When he is talking about the pain, it's his physical pain from Lyme disease that went undiagnosed for years which made the pain unbearable. He was never in a mental institution. He had several trips to the ER dealing with his illness and like many of us has some mental health issues like depression.
13 is usually taken as the number for bad luck. Ren is taking it as a measurement of the start of his illness in 2010 and when he wrote this track 2023. Weirdly his illness has led him to be successful because his art is based on expressing his painful journey. So 13 is just his luck ...
