I always stay within the canal. I do not over prepare in granuloma cases. I am sorry, I am not familiar of the concept of shaping beyond the apex in granuloma cases.

Coltene sells the 2.5% hypo solution. I have both full strength and half strength hypo solutions. I do not mix bleach. Its best to get the ones prepared by the manufacturers
In teeth with an immature apex, i stay 3-4 mm short of the apex. My activation tips also stay short of the open apex by 3-4mm. This will prevent extrusion of the solution into the periodical areas.
Im sure your Dentist will take all the necessary precautions while working on you during your root canal procedures. No need to be worried. You can ask your dentist to use a dental dam during your root canal procedure
