The serratus anterior muscle causes long-lasting chest pain, armpit pain and underarm pain, sometimes identified as shoulder blade pain.
The reason why relieving serratus anterior pain is so complex is that the muscle is intimately linked to bad posture.
In particular, a posture with round shoulders shortens the serratus anterior. But it will also cause forward head posture and thoracic kyphosis (aka slumped back), therefore neck pain and headaches.

Hi Oliver. I've had pain/soreness around this area for years now, I have it on both sides of the back. I went to a chiropractor for months, and then to a physical therapist, without any improvement. I've now instead been doing lots of dynamical stretches for the back (cat&cow stretch) for the last months, also been regularly foam rolling while doing different strengthening exercises for abs&glutes and back. I have a fairly good posture at the moment, but this constant soreness around this area still remains. I also added various strengthening exercises for the serratus anterior the last weeks, but this did not either help with the pain/soreness. I'm not sure if I can describe this pain in a proper way, but this area does always feel very tight and sore with pain. I try to focus on my posture when I sit, but it seems that if I sit too long then I get this soreness no matter what I do. Also if I'm standing up it gets easily sore after a while. Coming home from the gym, that area is always sore, and I need to rest in bed for it to get better. You seem to be educated and knowledgeable about this pain. I would really appreciate if you could give me some tips. I tried the stretch from this video, I can feel the stretch, and it does seem to have some effect. But I'm not sure if this will be enough. Would really appreciate your input.

Hi dear. It's good that you immediately think of your posture, because it's definitely what's primarily at stake. Basically, everybody who sits for too long will tend to slouch or slump, which will both push your shoulders forward, hence shorten serratus anterior. I'm also a bit cautious with the gym: it depends a lot on the exercises that you do there (check out my discussion with Charlie Richards in the comments). To understand the complexity of the issue, have a look at the video on forward head posture: ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-hdJ1H--uQnI.html. Step 2 would be to remove every prolonged source of things that you do with the elbows more than 20° forward (e.g., keyboard too far forward, or leaning back on your chair: ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-Fa4SFs85too.html), and every exercise that reinforces the muscles in charge of rouding your shoulders (e.g., push ups). Step 3 would be to leave the serratus alone, and focus on the antagonists (lower trapezius in particular). If you need any assistance for this, let me know!

Thanks for sharing its cause. And indeed, this pain does make you go crazy. It's not very painful but it is very irritating and kind of stressful. Although I don't sit at my table like you showed at 0:36, but while holding the phone in bed, I make this shoulder angle. I didn't feel any stretch while doing this exercise. Maybe this muscle needs a very slight stretch? Edit: I later followed the exercise shown at 1:38 by the video titled "Serratus Anterior Activation" of channel "Back Intelligence" and it helped me overcome this pain. Knowing the bad posture was half the solution though. Love your channel and the awareness you always bring, Mr. Olivier Girard!

You're very welcome, Morgan! It can actually also be that your rhomboids aren't too fine either, because both muscles suffer from the same issue, i.e. shoulders forward. I had a dislocated back for some years (probably 4th/5th thoracic spine) which caused constant upper back and neck pain...sometimes felt like a needle through my body... so far no physiotherapist could help me out... and after seeing this simple exercise i am wondering why. It prepared the muscels within 45 minutes in such a manner that the backspine snapped back into its normal position. It has been such a (pain) relief I can't describe! Thank you so much this exercise changed my life!

My problem is a bit different I guess. I went to a chiropractor who did manual adjustments. And he said when the spine in the upper part is out of alignment the pain can shoot out to muscles of the shoulder. No more serratus anterior pain but i still have some pain in my shoulder. It started 2 years ago when I was going camping and I had a super heavy bag with everything in it.

This helped fix my pec minor pain from benching. But instead of trying to stretch arm back with elbows facing where the books are (in this vid). I would put my elbow on other side of the bookcase, essentially trying to push arm forward.

Thanks for discussing this Doc! I am completely crazy from this, it's sooo persistant I've been frustrated for almost 2 years now. I spend money at doctors, etc, the pain persists. I sit at a L shaped desk. I turned my upper torso to my monitor, but didn't turn my lower body with my top.. I felt an instant tweak.

@@OlivierGirard This stretch helped a bunch, it's now in my daily stretching regimen. I'm currently just eating like complete shit. My BMI is 31%, that's obese. Which is wild because I still have abs.. I used to play college soccer and actually tested at 4% at the peak of my career. I'm going to really tackle my diet, sleep, and get more active. I can't be like this any longer.

@@glizz2188 It's not as painful anymore as it's stiffness in the joints which creates uncomfortable tension in the area. I just found "self back cracking" exercises from TikTok, check those out, they are very simple and help lots

You are 100% right on persistent "make me crazy" pain. This stretch (and the lying down version) are the only reliable ways ive found to get it to relax. I have had success combining it with fascia release work and have started to regain some normal use after too many years of issues.

I had a panic attack one night, but due to the scalene muscles (ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-nzY_zHLNV6k.html) which I had compressed in my sleep after a very stressful day... I was dreaming that I had a heart attack, so I woke up out of control, with my heart rate to the ceiling 😅. It ended up with the paramedics at home😂 Do you have constant anxiety or attacks like I had that time?

I have this same pain, gave me much of bad anxiety and panick attack cause I thought I was having a heart attack, got a heart monitor put on for a month and everything came back normal. All this started after going to a chiropractor

Thank you so much. I can feel my shoulder blade is sideways, and underarm pain. but can all these also cause nerve pain(nerve pinch like) down the neck because of the forward shoulder. I have seen multiple doctors so far they all say its just muscle pain. Thank God now I have some idea about the tightness of the serratus anterior which is pulling my shoulder blade side ways. Cant thank You enough.

Oh my ! I've thanked you already on one of your videos I've had (4 month ago ) terrible pains under the armpit ! Looking on the web I gathered that was serratus anterior : I'm on a fight against kiphosis and I had tried a Chloe Ting workout for "pretty neck and collarbone" when the pain (that subsidied nearly one month) started all of a sudden, living me no (mental) space for sleep. This was awfull and madening. Doctors didn't seem to know there was such a thing as "serratus anterior" and telling them about youtube beauty challenges made them look at me as if I was a mad hatter. Plus we were in covid ... i finally helped myself with cold showers and massage balls. Would you be french, by any chance ? If so, I want to move where you are (i'm French).

I'm currently working on my subclavius. I believe, thought my body posture has totally changed since the begining of my self taught journey towards health (nearly 3 years already) I'm slowly re-adjusting Bones, joints and muscles that never were properly functioning and it can be pretty painful at times. I recently discovered I had scoliosis too (there had been something about it told to my parents when I was a child but they didn't follow through and I had forgotten about it till this years, when I was filming me doing yoga and wondered what was that hunch in my back) and I'm doing the Austin Goh's Natural method exercices since last july but my, does it hurt !

I just did this for about 30 or so seconds and I felt IMMEDIATE RELIEF. THis has been bugging me for 3 days and I've been searching all over the net thinking I have cancer XD. I also noticed that I was much tighter on my left side than my right side!!

when I saw you at your desk, I realized it was maybe the sloping plastic camp chair I sit at for many hours a day, I am 81 years old & I sit at my Apple 13 " laptop Using my right hand. About two weeks ago I developed permanent pain under my right arm to back. I thought it was because of my asthma; I sleep with my arm thrown over my head to open up lungs. There also seems to be a calming effect.Now I think it is the chair I sit in but, being old the exercises seem daunting.

OMG I've been in pain for four months, thought it was another heart attack, couldn't sleep, four visits to osteopaths and I told them I had been heavily leaning on a kitchen worktop while online trading could this be the issue? I have this too, but I broke my shoulder 3 months ago. Unfortunately, the sling was ridiculous and never kept my arm immobilized. I’m mad that they never did anything about it and new it wasn’t healing correctly!

Hello. Please help with any direction or suggestions please.. I cannot sleep straight on my back at night without waking up in pain down my sides in the morning.... I've been sleeping sitting upright because I can't sleep my back strate. .. I have a strange feeling I got 2 different injuries.. I can lie on my right side with my arm bent under my head . But I cannot sleep on my left arm bent Under my head.. My arm just doesn't wanna go.. Then on my right side I cannot reach up the middle of my back properly and my shoulder is rounded and there is a short distance between the neck and shoulder.. When I do karate punches in the mirror my shoulder blades seem to do different things.. I also get pain under my color bone or up high on the chest under my shoulders If that makes sense... I KNOW THATS A LOT . BUT IM TRYING TO BE SPECIFIC.. I woke up balling my eyes out in pain this morning.. .. I've wasted my money on two P.T appointments but she was young and inexperienced and said " maybe you've had one rounded shoulder all your life.?".. This is ridiculous... It seems I'm going to have to find an answer first for them to find the problem.. .. I'm not the doctor and this is upsetting me cause I don't know what it is ... That's why I'm internet searching ATM.. Please help Also I just noticed leaning back in my stall with my back straight and flush against the wall one shoulder blade seems to stick out enough to feel on the wall and the right one feels flat / straight across the wall.. ( IT MUSNT BE OBVIOUS visually because the P.T gave me a massage and she obviously never picked up on it.. She did impingement tests cause I asked but I think she's missed something

Yes that’s where my pain is on the side and on the lower boarder of my scapula for 1+ year, you’re right I am crazy How do I determine if this pain is coming from my serratus or my lats? I stretch them as you shown but the pain does not budge. I tested my lats and it is tight and feels like paradise when I stretch it, For serratus I don’t seem to find the stretch, the satisfying stretch

Thank you Olivier! Brilliant video and so precisely explained. The diagrams correspond exactly to the pains I experience. Strangely, my pain occurs on both sides, either simultaneously or either side. Not sure whether this is common? I've only just found the video and have not yet tried the technique but, I think I'll start with 'lying down' version in your accompanying video. I have Paget's disease of bone in my T12 so have to be particularly cautious when performing stretches...

I wouldn't call bilateral pain "strange", David. This pain is related to rounded shoulders, which is often on both side, although it's often more marked on one side than on the other. My advice would be that you have a look at this long video (ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-9a5wkLnd54I.html) on how to fix forward head posture, because it's causal to rounded shoulders (as explained here in the long video from 1'52''). Furthermore, I agree 100% with you that lying down is a better position for you & everybody: more control, less weight on the tissues. Let me know how things evolve :) !!

Hey Chris. There are in my opinion several potential reasons for it: - the horizontal fibers of the latissimus dorsi are antagonist to the serratus anterior. Hence, when one develops pain, it exerts strain on the other; - it could be that both have been activated during the same type of activity (e.g., at fitness). Can this pain be exacerbated while running in particular? I find that when I run, I have pain under my armpit and in my shoulder blade. It usually goes away after a while and doesn't really bother me when I'm not running. Other exercise (like the elliptical). doesn't seem to exacerbate it much at all.

Yep dear, Travel & Simons write in the Trigger Point Manual that pain in the muscle may be activated by muscle strain during excessively fast or prolonged running. I guess it's due to the fact that you'll tend to round your shoulders in this context...

before i had costochondritis then it spread through my left shoulder and rhomboid, occipital and basically whole scapula and disturbed my life within these 6 months, this wasn't even enough now one week ago it has spread to the right side and my on my side and exactly on these points where you mentioned at 1:06 my right chest, right armpit, right serratus and right rhomboid plus when i deep breath then it get pain to my right chest, right armpit and right rhomboid

Yep, I see. I would hypothesize that the pain your felt due to costochondritis has altered your posture in such a way that you have started to round your shoulders and tense the chest muscles. Thereby, you could have entered in the vicious cycle that I describe in this video (ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-hdJ1H--uQnI.html), which can among others cause the pains that you describe. Could this make sense?

Hello sir...Actually i am feeling sudden muscle strain in my left shoulder, neck side and left breastside. It arise a week ago, when i woke up in the morning. I had severe muscle pain in left side with fever that I was unable to move myself. I took painkillers and felt okay but i am feeling random jerks with kind a heart pain ( 3-4 times a day) when i work. It went away with little bit stretching that time. Even, sometimes i feel heavy from my left side.I am wondering about any helpful exercise.

I did your exercise and it really fixed my problem. I had breathing difficulty and I was always burping everytime I press my armpits area. I had pain in my armpits, back shoulder blades for several months. My sinus also acted up. After doing your exercise, it just snapped and released the tightness in my armpits. I have been looking for lumps in my armpits, but none. Pain also runs to my breast that made me think I have breast problems. Thank you sooooo much.

Hello I have a slight winging of a scapula and I was strengthening this muscle to fix it but I think I strained it and I have pain when I take really deep breathes. Would this help and how long is recovery time?

Reg. recovery time I would be a zillionaire if I could predict this stuff... it depends a lot on how serious you are with exercises and modifications to your daily activities. Some people got better within weeks, some people never got better. What I know is that if you use your body well, things always go better than if you use your body wrongly... so just start working on it, and see where it leads you! Sir I have a tight pain in my mid back..and my upper back hurts if I try to do any stretch. And if I lie down on my bed.I feel my upper back muscles are moving..my right rib cage hurts a lot after I get up from sleep and the pain radiates to left side..if I bend my upper back it becomes tight n is painful.please tell me what exercise should I do..all this happened post c section delivery. @@MrLetsplayagame Hi, well I think over time got worse now the pain kind of spread out, now half of my face I feel pain left side to be specific, everything started 7 years ago and I kind of learn how to deal with it, exercise has been a key to keep it low, but there is some days that the pain is really bad and I just have to deal with it, at this point, I don’t know what to think. The pain is really sporadic and it could last 2 or 3 weeks after that I’m good for a month or 3 month depends. Listen, I recommend you to do physical exercise also mental health because this could give you of anxiety and depression a couple of days I was sharing my experience with a friend and he told me that probably I’m looking in the wrong place I don’t know if it makes any sense, but he could be right, good luck my friend.

Even when it's not flaring i have that dull pain there, and i tried so many things. Only the acupuncture sort my pain for the 1st time in all these years, but it lasted one month and a bit, with only 5 sessions to be frank, when it's supposed to be 10, i would recomend you this as well, if you didn't try it already. Wonder if you could help me, I received a vaccination recently and my arm was sore so I believe I slept awkwardly because of that? and the next day the a dull pain started towards the back of the arm pit all the way round to under the shoulder blade which I believe I have identified as the serratus anterior muscle. I’ve had this pain for months now which doesn’t seem to be going away. I am trying your stretch (just started) but one thing I wanted to ask (in case I should be doing these) is that I “feel” like I have some sort of small object buried under my skin around my shoulder blade (there isn’t any lumps it’s just a feeling). And I get some tingling on occasion. Im not sure if I have some inflammation that I may be aggravating? Full disclosure, I have had bad posture for years as well. Appreciate any advice as the doctors etc haven’t been very good at helping me this.

Round shoulders posture is very likely to cause tingling and pain around the shoulder blade (as well as neck pain, headaches, etc.). Hi dear, This deserves a complete answer... The desk is not the first thing to resolve, although it can help. What I mean is that working too much is just... working too much. No ergonomic setup will help you through a 10-hours working day that does not include several breaks🤯. What the best ergonomic setup will do is maybe let you work 20x30 min, but that's a max: - the lower back gets compressed after 25-30 min sitting, so you should stand at least twice an hour (check out this video on how to stand more: ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-jiOFLYKKigU.html). Per se, this still allows you to work on your screen if you have a standing desk; - however, you also have to consider your neck, shoulders and arms. Whether you sit or stand does not make much of a difference for them, if you keep working. And that's crucial in terms of round shoulders posture (i.e. pain in the serratus anterior). Fortunately, studies are unanimous🥳: people who take frequent short breaks work better, longer and with fewer mistakes than people who don't take breaks, or infrequent long breaks. So your health wants regular breaks, and so does your productivity (even if it takes quite some philosophy to get there). During your breaks, you should do this muscle relaxation exercise: ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-yxIkf36TRD4.html Hence, I can recommend a setup that will give you a wide range of healthy postures, from sitting 90° to sitting high to standing. But that should NOT be a reason to skip your breaks. That's it for me the boring one 😅... but seriously, trust my words. To the core of your question, now. Standing desks are rarely an issue. Just make sure that the minimum and maximum heights are according to my recommended technical specs (at the bottom of my B2B website: www.ergoh-conseil.ch/screen-work-training.html). Regarding the chair: The only one I know that lets you sit well supported at pelvic level whatever be your knee angle roughly between 90° and 120° is the HAG Capisco (store.flokk.com/us/en-gb/products/hag-capisco?store=auto). It's not cheap, so you should definitely test it before you buy it. I often order it for clients who have lower back or hip joint issues and who need to frequently change their sitting height. I have a little lower back pain, but mostly my pain is in my rhomboid or side area next to my ribs, and a fist height under my armpit. I went to a traditional massage therapist before and he told me that my muscle went and slipped into another muscle. Is that even possible? At first sight, the chair seems good. I think I have seen it already but I'm not sure. The min desk height of 72cm is kind of high: not a problem if you're tall, but if you aren't it could be a little too high. At worst, use a mini footrest :) I find the image of your massage therapist a bit strange, to be honest with you. Accidents have showed that more or less everything is possible, but I don't believe is something like this with someone who has a normal life... Your pain really look like serratus + potentially rhomboids, but anyway both reflect the same round shoulders issue. That's why I don't think that just switching from standing to sitting is likely to resolve anything... Now, regarding the duration of the breaks: 3 min every 30 min can be enough if you use them to relax all muscles (ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-yxIkf36TRD4.html), but 15 min every 30 min will be useless if you spend them on your phone. So you see, it really depends on what you do. My advice would be: get up twice an hour, either to work standing or to call or to have a coffee or whatever. And 1-2 times per hour, do the relaxation exercise above. Makes sense?

My pain is on top and around my left shoulder blades. I suspect it might be from my cervical since my xray shows that I already have osteoarthritis in my 20s... Also they give me ultrasound or shock therapy at the end of my sessions. Do you know if those even work? Thanks

​@@jsubroto2 Indeed, I'm not too worried about the 72 cm given your height (although it's not proportional). Reg. the armrests: they should be adjusted in height just below your elbows, and so should your desk. That's a first reason why they shouldn't be under the table, if we want to be accurate. But most importantly, if you slide them under the table, you're surely sitting too close to your desk: you'll see this here ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-a7qhAgXgQT0.html (sorry for the bad quality, I'll redo this video some day). What I mean with the image of your massage therapist: this idea that a muscle can slip into another feels kind of weird. I'm not 100% specialized in this level of detail but I never heard that before. Reg. the modalities (ultrasound, shock therapy, etc.)... you know, very often there is a bit of trial and error in this stuff. There is general guidelines, but also a lot of uncertainty. I think that the biggest issue is that it's symptomatic treatment: it does not answer to how you did this, whereas your body would surely recover by itself if the root cause would be removed (i.e. what you do which caused and now perpetuates the problem). That's where posture correction is a powerful tool...

Hey Joshua... There are a few things that need to be said here: - noisy joints have multiple explanations, and there are more doubts than certainties on the why. Most of the issues aren't serious, just muscle strain; - real shoulder issues have to be treated seriously. The shoulder joint is the most mobile one across your whole body, i.e. the most unstable one. When things go wrong there, they really do. Hence, it's important that a proper diagnosis is made by someone who sees you in person. If he tells me what he saw, then I can give you valuable advice on posture correction in your daily activities and exercises that you can do; - in the meantime, be aware that there are 2 common postural issues leading to shoulder disorders: shoulder abduction (i.e. elbows spread apart: ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE--kzOwko9jcY.html), and shoulders forward (which goes together with head forward: ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-hdJ1H--uQnI.html). Identify the situations in which you observe those, and fix them; - I would advise that you relax your muscles many times a day to avoid stiffen the tissues. Hello olivier. I'm been having pain between in my back beside the armpit and the part between chest and armpit. It also quite hurt at the back of my shoulder. It begin 2 days ago and now it so hurt to even raise my hand. I can straighten my hand horizontally and it not hurt as much as when i raise my hand but when i do that, my forearm also hurt. Is this seratus anterior pain? I just quite indecisive and doesn't want to start doing the exercise cause i scare it backfire and this start to affect my worklife. Please help me.

my left lower inner shoulder blade feels discomfort but not pain… it feels like the discomfort and tightness when you need to crack ur hand joints or something but don’t.. is there a term for discomfort like this? my neck posture also has gotten bad since covid started, i don’t know if it contributes though

I do one time on each side, typically 3 times a day, and you have to hold the position for 1 min without causing any pain

I have been having sternum pain, sternoclavicular pain which gives me a hard time to breathe, left shoulder, left trapezius, left rhomboid and left armpit. They did my ECG/EKG it was normal, they did Troppai it was normal, they did left shoulder pain MRI and posterior Thoracic MRI it was normal. It has been 8 months, I can't live normally. I have too much of pain every day as soon as I wake up it starts and remains throughout my day. What is going on ? Side note: sternum, upper shoulder and rhomboid pain is way too much and breathing pain is terrible. Now if i were to elevate my left shoulder doing abduction it was so much of pain like my nerves are stopping me to do this, feels like veins are too tight or stuck. And it also gives pain to sternum, shoulder and under armpit. I am sorry its too much but that's how I am going through every single day.

@@analomeli-rodriguez6205 nothing is really happening. I went through physiotherapy and chiropractor which made it worst. So I won't recommend physiotherapy unless if your physiotherapist is too senior and chiropractor could make it worst.

I think I found a way to stretch this while sitting down, By moving my left arm behind my back and grabbing the right side of the chair and then looking to my right. DO you think this would be a bad idea to do?

Great question, mate!! This is a great stretch for the supraspinatus muscle (ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-NgYEsJL3DWo.html), but put your right hand on your left collar bone when you do the exercise: you'll notice that it tends to shift forward... aka round shoulders, i.e. shortened serratus anterior. So although I started my investigations with this exercise too, I now don't think it's the best idea 😅
