According to Prometric guidelines for the nurse aide state exam, this is a step-by-step demonstration video for assisting from the bed to the wheelchair using a transfer/gait belt. This skill is one of the 21 testable skills on the Texas State CNA Exam. Remember to include indirect care with this skill. You will start this skill with the client in the bed.

Hi, I thought before we finished the skill we had to unlock the wheelchair because leaving them in a locked wheelchair is “false imprisonment”

Hello Johana, You are testing on Assisting a client from the BED to a Wheelchair. That is it. Period, Nothing else. When you put the client in the w/c the wheels must be locked for safety. You will end your skill with the w/c wheels locked. The definition of false imprisonment is to prevent a person from leaving a site or position at his or her will. Having a w/c wheels locked is NOT false imprisonment because the client can unlock the wheels when ever they want to and move from one location to another. Take care

Good Job Dr Lee. But I just have a question. In most of the videos I have watched so far , you are not performing the task with gloves. Or is the gloves necessary for the tasks ? For example. The mail care and the dressing of a patient with a weak arm. You didn’t were gloves ma’am. Please could clarify me on this. Thanks 😊

Hello, Great questions. Prometric does NOT require gloves for the skills you mentioned (dressing the client with a weak arm, hand and nail care, foot care, or feeding).

Are you supposed to leave the wheelchair locked or unlocked. Another nurse said we leave it unlocked so that the client can move around if he wishes

Hello Dimple, to my understanding, each candidate will be given a card/paper with three sills on it. The handwashing and indirect care skills will not be listed on the card.

For number 16 on the checklist "to pivot without taking steps and to have resident touch back of chair with legs against wheelchair before sitting", how would we do that if you just shown full pivot and seated immediately without asking if the resident can feel the back of the wheelchair before being seated? im confused on that part of pro metric checklist.

Hello Miriah, I ask the client to stand, turn, and sit all in one smooth movement. After the client is on the chair, I will ask the person to move so that their back and buttocks touch the back of the chair.

I just have a a few question about this skill. What I have learned from my CNA class is that; You need to help client to stand first and then ask him to step on the side going to the wheel chair and then ask him if he/she can feel the wheel chair behind her legs and the arm rest. And give her a cue to sit down. Thank you so much for your time.

Hello Raul, Thank you for asking for clarification. I teach according to Prometric guidelines. So, with that being said, I am going to post the guidelines from the Prometric Skill Guidelines below. If you don't have access to this document, I will also post the link below. Notice the guidelines states, THE RESIDENT IS ABLE TO STAND BUT CANNOT TAKE STEPS. If the resident takes a step you will fail this skill. The candidate is asked to transfer a resident who is lying in bed, into a wheelchair. A pivot transfer technique is required and a transfer/gait belt is required. The resident is able to stand but cannot take steps. Hello Netz, I hope I am understanding your question. We don't want to place the wheelchair at the foot of the bed for three reasons. 1. Step 3 of the instructions says, "Place wheelchair near resident’s bed before assisting resident to sit at edge of bed". 2. Step 6 of the instructions says, "Provide support to assist resident to sitting position on side of bed, without pulling resident by lower arms or hands? 3. You are required to transfer the patient using a PIVOT technique. The patient CAN NOT take a step. If the wheelchair is at the foot of the bed and the patient is assisted to sit on the side of the bed, how will you complete the transfer with the patient taking a step? www.prometric.com/sites/default/files/NAClinicalSkillsChecklist.pdf See page 10 for the steps to this skill.

@@txcnaskills2599 I think this is a perfectly reasonable question and you misunderstood. You preform the skill exactly the same as you did in the video, except you position the wheelchair to the left of you rather than the right. I see nothing that would be wrong with putting the wheelchair towards the foot of the bed rather than the head. In fact, that curtain wouldn't be in your way either.

Prometric REQUIRES you to lock the wheel chair wheels and to raise the foot pedal BEFORE the client stands. If you don't do this you will fail the skill.

Dr. Lee, is there any specific place for the wheelchair to be placed? I’m a UAP “care partner” when I do a transfer I usually place the wheelchair facing the head of the bed, I think become I’m most comfortable, because I’m right handed. By doing so am I still within the testing guide lines.

Hello Heavenly, Prometric ONLY requires that the wheelchair be close to the bed so that you can safely transfer the patient without taking a step. The skills checklist for WC (#3) states, (Place wheelchair near resident’s bed before assisting resident to sit at edge of bed) The key things here are: 1. place the w/c by the bed 2. lock the wheels 3. place the foot pedals up BEFORE assisting the patient to the SIDE OF THE BED Prometric does not care if the W/C is at the HOB or at the foot

Hi Dr. Lee! I'm currently studying for my CNA State Exam (Texas). My question with this video is... will I fail if I don't put my patient's shoes on? I was told I must put them on myself when doing this step. Also when sitting them up, will I fail if I sit them up myself? Because I was also told that I had to do that myself, and that the patient couldn't help or else I'll fail automatically.

Hello, For this skill, the student can let the client put their shoes on independently. Also, the student is to assist the client to the side of the bed. Make sure you don't pull on the patient. You can put the shoes or have the client put on the shoes.

Hello Omodolapo, Prometric does not require us to put the resident's shoes on. What is required is that the client HAS Shoes on. It is acceptable for the candidate to ask the client to put his or her shoes on.

The skill asks the student to transfer the patient from the bed to the wheelchair. Locking the wheelchair is a step in this skill.

it be okay if I raised the head of bed then assist them so they’re sitting on the side of the bed? I’m just wondering since what if the resident has no strength at all? Ik in a testing environment this will not be the case but in a work environment would it be more efficient to raise the head of the bed?

Hello Lianathewolf, The raising of the bed in a real situation will depend on the patient. I would ask the patient if he or she wants the HOB elevated and do what the patient requests. The healthcare worker should not make the decision for the patient. I hope this answers your question. Take care.

Hello Yareli, I didn't make a video for feeding, denture care, and oral care because Prometric will NOT have the client nor candidate take off their facial masks during the skill exam. Things may change after the COVID 19 epidemic.

Not gonna lie, this seems a lot more complicated than what I was taught before. When I took the class at the beginning of 2020, the testing group I was with was Pearson Vue, but suddenly Pearson Vue says they’re not holding the test anymore. (At least in Texas.) That’s why I’m with Prometic and they have requirements that seem more confusing.

Hello Lianathewolf, Prometric started testing in Texas on September 1, 2020. The guidelines for Prometric are a little different from Pearson Vue. This is why it is very important to pay close attention to the skills checklist.

i also positioned the wheelchair same position you done and she said the wheelchair must touch the bed . I did don't know why must touch the bed if i did a perfect transfer. any suggestion for next time. Thank you !

Hello DB LM, There is NO WAY a client would not pass the state exam because the person didn't check the bed. When you look at the steps for this exam, checking the BED is not listed. Therefore, how can an evaluator mark you off for something that is not there. The state NEVER says the wheelchair MUST touch the bed. This will have to be proven to me because both of those things you said are NOT STEPS from the state from this skill. Someone is not being honest. A student will fail for not locking the wheelchair, if the client takes a step, not having on shoes, feet not touching the floor, or if the student pulls on the client. All of the failing points are listed as steps by the state.

Hello Vivien, I encourage you to always check things out for yourself. As of today, nothing has changed. Prometric continues to state: The candidate is asked to transfer a resident who is lying in bed, into a wheelchair. A pivot transfer technique is required and a transfer/gait belt is required. The resident is able to stand but cannot take steps. The role of the resident is played by a person. Check your source.

Dr. Lee, I have a question about this skill. Do we have to make the resident feel the chair behind their legs before sitting? Also, the skills checklist provided by Prometric says that the resident cannot take any steps, so does that mean they cannot turn their feet before sitting in the chair? When I practice this skill it seems like I am being too forceful while turning them to the chair because their feet are still oriented in line with the bed. I wanted to make the resident stand and then turn their feet to align with the chair so that it is easier to sit. (I hope that made sense) :) Thank you!

Hello Amber, I think you are asking two questions. 1. The Prometric skills checklist states: Move wheelchair footrests out of the way before standing resident for transfer (e.g., lift, swing, or remove)? Therefore, you can stand the resident on the side of the bed. The resident CAN'T take a step. So , I would make sure the resident's feet are positioned where I want them when the person stands. 2. If you pivot the resident to the chair in one movement, the resident will not have time to feel for the back of the chair. Prometric skills check list states: Transfer resident as a pivot completed without having resident take step(s) to reach wheelchair, and before assisting resident to sit in wheelchair, positions with the back of resident’s legs against the seat of wheelchair? The last sentence is to make sure the candidate is sitting the person in the seat of the wheelchair and not on the arm rest.
