Sir the gic luting cement can i use it as a base or its a different type and it dose not work ? Please answer

You can but it wont have adequate properties which is required for base since the particles size is small. Theoretically its not right.

Dear Dr.Suresh 🌹💚 I have 3 questions if you may. 1) in deep composite cavity like 0.5 mm of remaining dentin thickness is remained .. if i put the bond and composite directely is it right that the monemer may get inside the tubules and irritate the pulp.. because some say yes and some say no the bond will protect the pulp ? 2) composite is connecting to tooth by micromechanical mean.. if i decided to put glass ionemer cement in the floor of the deep cavity than i will apply the bond .. how the bond will connect to the glass ionemer as the self etch will not make micro pours in the glass ionemer or should i use sand blast here or what ? 3) I heared doctor said do not use resin modified glass ionemer under composite as the glass is setting by acid base reaction means it needs water thus will absorb it from the dentinal tubules which may lead to necrosis and sever post operative pain .. use the capsule or 2 pastes glass .. is that right or wrong dear Doctor ?

1) at 0.5 mm remaining dentin thickness liner is must before placing composite. 3) resin modified gic is best for base. Pls check my fundamental of tooth prepration video

Sir if we are not using sandwich tecq then after applying liner, we etch the liner also Or just the sides of the tooth and not liner becoz liner is alkaline and if we etch then it becomes neutral and there would be no use of liner??

Great video sir! One question: when you apply the base /liner on the dentin, do you dry the prepared dentin and clean it with benzin or Corsodyl prior to it?

There may be some articles which say it may benefit with corsodyl. However hypochlorite has been recommended in some or infact many articles which will help to arrest the bleeding. If it does not then case it not suitable for pulp capping

Hi sir I would like to share a case scenario which I recently come across please give your valuable opinion sir 19 year old male patient 36 - class 1 caries buccal pit and occlusal surface. I had excavated the caries . Caries is too deep to near the pulp. I restored with the light cure composites. On restoring i apply the etchant, bonding agent and while light curing the patient feels slight senstivity and the sensitivity subsided after curing the bonding agent I restored the whole tooth with light cure composite. No liner applied The patient doesn't feels any sensitivity. But the patients reported after a week and complaining that while having any thing hard on that teeth like seeds I feel some pain on that area and there is no issue with sensitivity I checked for any high points with articulating paper and there is no high points There is widening of lamina dura in 36 and no periapical pathology If the pulp is dead the patient might feel severe sensitivity and pain, but in these case no history of pain and sensitivity. Pain occurs only on having hard food items like seeds Is there anything wrong with pulp protection If the pulp is inflammed the patient might have a history of senstivity Please give your valuable opinion in these case sir

Dear sir i have 5 inquiries you may ❤ 1) can i apply a varnish to all cavity walls after I finally cure the composites , so i get the benefit of gap sealing for any polymerization or any future composite shrinkage and guaranteed that varnish will not affect composite polymerization because it is already polymerized . 2) would you correct my steps here In deep composite cavities we put the liner at 1st then etch and bond to ensure acid etch will not affect the pulp is it right 3) we can put the liner in axial wall if i saw that the cavity is deep to the pulp Or it is only should be placed in cavity floor 4) will the liner prevent the formation of resin tags at the area we put the liner , means if we put broad amount of liner in broad deep cavity will this lead to weak composite bond or the the liner will not effect the resin tags formation 5) in composite why patient may feel pain because of monomer penetration although we sealed by the bond is it a curing mistake from the doctor or the bond may irritate the pulp

Answer 1- Composite has a micromechanical bond and the initial gap is very microscopic, varnish wont seal it. In fact, nowadays even amalgam sealants are used instead of varnish. I have explained this in my series of Fundaments of tooth preparation. For example, both the pulpal and the axial wall can affect the pulp but not the gingival Seat. Hence we only give liner in the pulpal floor and axial wall. Sir if the dentin thickness is less than 0.5 mm during composite restoration then what will be better... Using a gic base or not using any base. As u said we can use base it we want...? What u prefer in practice.

Gic can help. Because it gives the advantage of sandwich techniques. If you plan to place a liner even then gic can be placed. I recollect that I have mentioned this in some of the video, but I am not sure which one.. you can check ny latest video which also covers pulp protection in brief in the second half of it. this..ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-QkGwtRHeBZ0.html But there is also something called as sdr Composites which is similar to gic and used as base below restorative Composite. Even that can be used but it is expensive

No. There is no difference as such.

Sir in deep cavity we apply liner and base(gic) also if we are using sandwich techq so can the chemical bond still be formed no matter liner is there or not?? I mean isn't the liner produce any hinderence in chemical bond of base with the tooth?

Liner is applied only on the floor below the gic.. the gic still touches the sode of the cavity and it will have chemical bond and the side and seal the cavity.

Yes. Both the tooth structure and the base had to be etched and bonded for COMPOSITE. However there are some articles which says bond strength to etched or non etched gic base is almost same. Some say etched is better.. I hope this answers is helpful :)
