This is a video about basics of inflammation of dental pulp referred to as pulpitis. In this video ी have described the details about different classification of pulpitis. I have broadly classified reversible and irreversible pulpitis. This video will help in correct diagnosis of diseases of pulp and help in accurate treatment planning.

I had a deep filling on one of my back teeth and it started to hurt two weeks after. It’s been a long time since then and I recently realized that the pain is not in the treated tooth but the cavity free tooth right next to it. I’m pretty sure it’s reversible given the symptoms you’ve mentioned but how is a reversible pulpits treated I really don’t want a root canal. Also I really don’t get how it’s the tooth next to it that hurst(I’ve tested multiple times and it’s only that tooth that reacts painfully to the cold and also when I knock onto it) . The dentist had to leave a bit of gap between the two teeth, do you think she let bacteria enter this way? I really need help and answers please

If tooth reacts painfully to cold stimulus then it's reversible pulpitis. And I think because of the gap that was left between two teeth enabled food impaction which served an ideal environment for bacteria to grow (poor hygiene could also lead to this). There might be a developing caries on distal side of your tooth that's hurting which you probably can't see in the mirror. I'd recommend you to visit a dentist before it becomes the case of RCT. (Dentist here)

Also, crowns (capping on tooth) are recommended to cover the naturally existing gap between teeth, a filling cannot cover that.

@@noshabashahzadikhan4108 thanks! I actually went already and turns out it was an infection of the nerve caused by placing the filling directly on the nerve. I didn’t need a crown and it was a really quick procedure I’m not even sure if it was a root canal. The dentist said he’d just cleanse/disinfect the nerve and it hasn’t been hurting ever since. Maybe you can tell me whether that was a root canal or a different procedure?

As pulp do not have proprioceptors how it can have reffere pain please explain

Yes Dr...during inflammation of pulp...there are few cardinal signs ( as with other tissues of body)eg redness, swelling ,colour change, temperature raise ...now due to swelling there is increasing pressure....this increase pressure leads to generation of impulses at at an area having greater density of nociceptors and thus pain is felt at the area where there is higher nociceptors density irrespective of local area

Sir if caries have involved the pulp then is it possible that the tooth has reversible inflammation?? Or the tooth has developed irreversible pulpitis??

Yes Dr As per your question yes there is chances that only coronal pulp is involved and hence there is reversible inflammation for which we may only remove partial pulp as in cases of partial pulpotomy

I get a filling 2 weeks ago my teeth are not really hurting but the gums around it are quite swollen my mom doesn't want me to go to the dentist again I'm very worried 😭

Don't worry it may be inflammation of gums which is usually due to plaque and calculus accumulated on teeth which leads to gum inflammation as a result of improper oral hygiene ...you need to visit dental specialist ...it will be easier for you..just don't worry and no need to delay

Thankyou Dr... regarding dull pain.... as seen in irrevrsible pulpitis is a chronic pain and explaining by an example suppose we have two nails... One with sharp point and other with blunt point... If we pierce the sharp end nail to skin we will feel intense piercing type of pain which will be localised(as seen in reversible )... While if we pierce the needle with dull end to skin then pain felt would be intense but not piercing and spread to a little more wider area(dull pain). Hope you got the point

I'm almost sure I have this I have pressure build in my head dull pain in the tooth but only when eating cold items is my reversible

The symptoms might include REVERSIBLE conditions but diagnosis is based on clinical evaluation,history(that includes your pain type which is dull in nature) and also radiographs...so i would suggest you to take an immediate opinion from your family Dental surgeon or specialist

I get this strong throbbing pain in my upper molars that come on very suddenly as quick as they come. Nothing provokes it and i went to the dentist and they found nothing, but i do have fillings on both of those teeth. What to you think this may be? I had abscesses on the bottom that they think caused referred pain but they were extracted over a month ago yet i still have pain in the upper teeth. What could this be?

Sudden throbbing pain may be due to reversible inflammation in these upper molars so might get re evaluation done of the fillings The second condition would be that if you had silver amalgam fillings then it may lead to such type of pain due to conductive nature to pulp In both cases the condition is reversible so get your fillings checked again and doctor may need to redo them if they are very close to pulp Like INDIRECT PULP CAPPING

I was also on clindymacin for 2 weeks both pre and post extraction so i don’t know if it’s an inflammation issue

Yes then it’s due to thermal conductivity to pulp as you have silver amalgam fillings so you need to get refilling done or may be IPC

@@guspolinskipolkakingofthem615 but 2 weeks is long time for throbbing pain so pls get checked once more and ask your doctor if there might be a reversible pulp inflammation due to this nature of filling and he may get an X-ray and evaluate to get it re done as these silver amalgam fillings are deep and almost near to pulp

If I create suction to my tooth I get the most excruciating pain also very sensitive to cold could this be pulpitis?

Yes this is mostly pulpal inflammation /pulpitis

However in your case this is an initial stage so must get it checked so that earlier you get treated for REVERSIBLE PULPITIS before it turns to reversible pulpitis

Very nice video sir. What will happen if direct pulp capping is done in irreversible pulpitis( due to excess drilling)....how long will it take for the tooth to become dead.

Thanks for your appreciation doctor Yes Dr In case of doing direct pulp capping in a tooth that has irreversible pulpal inflammation will lead to increase in inflammation and further development of abscess as inflammation or pulpitis is a consequence of an infection or bacterial infection so condition of tooth will worsen further In such cases appropriate treatment is non vital pulp therapy Hope you got my point doctor

I'm really scared the swollen tooth is really close to my front I'm having an anxiety i only have a small cavity on that teeth before i went to dentist i don't feel any pain on that teeth but after i went there my gums became swollen 😭😭😭 I'm just 17 I'm really scared on root canal 😭

Thank you for this very informative video doctor. I recently got wisdom tooth extraction, and now that the hole continues to heal, the adjacent tooth of the extraction site is being problematic. It hurts if it gets hit by cold water, and excavating wisdom tooth extraction site has become extremely painful because of it. It’s like the tooth is overly sensitive to the extent of pain. It started occurring a few days ago. The pain lingers even after spitting out the cold stimulus, but eventually goes away maybe after 15 min or more. I was curious whether this was a case of reversible/irreversible pulpitis. It currently aches as I type this as I had just brushed my teeth and excavated with a syringe not so long ago. 😂 I am very concerned as I am also diagnosed with periodontal disease. Will have to check in with dentists soon.

If pain is persistent even after removal of stumulus then definitely it’s a case of irreversible pulpitis and you may need to get pulp canal treatment so preferably get your tooth checked by specialist endodontist

Is it possible that after the filling she over drill it because when she's drilling after the filling it started to hurt and it's been 2 week my gum are still swollen it doesn't really hurt but sometimes it feels weird I'm really scared when i think about it i always cry that tooth is really close to the front tooth my mom doesn't want me to go to dentist again😭

Ok may be you had deep caries or we may say deep infection inside tooth and may have been involving pulp tissue of tooth so that's y the Dr needs to remove that caries or infection going further deep...you need to go to the dentist as without going it may further worsen your situation...just ask this question from your doctor that is your caries deep that it invoves pulp tissue..it he says yes..then you need pulp therapy

How does reversible pulpitis become irriversible? I think my tooth developed irreversible pulpitis when I wiggled it a bit with my tongue. ( i had a deep filling three weeks prior on it) is it possible this could happen upon just wiggling the tooth a bit?

Yes dr...as i had explained in the video and further elaborating that if condition or inflammation of reversible pulpitis are left untreated then eventually the bacteria will invade the pulp tissue and damage to such an extent that there would be infection, may be assc with swelling and body immune system Breaks down the dead tissue and ultimately abscess may be formed(in easy term long standing, untreated pulpal inflammation which had signs symptoms of reversible pulpitis will be now irreversible).... as far as wiggling(periradicular inflammation may be developing)is concerned pls visit your dentist for clinical and radiographic evaluation

IPC is indirect Pulp capping- usually there is no pulp exposure and only infected dentin is removed and affected dentin is covered with calcium hydroxide to remineralise DPC is direct pulp capping in which small pin point exposure occurs due to various reasons and in such cases calcium hydroxide is placed over that exposed area to restorative placed above it

Usually 2-14 days . In two days your coronar pulp will be fully inflamed and then the root pulp will start to inflame
