Remember! Don’t give up. I had a PSA of 3,280 with a Gleason score of 9 at the time of my diagnosis. Plus, way more than 100 tumors in my skeleton, even broke my neck because of 7 tumors in my neck vertebrae. That was 15 years ago!!! And having loved ones around you definitely help too.

After 3 years of rising PSA and 3 biopsies they finally found a 4+4 Gleason by doing a external entrance biopsy at Johns Hopkins in Baltimore. PSA was now up to 40. I just finished 28 IMRT treatments at Ann B Barshinger treatment center in Lancaster Pa. Now I have a HD Brachytherapy scheduled for May 22nd at Fox Chase Cancer Institute in Philadelphia. I am also on ADT for two years. One Dr said I should be cured after that. That is the "C" word I am going after. Hoping for the best!

Four months off of ADT my testosterone is now normal and my PSA had remained at.04, so guess that would put me in durable remission. And I feel normal again!

I was diagnosed with stage 4 prostate 1 year ago and was placed on Lupron & Casdex. But I changed my team after I got a second opinion, And I was told my stage 4 was very early and had just a had a few specs on my lymph nodes, My new team of doctors immediately ( 3 months later ) started me on Eligard & Extendi and I went on an extreme diet of green drinks whole foods & wild-caught salmon and ( not much tuna due to price ) and I perform a Chinese exercise ( I'm not saying this work but I performed the movements every day) 7 months later I'm in remission. I still have my prostate before this the doctors on my team later to me about radiation and other therapies but, as I went back for my 3-month check-up I was told I'm still in remission and as of now there is nothing to radiate, I'm down 25 lbs and I like to think loss did to my extreme diet and exercise regimen because I make sure to eat 3 small meals a day whether I'm hungry or not my PSA is in the neighborhood of undetectable I don't know how long this will last but I'm not giving up on my plan.

Loss is not due to your diet it is because harmone injections you loose your muscles mass pl do weight training.to overcome weight loss

After 3 years I had a 6 month remission felt really nice to not have to go to Mayo for 6 months now I’m back to 3 months so I’ll take what I can get and do what I have to as new treatments keep coming.

I now fully understand my status. My PSA has been 0.03 on four occasions after one year treatment using hormone therapy and brachytherapy. According to Dr Scholz's explanation I am on remission. I haven't reached the cure stage.

Just had my 2nd blood test after 6 months of having radiation. My PSA has dropped from 1.3, 6 weeks after radiation to 0.7. My Radiation Oncologist was very happy with the PSA number. Next test in 6 months.

I had PSA 14 Gleason 4+4 prior to proton radiation, 4-months after radiation 1.14 PSA. Going to have another blood test in 4-months. I’m 6-months after radiation.

My concern is I was prescribed ADT hormone therapy with Gleason 4+4 I declined due to all the known deleterious affects and reduction in quality of life, I’m hoping it works out, so far so good I feel absolutely great with no urinary or other problems.

@@Benb55 My thinking is if I reach .5 or less PSA and/or continue a down trend, coupled with PSMA surveillance hopefully all will be well. If any spread is detected early I think it can be successfully addressed, depending upon the nature of the metastasis.

Me: diagnosed 2016 Gleason 9 PSA over 300. Lupron, Abiraterone, radiation, chemo. By 2023, bone Mets hips, ribs, spine. Mid-2023 full course of Pluvicto almost no side effects. Bone scans, CT, MRI now show no active cancer. No new tumors, no activity in older ones. PSA now around 0.7. Every finger is crossed!

As a patient on Lupron and Abiraterone after surgery and full pelvic SBRT prior to ADT, I am finding hormone therapy tolerable (2 years) through intense exercise and vegan diet. I can't build new muscle mass, but I manage to keep what I have. Mental side effects under control as well from these behavioral adjustments. My PSA is undetectable and I see people that stay on it the rest of their lives. I am leaning that way as a 63 year old just because I see stories of 20 plus year survival cases on ADT. Example Michael Milken 27 years plus on ADT, he had the resources to get the research, so I may follow his example.

Thank you for the excellent informative video which demonstrated the appropriate balance between research evidence and the caring aspect of the medical profession regarding prostate cancer. I may have watched all of your videos since my PCa diagnosis in 2014 and undoubtedly they have been immensely supportive enabling me to relate positively to my cancer and evaluate the suitability of treatment plans. After 6 years of active surveillance with annual MRIs, I had a prostatectomy in September 2020. Unfortunately, I had a recurrence in June 2021 and then 33 radiotherapy treatments completed in January 2022. I started 150g of Casodex in August 2021 but after a discussion with my oncologist, I stopped this in July 2022. My next PSA test was due almost 10 weeks after I stopped the hormone therapy and the reading was

Had radiation treatment for 20 days. 3 weeks later PSA went from a 15.6 to and 0.43. 4 weeks later it's down to 0.19. Today it was o.54😮. Urologist says that bump is normal. Next PSA is in 6 months, hoping psa is still around.0.5. Though he , my urologist, said don't panic unless it is a 2.5 or higher.

Another Excellent and informative video, my PSA is holding at 2.5 two years after ext radiation when my initial cancer was 4.77 PSA. I never had a follow-up from radiologist or urologist. You have to be proactive with your own health, I’m now seeing an oncologist about the PSMA going forward.

PSA was 4.2 and had MRI and biopsy and was diagnosed May 10, 2023 with prostate cancer (Gleason. 3+7). Teal treatment….3 months on Firmagon and 5.5 weeks of Radiation . Had brachytherapy surgery Oct 2023. Just had first PSA after treatment and was .3🙏🏽🙏🏽🙏🏽

I had a PSA of 9. Down to 0.08. have had surgery and now chemo. Doctors said I am good for 15-20 years. That would make me 80 plus. Exercise is very important.

Had localised PCa. PSA had gone to 54. Biopsy showed Gleason 3+4. After a lot of search on Internet, firmly decided (in spite of several well wishers suggesting surgery)to go only for Cyberknife. I then searched for the most reputed Doctor in India with a proven track record on Cyberknife. Had the treatment (with no other concurrent treatment) with absolutely no side effects. In 4 months my PSA has dropped from 54 to 4.1. PSMA and MRI scans have shown substantial positive results. Keeping my fingers crossed that this is remission and it progresses to a cure.

My OSA was 427, Gleason score was 4+3)=7. After treatment at Cyberknife my PSA has gone down to .3. The quality of life issue is my only continued hope. Having to get up 6 times a night to urinate is not a lot of fun. If anyone any knows of the cure for that I would love to talk with you. I am praying that this next Oncologist visit I will be told no more Lupron (The drug from Hell). A 6 month shot of that and there goes the quality of life.

You might try nocdurna 55.3 mg sublingual tabs. They worked for me making it only 1 to 2 times a night vs 5-7. Huge difference in life quality.

I had an enlarged prostate.PSA was 5.83 Dr did A TURP and sent in sample for biopsy. Came back with a Gleason score of 4+3=7. Diagnosed cancer confined to the prostrate. After a week removed catheter and gave me a hormonal injection saying it will shrink and slowdown growth of cancer... The PSA after TURP was. 0.56. I did a PSA after 3 months came down to 0.08.. Dr wanted to do Brachytherapy and radiation immediately. I disagreed and went for a 2nd opinion suggesting removal of the prostrate. Side effects of excessive sweating and ED.. 4 months later still have those side effects... Thinking of having a removal of the prostate... Believe very little to no side effects in a experienced surgeon's hand

These videos was a blessing to me after last year being diagnosed with 3+4 and psa rose to 25. Thank God i was non metastatic and i was treated with standard radiation and lupron for 6 months. Now my psa is down to .2 my doctor suggested that he continue with 18 more months of lupron based on how high my psa have gotten at diagnosis. I agreed to another 3 month shot after my psa reached .2. But im debating should i stay on lupron or stop the treatment in August at the end of this additional 3 months shot, as the doctor agreed to do also?

MRIdian with ViewRay offers gating which shuts off the beam if the target area moves out of the beam, or the rectum or bladder move into the beam and would be subject to injury. Standard treatment checks positioning only at the start of the session. A real-time MRI takes 4-8 views each second and automatically turns off the beam if that's appropriate. Adaptive therapy allows for adjusting the size and shape of the treated tumor on a daily basis. These innovations make it safer to consider repeat radiotherapy for recurrences when the tumor is seen on PET scan.

I’m being treated at the VA. My RO told me that whatever my nadir ends up at (currently.88 9 months post radiation without ADT) if my PSA goes above that 2 points ( PSA 3) I will be considered in chemical reoccurrence. That seems much higher that this video suggests.

Thanks again for another very helpful video, especially for those who had focal treatment. Usually PSA checking for this type of treatment in UK is three months after initial treatment and than six months after the first test and one year after second test which fallow every year. I have to mention your advice for the PSA checking interval to my consultant doctor .

I had already 5 injections of lupron and PSA dropped from 134 to .001 and one more after 3 months will be the final.No much fatigue or urination problems. Very costly treatment around one lakh.No idea what will be after the above? KP ASHOK .....

After 11 years on AS I had a HIFU hemi-ablation treatment 2 years ago which thus far has been working. I simply view myself as being on AS again. However, having now had 5 MRIs with a GBCA I am reluctant to have another one and will instead seek a PSMA PET if my 6 month PSA check-ups indicate a problem. It would be helpful to have a video focused on the nuances of monitoring after focal therapy.

I had Brachy done 9/22. Mine was located in Prostate only, one side. Whole Prostate was treated with Poladium. My PSA going into this was 6.8 - 7.0. I had 3 areas: 2 were 3+3 and one was 3+4. On Dec 15, 2022 I had PSA check and it was at .5! Hoping when I have my 6 month in March, its even lower but pleased as it is! Symptoms have been all over the place but around 90 days, started to settle down some. Still up at night a lot! Hang in there its doable!

@@marka9073 First and foremost, I could not have done any of this without this youtube channel: PCRI! The information here has been substantial and incredible! I was fortunate to get to the local doctor that I wanted, Dr. Steven Frank at MD Anderson, he has some youtube videos. He and his staff have been nothing but incredible! I had LDR and the process was flawless, other than not being able to use the Space Oar Gel. I was in and out by 1pm, which also included an immediate MRI to check placement. Palladium 108 was used. My side effects have been all of them, but really began to lessen around 90 days when the Palladium hits its 1/2 dose mark. My long term has been frequent urination at night. Currently I am fighting off a UTI that could be from a cysto procedure or prostatitis. I went with LDR because I did not want to do all of the daily prep for radiation. I would do it over again as the correct choice for me and would recommend to others. Surgery was not even an option for me, did not want it! Within the first 90 days things can get a little rocky with regards to bowels and urination, but you just to plan and know where the restroom is where ever you are. Have not had any incontinence. I have traveled with no issues on 4 hour flight. I started on Flomax and am now on Rapaflo. The side effects are less on the Rapaflo for me. To be honest, I did not have any issues before and am taking it as part of my Dr's recommendation for now. The procedure has not slowed me down in any way and I am 68. As far as the ED, cant address that because not active nor is that a big concern for me right now. Ask away if you want more.

@@ejcsds Thanks for sharing. Always good to hear good outcomes. Hope continues well for you. I have so many questions. 63 yrs with PSA at 5.1. Initially diagnosed on 6/22 3 areas 3+3. After MRI had 2nd biopsy 12/22. 4 areas on 1 side, 2-3+3, 1-3+4, 1-4+3 via 2nd opinion JHHopkins(local path said 3+4 for the one JHH says 4+3). All low involvement with the 4+3 at 5% involvement with 60% of that pattern 4. So very small and all contained and verified by MRI and PSMA PET scan. How much involvement did you have? All one side? What decision process did you go through to go forward treating Favorable Intermediate 3+4 vs. Active surveillance? Do you live near Texas/MD Anderson or did you fly in? Currently researching, monitoring PSA. Will move to treatment if any progression.

@@marka9073 I am in Houston. My initial Urologist and organization called mine as 2 3+3 and 1 3+4, all one side only. JH also bumped my 3+4 to 4+3, but MDA had my slides last and called it like my first Urologist. My decision was to take action now and not wait. No one suggested AS. Not sure what your "involvement" question is about. None of the folks I consulted ever tried to convince me of anything. I went in to MDA wanting to talk to a specialist in each area: Surgery, Radiation like Photon and Brachy. Two doctors that I spoke with, 1 at MDA and the other at my original urologist, both stated they would do Brachy without question. Of course the surgeon wanted to cut! But I had already made up my mind before knowing their position. I cant say enough about the support staff of these MDA Doctors, they are incredible. The Mays clinic is where mine was performed, located outside of DT Houston has hotels all around because it is in the Medical Center. In reality, after my full MRI scan revealed no metastasis, I said lets move forward! I did not want to take any chance of it getting out! Could I have waited this out, maybe only one person knows the answer to that! I have learned so much from Dr. Scholz, that I probably should have considered a 4 th opinion from him! The only thing that I wished MDA had, or anyone for that matter, is the ability to talk to like patients for their first hand experience.

Can you give some guidance on possible reoccurring prostate cancer and/or cure rates where the patient like myself has had surgery followed by radiation together with 6 months of hormone treatment to combat localised lymph node. I feel most of the comments are about people who have has either surgery or radiation and not a combination of both. Thank you for your informative posts.

My "cure"lasted 24 years (proton beam) , now I am on Lupron with PSA of 0.21. PSMA scan indicated non metastatic but too much for surgical treatment. Obviously, I will be hoping for a new class of treatment, maybe a radio isotope tagged to seek out the cancer.

You guys are great! I'm 57 years old. My PSA was 60 on 12/6 as of yesterday its 75, my doctors told me it spread, so no surgery. My PSMA showed some enlargement in my lymph nodes around my trachea, but nothing near my prostate. I have 1 spot on my pelvic bone as big as a pencil point. I'm getting a biopsy on my lymph nodes next week. I'm told , no radiation and just hormone therapy. I'm more than happy with that, but I dont hear of anyone just doing hormone therapy. Did anyone out there just have that?

My husband has lymph nodes and left hip spread, on my had hormone therapy and then had his testicles removed.... Going for a PSA test on the 30th January... All the best my husband is healthy as ever no swelling in any of the lymph nodes and almost no pain ... All praise to god

Mayo in Rochester may be a consideration for you, there are physicians there that will treat Prostate Ca with radiation if there are not to many areas involved……

Hi Dr Scholz and Alex, I had surgery which failed. My PSA is 0.8 and rising. I have been advised to have RT and ADT, however I wish to avoid Zoladex as my quality of life is paramount to me. I understand that the ADT would only add about a 10% better cure rate to the RT alone. Am I right? Thanks. Paul

Hi, my brother’s prostate cancer wasn’t discovered until it spread and effected his urinary system. He now has two tubes coming out his back to drain his kidneys and the doctors say the cancer might have spread to his bones. He gets kidney infections and I am wondering if he can still get hormone therapy. Thank You, God Bless.

I had prostatectomy surgery in December 2020. My PSA was 5 and my Gleason score was 9. For two years after surgery my PSA was .008. I recently had my yearly physical exam and my PSA was .022. My GP didn't seem too concerned but he isn't a urologist. Should I be concerned about reoccurrence?

I am being treated for localized prostate cancer - the cancer is only in the prostate - using hormone (Eligard injection) treatment in conjunction with Casodex. I had my first injection 2 weeks ago and it is for three months before a second injection. Before the hormone treatment began the doctor surgically removed the inner part of the prostate. I guess it is called "resection of the prostate". 1. I am experiencing very severe "extreme fatigue" side effect, which makes me feel often like I could faint or pass out, and like life is being sucked out of me. Is this normal? Does it decrease over time? Is there any medication to ameliorate or get rid of this devastating side effect? 2. This extreme fatigue comes upon me quite suddenly and lasts a t least a day and then goes away, and then later comes again. When it goes away I feel well. But it always returns. It is accompanied with a distaste for food so that I have to force myself to eat and a general inability to sleep. 3. Is radiation therapy a must after or during hormone therapy to obtain a cure? Please comment and advise me....Your video has been a big positive help in understanding my condition . Thank you so much!!

At about 10:10 in the video - the discussion on durable remission, the number of "undetectable" PSA for men who had surgery, and less than ".05" for men with radiation (after coming off ADT) is used. Is the 0.05 correct - or should that be 0.50? Less than 0.05 on a standard PSA test would be undetectable since the limit is around 0.1 for detection of PSA on the tests I've had.

I had a prostatectomy 8 years ago and PSA has been undetectable since then. Just had two PSA tests to confirm 0.17 Question is to start salvage treatment or wait to see what a PSMA Pet scan shows. I am told that scans show nothing until the reading is at least 0.5, should I wait? Thanks

PSA can rise with age for people who still have their prostates whether or not they have cancer, and can vary quite a lot from one person to another, so the level considered "normal" is a wide range and PSA level history has to be considered. It is very sensitive for monitoring recurrence of cancer after surgery or radiation treatment.

I had prostatectomy 9.5 years ago, PSA stayed undetectable till a year ago when it showed up at 0.18. PSMA Pet 6 months ago was negative. PSA now up to 0.35. Radiation Oncologist wants to do prostate bed radiation plus hormone therapy. I am 78 years old recovering from shoulder joint replacement surgery with more joint surgery on the horizon. I do not want to go on Hormone therapy, my muscle loss is already bothersome. I have requested another PSMA Pet scan. Any thoughts/suggestions? Thanks!

i am a little confused about your definitions of “…in remission…” and “cured”. i was diagnosed with high risk prostate cancer in 01/18, treated with 9 months of lupron, then 25 days of radiation and finally LDBT. my psa prior to diagnosis was 5.4, then dropped to

With full-gland radiation treatment, the general assumption is cancer that existed within the gland will be destroyed as a result of the treatment, but what is not known is whether or not any cancer cells escaped the gland prior to radiation treatment. It might take a few years for such escaped cancer cells to work their way through your body, lodge in a distant location, and begin to replicate. My interpretation of what the doc is saying is if your PSA remains low five years after RT, you can fairly safely assume no cancer cells escaped prior to the radiation treatment, therefore no metastasis, therefore "cure" is achieved. The situation is different with surgery because surgeons leave a sliver of prostate intact so as to avoid damaging nerves and adjacent organs. There may well be cancer cells that were missed by the surgeon, in which case the localized cancer might "come back", meaning it was never fully gone, and it might take a decade for that to happen. I could be wrong. I'm not a doctor. This is just my interpretation.

I was diagnosed with Prostate Cancer back in 2021 with a Gleason score of 3+4 and and a PSA of 12.9. After much debate and seeking a second opinion, my oncologist (2nd opinion) suggested MRL. I underwent 7 sessions starting in Aug of 2022. My PSA level dropped from 12.9 to 3.7 after 3 months of having radiation and currently is at 2.4 after 6 months. Can i consider this as positive news or is it something to be concerned about. I am meeting with my oncologist in the coming week to discuss, in the meantime Dr. Scholz your opinion would be greatly appreciated.

I don't think what most lay people regard as "cure" is the same as medical people define it and it is rather misleading to take "cure" as meaning 5 years' biochemical recurrence free and the like. At least the definition should be explicitly explained to patients who may not have educated themselves about their disease. Too many surgeons and radiotherapists fail to do this for what may not always be pure motives.

I received RT at MDA center in KC,MO at 73, 7 years ago. The cancer returned 2 years ago on the two lower lymph nodes. Am now on ADT and did another round of RT. The PSA has again risen to 6.7 and am waiting for an appointment to have another PET scan to see if the cancer has spread to other places. It's a never ending round of battles.

In the US of A my insurance was charged $34,000 of which the negotiated pay out was $17,000. I suspect you are talking about a MRI. The PSMA scan uses a marker injected during the scan and is a head to toe look for prostate cancer cells. If it the PMSA scan is that inexpensive where you are, I’ll be seeing you sometime soon…
