Prescribed Overdose of Sodium Valproate- FVSD Life

What actually causes fvsd? Is it only if the woman overdoses while pregnant, or is it just at a certain level of MG's that could still be at a therapeutic dose for extended use no matter if it's mother or father? I highly doubt that he would have it but I was taking 1500mg for nearly 10 years prior to my oldest being born. I had never heard of fvsd until I started watching this video so I really have no idea about what happens with it that's all, so please excuse my ignorance on this topic.
