked out I made my doctor take xrays to prove that everything was actually okay

Hi Seth, have you tried massaging your foot with essential oils? I mentioned dear daughter Indi, with her medial amputation last week? We use frankincense (anti-inflammatory), rosemary and oregano oil (known as Bone knit in medieval England). Indi says it's soothing and reassuring. Indi also says, "Seth, stretch those hamstrings!" You guys are beautiful! X

I had a stress fracture/reaction in my right fibula in early August following a Half Marathon. I maintained my usual training load in the build up and didn't taper and also had an intense 30km trail event the week prior which pushed me over the edge. I have now returned to running after physio, walk/run routine and now easy runs. I feel an ever so slight tingle every now and again in the stress fracture site. I'm hoping this is just phantom pain, I am very cautious of re-injury. I have adapted my training schedule and now incorporate dynamic stretching and jog pre run and static stretching and light jog/walk post run so hopefully this will help. Hoping this is just phantom pain and I haven't started back to early..>!

I still feel pain during and after jogging or running.. and when I massage or stretch it calms down.. I'm scared to run... It's been months and almost a yr and I feel hopeless

I have had 5 stress fractures in my feet during my Highschool career. I usually feel the “phantom pains” when weather changes dramatically. Either a storm is coming summer or winter. Or if it’s very high pressure systems. You said that the weather got pretty warm when you started to feel the pain again in a video if I remember correctly so maybe you are experiencing the same sensation? That is my experience with that feeling and I hope that helps!

Glad you're doing well. Responding to QD: I had a hip/sacrum stress fracture (Oct 17), but after 2 months rest/rehap/x-training I got back into training for Boston. I did not feel any phantom pain at all. This is the first I ever heard about it. How do you tell the difference between phantom pain and real pain?

@@SethJamesDeMoor the initial injury was last year (very similar to yours in that there was no fracture found). I took a couple months off completely but the phantom pains lasted several months beyond that. Recently though, the same thing phantom signs have been happening (same area) as my volume is pretty high at the moment. That seems to be the trigger for me, so I just have to be mindful of it 🤷‍♂️ All the best man!

I might have shin splints. I don’t know though, because everyone says they’re really painful, but these are more just slightly annoying pressure-like pain coming from mid tib. 🤷🏻‍♂️ Oh well, I’ll just move to the trails for a bit to recover

i destroyed my foot about 5 months ago kicking a dog who was about to kill my kitty. That hurt more to me than the dog because then I couldn't walk at all for about 3 weeks. I suppose I broke several bones in my fingers. It was that terrible. But to be honest I didn't visit the doctor. I did submerge my foot in hot salty water on evenings everyday although. The next 2 months I tried several times running just to fail at 1km because of the pain, impossible. The last month I began to run again to the point where I was before (5-10km). I used that time to deadlift and doing simple plyometrics, nothing too hard, just enough to train legs muscles to recover stability The phantom pain is still there but it doesnt impede me to run or walk. It is in fact receding. I just make sure to warm up every part of my legs real good or else I just dont start until Im sure the blood is pumping and the muscles are ready. The point I think is that the only way to get over an injury is to get stronger

I was diagnosed with a Osteochondral stress reaction in my knee. I took 35 day’s off but not consecutive. Do you think another 25 consecutive day’s off are enough?

Maybe you should add yoga to your crosstraining regime(for stretching) man, you look tight ps sorry for my bad english pps monitoring some endocrinological parameters (especially testosteron) can make sense. Many people show a drop when they reach overtraining and then injuries are more likely if you have any questions, just ask (first point wasnt my point but from one of my staff with an orthopedic prosthetic education so it can take a day till i ll get an answer)
