In first eg of dandy walker syndrome, why there is no hydrocephalus. In eg with only inferior vermian hypoplasia, there is 4th ventricle dilatation n Blake’s pouch /posterior fossa cyst, there should be obstruction with hydrocephalus.

What if your diagnosis is MCM with a posterior fossa cyst thats 2.1cm X.1.1cm and causes severe headaches with intercrannial hypertension? Is that considered a nothing-burger? According to doctors it is, even though I'm losing my vision due to glaucoma. I'm 59 years old and have been suffering for over 30 years and would really like to find a Neurosurgeon who's familiar with my condition. I'm in Colorado. Thanks so much for the great video! Kat

Thank you. Cisterna magna is located between the cerebellum above and foramen magum below and back surface of medulla in front, and CSF from foramina Magendie and Luschka drains into it. So if I see enlargement of this CSF space just below the cerebellum and without mass effect, then mega cisterna magna. Arachnoid cyst is typically more posterior and can have some mass effect on cerebellum and vermis. If Arachnoid cyst is in the typical location of mega cisterna magna, then I think it's tough to tell. If I see sign of CSF obstruction, then it's more likely arachnoid cyst (or blake pouch cyst), since mega cisterna magna is not expected to cause obstruction of CSF.

What is the cause of hydrocephalus in dwm…???as i know the cause of hydrocephalus in Blake’s pouch cyst is persistent black pouch and foremen magendi is not formed………

Sir am a lady pregnant 24weeks my ultrasound came blake pouch cyst -skull-(small well defined an echoic cystic lesion in the retrocerebellar aspect measuring 10x10.2mm likely S/o Blake pouch cyst).ventricles are normal n neck abdence of masses eg.cydtic hygroma -normal...tell me sir😢what is d situation now am 6month running

Sorry but I'd defer any meaningful opinion to your physician. It would be irresponsible to make an assessment without a full clinical picture and images. Having said that, isolated Blake pouch cyst with normal vermis and no hydrocephalus or other significant findings often has a good outcome. Please refer to your clinician and with a follow up ultrasound.
