Today's Video:
The P-L-I-F is defined as Posterior Lumbar Interbody Fusion
This approach requires a 4-6” incision in the center of the back to access one or two levels.
Initial posterior fixation is used to expand the intervertebral space.
The PLIF approach removes the spinous processes with lamina, and facet joints.
The spinal cord is retracted and the surgeon can access the disc.
The surgeon can then remove the diseased disc nucleus to prepare the vertebral bodies for fusion.
After the disc is prepared, a biocompatible polymer implant is filled with bone graft material and placed in the disc space.
Procedure is repeated for the other side.
Additional bone graft may be placed around the implants to increase the surface area for fusion.
This procedure is normally accompanied by a posterior fixation with pedicle screws.
The implant maintains the spacing of the vertebrae while the fusion takes place.
After the fusion is completed and a solid bone mass forms the two vertebrae are joined together.

I had this at age 29 during my 3rd pregnancy. 3 babies in the span of 4 yrs. I suffered constant pain no matter what position I was in. Doc said theres nothing that can be done because Im pregnant, I cant take pain meds. Basically bed ridden until I gave birth. Then got into pilates specific for the back and slowly got better. Thank God! Its been 15 yrs and I am back to normal. No more pain.

I love the ending - Less invasive :) - How? There will be drills, saws, hammers, knives, screwdrivers, involved in cutting, chopping, and stuffing - finally glueing, sewing and taping. Follows are lots of pain medication, therapy, and weakness until you recover fully - but if you are lazy, don't expect an easy road to it - how do I know this? I had three surgeries :) on my back, I wear the scars proudly!

What was your condition ? I have been diagnosed with grade 2 spondylolisthesis and recommended fusion surgery Does the pain go away ? Or is there a new different kind of pain ?

@@sanaparker6257 Ok so this'll be very relatable to you. I just recently had my L5S1 fused for reverse spondylolisthesis (where bone moves forward instead of backward, but effect is the same unstable spine). I also had a herniated disc between my l5 and s1. So fusion is the only way to correct it. Unfortunately I didn't have a choice and I doubt neither do you if you have herniation as well. I just got out, 2 months in and so far 0 pain. I'd suggest do your research first and get a good doctor. I did my research and saw multiple surgeons and the one I went with did it from the pelvis (ANTERIOR) and I had no screws on my back. I'm 75% of the way to normal activity, but that's bc I'm being EXTREMELY careful and not overextending my spine and working to strengthen my muscles to keep a good back and taking it one day at a time. Hope this helps :)

I just had both a PLIF fusion and a TLIF fusion on my L4-S1 last Monday. My drop foot has improved significantly. I'm slowly recovering, walking when I can. The surgeon told me that the worst thing I could do is stay in bed all day. I'm not in as much pain as I thought would be and I'm improving every day!

Ryan Chansler kwebsterlmt@gmail.com is my email Ryan I would love to talk about your procedure and foot drop as I had a lumbar surgery a year ago and was fused from L4-S1 with 2 cages immediately after surgery I had foot drop on my right side and no explanation why this happened. I’m waiting for my 2nd surgery now to have a new Surgeon address severe spinal stenosis in my L3-L4 and remove the screws from my last fusion he said my foot drop is permanent 😢

i am scheduled for same surgery on march 27th, kinda getting cold feet after i have heard the low success rate and the adjacent disc syndrome that often happens with more surgeries needed. I hope you are well now. Can you tell me how you are feeling now. I was thinking of getting the nerve stimulator device instead. Would you reccomend the fusion over the stimulator? Thank you.

Cindy Shanks Hi Cindy, I’m sorry to hear your going through the same thing. As for me I am preparing for a 2nd Lumbar Fusion of my L3-L4 I’ll be fused from L3-S1 when it’s all said and done. Considering I just had my back surgery a year ago it’s a shame.Your right about the other disc being compromised it’s almost guaranteed. Do you have stenosis? I was never offered the stimulater. I will say that I workout everyday and soak in epsom salt each night it makes a difference. My foot drop is permanent as a result of my back surgery If you can avoid this procedure do so as long as you can. Our bodies are designed to heal themselves so long as we take care of them. Keep me posted I wish you well.

my doctor said if you did this, the adjacent disc would degenerate faster by compensation of bending to the fused disc... hence, you would keep doing fusions every couple of years.... god bless you

My doc did my L5S1 fusion by the anterior approach, when I see how complicated the posterior approach is, I say a silent thank you to my doc. If only I hadn't gotten addicted, no no excuse me, dependent, on prescription pain meds, (Lortab, I am looking at you) the whole job would have been perfect. My back healed up great, but it took 5 years to get off pain meds.

I'm confused. What's the purpose of joning the 2 vertebrae? I only see one could have decreased mobility. Is it to eliminate the pain from disc pressing on the nerve?

Pain meds are a deal with the Devil. I don’t know if I will ever be free of them. So much pain so many places. Thankfully I tolerate them well and am not compelled to abuse.

Yes, once a disk is degenerated and weak there’s no saving it. The nucleons herniation will keep leaking out and pressing on your nerves. It sucks because a fusion only leads to more down fusions down the road.

Lying in my hospital bed, just had posterior decompression with fusion and instruments, hope i said it right! 3.5 hours in surgery, have a great surgeon .Dr. David allen jones, tahoe fracture! Anestia is waring off and pain from surgery is begginig! No doubt i made the right decision, good luck ti you all

When I had an L5-S1 fusion they just laid match stick splints taken from my left iliatic crest and laid them in place across the space between the L5-S1 gap and hoped for the best...

That is my diagnose, degenerative lumbar @5 after my x ray, I take now B complex, and my exercise is back bend downward ,it works slightly. Before that dianose, I have a problem with my bowel. My left feet have pain also.

So I’m having this done on Monday on my L4. 6 months of PT, steroid injections and nothing is helping. I stopped PT and almost immediately started feeling better. 2 months in with no pain, and one morning my back gave out again. This time I was wheelchair bound for almost 2 weeks. So I agreed to do the surgery. But reading these comments have gave me great points to ask my doctor. I’m only 33 and my spine isn’t getting any better

Same here! I'm going for surgery tomorrow 4/18. I had an L5 discectomy in January 2021. My L5 ruptured after falling onto the toliet when my back gave out randomly. It caused permant peroneal nerve damage down my entire left leg & half the foot. Then reherniated around same area at the PT place like 3 months post surgery. I suffered large blood clots in the left knee (damaged the nerve on the femoral head) & ankle/foot as well after surgery. Now I will be going for a P.L.I.F tomorrow at L5-s1 and to now hopefully stablize it as well. I feel your pain! Good luck on your surgery 🙏 💓 🥰🥰🥰

I was curious since I have hydro syphilis and I have a shunt in the head which carries the cerebral fluid down the back. Could having the shunt on how it works could it cause for the disc to dry up

I am 2 weeks 6 days out from surgery. I left hospital after 4 days. I stopped 98% of pain killers after i left hospital. I take Panadeine forte and lyrica only 1-2 times a day. I have surgery pain but nothing that i can handle. My hips and left leg get very tight which i gently stretch every day now that im feeling less pain. Stretching slightly helps with sharp pain. I walk as much as i can. I started my own light weight-body weight exercise. My surgery took 7hrs. The ruptured part of my disc had grown around the nerve and turned into bone. When i woke up my face, chest and arms were very sore from laying on it for so long. The first 4 days was very very painful. But i kept going. Dont give up.

@@ryanb1938 I'm 19 years old, I have l5s1 disc bulge purely because of bad posture ( slouching ) , my pain is concentrated at the lower back only , but stretching doesn't help me . 4 months passed 😢 Doctor said surgery is not required but pain doesn't go away

In Europe this is done from the front. Doing it through the spine there's limited space and too many nerves = more probability for something to go wrong.

@@JohnDoe-gq8tt my situation was very specific because of how young I was when they did them but I actually recovered beautifully! Almost no mobility loss whatsoever and just a weight lifting limit :) but that’s something my physical therapist said I could even increase slowly as I’m comfortable

fact, fusions are not natural movement after and has a high risk for ongoing pain after that comes and goes long term because cutting of the bone like that. is why so many still have issues after disc fusions. I refused that type of treatment, but now after surgery it never hurts. i have issues with other areas, but 4 of my lumbar needed it and today they are pain-free. i think i made the right choice. I made it due to it being not a normal movement after...

Если так сделать то будет полный звиздец дискам сверху и снизу , так как на них ляжет двойная нагрузка . Британский профессор просто сказал , что со временем воспаление грыжи прекращается и она уменьшается и образуется на месте разрыва типа защитного хряща , но у каждого время на это разное , зависит от возраста и здоровья . В начале надо обезбаливать и снять нагрузку со спины везде и учиться правильно ходить и лежать .А такие операции только при сильных травмах дисков спины когда человек парализован .

It become infected and 9 days later i was in intensive care having almost died from septic shock. After 11 days in hospital, a month on IV antibiotics at home and 17 months of oral antibiotics (with a further six months to go) I am disappointed to report that the fusion has made no difference to my pain.

Today marks a year I got one l4/l5 l5/ S1 and I feel awesome! It’s a shitty recovery but anything is better then what I was feeling. I’m also 27 years old so In the grand scheme of things I’m very young to get one.

V. Stiviano running, sprinting and almost at 100%. Sometimes I forget that I had the surgery. I just need to make sure I stay limber and work on my core.

Hey glad to hear you feel better how are you now I am thinking I may need this same surgery l4l5 s1 fusion I had a disectomy and I got 100 times worse wish I never touched my back but can’t go back now it’s been 6 months and can barly do anything still trying doc said I got lots of inflammation and try a injection first I hope it helps but am wondering how are you doing and what was your pain before surgery like back or leg pain hope your doing well

@@thomasghee3701 been about 3 years since the surgery and i am almost dunking on people again. Before the surgery i could not do anything and pain was always an 8 out of 10. I have my life back now. The only thing i sopped were heavy squats but ill live. Wish you the best. Max out all the holistic approaches first then look into it if it doesn't get better. PT, Stretch and build up strength in your glute Medius.

I am a powerlifter few days before the PLIF. I suffer degenerative L5-S1 which causes limitations in my lifting so i decided to do this surgery. No possibility for other types of disc surgery. My question is ,is It possible for the screws to break considering there will be no loaded lumbar flexion and the neutral spine will be maintained during the heavy lifts. I know that the greater risk of damage is for the upper region due tu compeneations in case of causing loaded flexion but what about the fused region and the screws?

doctor i have a L4 L5 S1 i just got my new i had 3 MRI but this one says L5 S1 broad posterior disc hernation which is slightly more prominent than on the prior study deforms the ventral epidural space and there is left greater than the right foraminal extension with abutment of exiting left L5 root and narrowing of the foramina diminished disc space height ventral extension of the disc remains and discogenic chages and scmorl;s node in the end platates.mild facet hypertrophic changes remain L4L5 S1 ,,,,,doc my friend told me narrowing part of stenosis ,doctor can you help me out with this ,,thank i been watching ur video;s there very good ,,thank you,,ps my doc can be fixed with endoscopic laser surgery,doc what do you think,

Best Animation. I had this done 6 level's.The Pain level is different for everyone, my pain is unreal most of the time. I had surgery May of 2018 and I suffer from numbness and tingling, and feet going Cold. I been suffering for 8 years since a couple of Geniuses decided to put wax stripper on tile's, go figure.
