I NEVER comment on videos, but something you said stuck out. In your list of pros, you said you love learning about different people and used the example of having an Indian family and then a Japanese family and just enjoying learning about everyone. That right there. That’s that love and energy I was looking for in researching whether I wanted to pursue home health. I love your energy and your love for your work. Thank you for this video and your beautiful spirit❤️

Mmm I experienced a toxic environment with one of the homes I went to. The parents always argued and the mom treated the sick kid poorly at times. It was beyond stressful to be there for 12 hrs. Then a situation happened where I was treated like trash by the parents, and I was like yeah this house is not for me at all. My supervisor respected my decision to leave and was totally understanding. The other houses I've been to have been perfectly fine though. The families are usually very grateful to have you! Sometimes there are environments that are a hot mess but you have the freedom to decide if you want to work there or not.

Always take care of yourself. Don't feel that you have to be there for the "patient" driven by the fear that if you don't help them then no one will. They're toxic to everybody.

I've been a home health RN Casemanager for 6 years and have worked for small and large agencies. You can find full time positions with benefits but typically those are RN case manager positions. Lots of perdiem positions available. There are unskilled agencies with low acuity patients typically medicaid with lots of home supervisory visits and skilled agencues typically Medicare. Rn home health responsibilities can be immense especially when working for a medicare agency. Medicare patients tend to be older and higher acuity with Picc lines, trach care, woundvacs, labdraws, catheters, ostomies, chest tubes etc. The documentation requirements are high with Oasis, ROC, Recerts. I spend a lot of time delivering pt care in the home and documenting after the visit especially with new admissions. You have to make a lot of phone calls to doctors, social workers, PT, OT, ST, POA's etc for orders, changes in condition, order clarifications. On call for my agency is every 4-5 weeks for 7 days. I've had to make home visits in the middle of the night on occasion. The agency expects me to work the entire weekend, granted every agency is different. I typically see 6-7 pts per day which is a lot when your driving from house to house. Some days are smooth and I'll get home in 6 hrs. Other days work is crazy busy and I get home after working for 10hrs. There is a high turnover rate burnout is real. I recommend starting out partime and asking good interview questions, regarding mileage reimbursements, expected number of pt visits per day, is there an on call rotation, how many nurses are employed in the agency etc. Your schedule is relatively flexible depending on pt needs. Part time nurses have the freedom to decline assignments not so for the full time nurses.

I did home health for a few years and couldn't stand it, didn't like taking people to doctor appointments, didn't like cooking for people, wasn't comfortable being around family members, you either like it or you don't, im more comfortable working in a hospital setting, a lot more work but the time goes by a lot faster, thanks for the great video 👍.

@@mercygonoRN hi mercy, I found your videos beyond helpful particularly as it trying to pursue a career in home care. Did you find that once your company trained you and once you got used to everything, the job as a visiting nurse became much easier? I’m a new hire and I am a little nervous as there are so many new things for me to learn and I’m not exactly sure what to expect. Thank you.

Most agencies now offer some benefits. May not be as good as hospitals, but they do offer and vacation time too.

I'm a non-medical home health aide. I work part time and out of town. What I like about home health is being in clients' homes since I myself am a home body. I like one-on-one care. Pets, yes, I'm fine with pets. I had a client that I worked had three dogs that I walked. First day there, one dog got out of her collar and ran! Praise the Lord I caught her. Smoking clients...I am sensitive to cigarettes, especially in doors. I had smoking clients despite telling agency no. These clients were so sweet, so I stayed with assignment. So I guess smoking is something negative in home care. I have not had a client who has medical marijuana card yet. That will be interesting especially for smoking. ☺. Yes, drive time is stressful, especially when taking clients to appointments. Going to appointments can be stressful if there clients that take more time to get ready. Then traffic and getting to the appointment on time. And often you use the client's car, so it takes time to adjust to each car. Yes, like you, I like meeting clients in their own homes. Yes, with split shifts are good for running errands or picking up another client. I hope to pick up more clients in rural areas.

@@mercygonoRN 💙. I'm very passionate about Home Care. And sometimes people don't even realize there are non-medical home health aides. I've been in training sessions mixed with nurses that didn't even know about people like me. In my state basically non-medical means you do almost everything except vital signs. I've even changed wound bandages . Basically you're just not getting paid what the people who have certifications are getting paid. And that's okay. If I decide to go back to get my certification I will. I don't know if I'll ever go through nursing school but that's another long story. LOL

If your looking to becoming a Chha do not do it. Majority of clients think your their maid. Some think that you should be cleaning or doing anything just to stay busy during the whole shift. Aides don't get paid by the minute. Some clients are also very racists.

Excellent job of telling the way it is. I'm a retired RN - One of many jobs was providing home care under all kind of situations. the most unique situation was a college student ina dorm room on a venitlator and straw controlled wheelchair. Little did I know that my healthy, athlectic adult son would become severely disabled when I was ready to to retire. I already knew the flip side when we have had home nursing cna's in the home. I'll leave it at that except to say family and friends think it is so easy to hire an agency and have everything work well.

As a home health nurse what are you typically doing? IV med infusions? Blood draws? Wound changes? I’ve been a bedside nurse for 10+ years but want to know what a nurse does at the visit. Plus, how often do you insert a Foley catheter? Can I request smoke free homes as I can’t stand the smell of smoke.

As far as going into bad neighborhoods, thankfully my company actually has security guards that can escort you into homes in not so great neighborhoods if you’d like. My company does have benefits for full time nurses. I don’t have complete control over my schedule though. I have to see at least 6 patients a day and I’m assigned these patients daily. However, I do control what time in the day I see those 6 patients. I wish I had got into visiting nursing sooner in my nursing career.

I worked in a neighborhood called Pine Hills. It is jokingly called Crime Hills. I went in with an open mind though it was good for awhile. I thought we had a repore with my client. Then there were three shootings in about a three month period ,one right on her street. Then all of a sudden when BLM started she became very verbally hostile to me being white. I told my agency that it was time to leave her. I got the standard you have to give 2 weeks notice to leave a client. I know other caregivers who have been sent into unsafe and extremely dirty houses and told the same thing. Where does our safety come into play

In New York City there are some pretty good agencies out there that have unions and benefits. A person just have to make sure they choose a good agency to work as a Home Health Aide.

I am an RN, so LPN do less documentation. Where do you work? In my 10 years of home health nursing, I have not to experience that much amount of flexibility. Some agencies make you clock in/out and others use programs that monitor your time. You have it really good!

they always ask if pets are in the home and what type of pet. Remember people do have allergies. So if you dont want to say your afraid you can always let your job know you have allergies to dogs and cats to avoid getting patient's with pets.

After working in AL facility for 3 months & dealing with the drama, I went back to private duty & I’m sticking to it! I enjoy one on one caretaking & not worried about the call pendants going off at once (because the lazy coworkers don’t want to help)! I also don’t have to deal with racing to grab an available walkie from the previous shift!

So let’s say a home care pays 35$ a hour and you work per visit does that mean your hours don’t count? or you get paid by visit? I like the pro about being able to go shopping and go home when your not at a patients house is that shifts or visit?

Mercy, thank you for sharing everything with us! I think I'm about to go into home health after 15 years of acute care positions (including leadership). I resonate with your reasons, such as avoiding the politics, the autonomy, and just enjoying other people and their lives (for those patients in which you can) in a more relaxed environment for them. Love your beautiful spirit, and although your make-up is incredible, your natural beauty don't need it. God has blessed you richly and may He continue to do so.

Uh... that seems like a bad idea. I do HH in a lot sketchy neighborhoods. Makes me think of it but there are more things that can go bad if you have a gun or even if feel like you need use it that could have VERY bad consequences. At worse you’d be the nurse in prison for shooting a patient. At best, you’d lose your license and never work as a nurse again. Just keep your head and if you feel unsafe, get in your car and leave.

It depends on the agency. My agency offers both salary and per patient. No hourly.

I was offered a home health care job at Bayada for adults needing vent/trach care. $31/hr. To put this into perspective, I work psych in a hospital and make $54/hr with benefits. $63/hr on weekends. 3 years BSN.

Bayada is private duty not home health, right? Private duty is always less, you stay there the whole shift and likely work only about 10-25% of the time there?

Thanks for your video! I am a nursing student and thinking about where to go after my graduation. I am very interested in home health nursing, but I am not sure if it is a good starting point for my whole nursing career journey? Some said once you start with home health nurse, you grow slowly, it is not like working in hospital setting where you can learn a lot in very short period of time. It that true? Thanks!

Hi thank you for this video, it is very helpful. What happens if you get a really bossy and controlling co-worker who comes along and tries to shift things around and make changes? Meanwhile you have been comfortable with your patient for three years and comfortable with the other aide who has been here for six years? But that aide left and now you're stuck with a controlling co worker?

Right now with the computerized electronic systems. I have started noticing the agencies are not responding to your calls, and communication amongst nurses and office/clinical support has decreased. I am a seasoned HHRN, but I am afraid the new nurses trying to get into this specialty are having a rough time making the transition. This problem, and the constant changes in the work culture and demands, is causing a lot of discouragement and nurses leaving in droves. That's why I became a travel nurse to free myself from some of those annoyances, if not all of them.

So when you travel, you go to another state per se and see different pts in those particular allocated counties and zip codes? So it means you HAVE to drive your car there no matter how far it is from your home?

@@magatsheni Yes, that's correct! I drive to different states within. the US, or I may get a contract in my state a few hours away from home to receive the tax free stipend. It is tiresome, but overall enjoyable lots of amazing views. I come home and take a long break before my next contract, or I take 1 or 2 weeks and extend.
