I had 3 stents placed in my coronary arteries. I had high cholesterol(inherited) on was on statins for over 25 years and have run 4 times a week for over 35 years. Still I got blocked arteries. Luckily the exercise created collateral arteries and the statins turned the soft plaque into calcified plaque which is a good thing. I had zero symptoms but got a calcium CT scan score and it showed the issue. Now after quick recovery I am still on the statins, Clopidogrel(plavix) and an aspirin. I have improved my diet removing ALL junk food and lost even more weight. My BP was always normal. Diet will help but if you had hereditary factors that typical means that your liver is making most 80% of the cholesterol so hence need for statin and reducing simple sugars reduces inflammation which is now seen as just as important or even more important than just the cholesterol number. Look up Vitamin E Tocotrienol with Squalene, Sterol and Carotenoid together where you are. I am personally taking Tocovid SupraBio 200mg x 2 a day for cardio prevention esp atherosclerosis. It's long term insurance from stents. My work is stressful and stress produces cortisol which affects the artery...to mitgate stress I use Magnesium (there are many types of Magnesium but look for one that is good for anxiety). I take this one capsule a day in the morning amd one at night... and gotta drink water throughout the day because it will turn my pee bright yellow.

Would like to receive your advice, my father (68yrs old) has a stent since 2007 but last month we knew that the vessel is totally blocked (as doctot said it happened before and followed by heart attack which father did not feel at time). And now another vessel has a stent, but the previous one is totally blocked. Heart function is 46%. Blood vessel's vitality is 80% on blocked area. Sorry for the EXTREMELY late reply. I just saw this. You are absolutely correct. The wall is damaged-. That's why it's imperative patients are placed on some sort of antiplatelet therapy (Plavix or Brilinta) to keep the platelets from sticking to the vessel wall until the inner protective lining known as the intima regrows and protects the vessel. If not, clot will form and shut the vessel down.
Dr. I was recently diagnosed with advanced CAD . My calcium heart ct score was literally of the chart 2800 ! My cardiologist also had me do a nuclear heart stress test. Which to my amazement I passed. At this point he does not recommend surgery. Would this procedure namely the rooto rodder be a could option for me ???

Update to my situation from a year ago. Hopefully this helps someone. My heart calcium ct test was off the chart. I had a score of 2800. If you have a score that high don't wait. Have a heart catheterization to find out what's really going on ! Short time latter I had emergency quadrupled heart bypass surgery. I had 90 and 70 % blockages in my heart arteries. I got bad advice! Don't wait to have a heart attack! Get the heart catheterization as soon as possible!!!
Tim.. Were you supplementing with calcium daily basis prior to that checkup? Just asking because a relative had a minor stroke and the clot had to bd removed surgically... it affected abit of his speech now but otherwise OK. Root cause was calcification... he was taking Calcium diligently daily without Vitamin D3 and Vitamin K2 MK7. If any of you is taking Calcium dont do it daily. Try twice a week and always take with D3 and K2 MK7.
@@fuquplz9983 mom did not do it yet. My sister's husband did it. They did endiography or something like that but he has a lot of pain. He is drinking painkillers. They tried to clear the blocked artery but could not do it since it was a v shaped vein. They left a metal piece inside so that the artery remains open. Maybe they will remove it later, i dont know. But he is in pain. :(

No ECP is the non invasive method and cardiologist don't offer it to thier patients unless thier are no other options left. This approach is crazy coz who wants to try other methods which are invasive including one in this video while we have non invasive one with little to no side effects. It's available under the NHS in the UK but you are never told about it. Make your own research its called external pulsation system. If you can afford it its available privately in the UK and US as well as Indian subcontinent.
My father had 4 stents but after 1 year he developed problems and then we had to go for a bypass. I would request u all not to agree for implantation for more than 2 stents. It makes it difficult for surgeons to do surgery.

Antiplatelet agents like Plavix allow a controlled regrowth of the intima and helps limit the aggressive intimal hyperplasia that occurs normally in vascular damage that leads to restenosis and vessel occlusion. The concept of endovascular mechanical recanalization was actually developed by a cardiologist who designed the first atherectomy catheters for the coronary arteries! But you are right. To date these devices are not used in the small tortuous vessels of the heart.

@@MrSplash42 That's a great question. You can actually be checked at any age but life style and family history are important factors to consider in determining if you SHOULD be screened. If you told me that your mom or dad died in their 40's from a stroke or heart attack and your a current smoker with a sedentary life style, I think that would be a reasonable scenario for screening at the age of 36. However, if everyone in your family lived into their 80-90's and you currently exercise regularly and don't smoke, I think you'd be wasting your time and money. Rarely, their can be congenital anomalous origins of the coronary arteries that cause one of the coronary vessels to pass between the aorta and pulmonary artery (malignant course) which has been linked to sudden cardiac arrest in otherwise young, healthy individuals. Screening tests for arteries include the ABI (ankle brachial index), carotid ultrasounds and coronary calcification CT). Some screening facilities will do these test for cash and give you a result that you can take back to you doctor. I would first discuss your concerns with your personal physician and ask for their recommendation. May just start with a simple EKG. Hope that helps.

Is this main artery to your leg? If yes, I have successfully treated completely blocked arteries on numerous occasions. But not all blocked arteries require treatment. The body can develop small alternate channels around the blocked segment other words
@@ktmdays Fox Hollow, the company that designed and distributed this atherectomy catheter made a small one for the coronaries but I don't think it was very effective and may actually have been dangerous (perforating a leg vessel is one thing. Perforating a coronary artery could be fatal)! A company called penumbra has developed a small aspiration thrombectomy catheter for use in the coronaries but is usually used for more acute or subacute occlusions. With the additional clinical information, I agree with your doctors. It sounds like you have developed good collaterals around the area of blockage and have compensated appropriately for your vessel disease so try not to worry too much about your baseline disease. At this point, intervening may cause more harm than good. The best way to maintain those collateral vessels is regular aerobic exercise and a good, healthy diet! Good luck!
Hi Dr., My father is 86 and he is about to have a bypass to deal with blockage in an artery in his leg. The blockage goes from his groin to his knee. Is this procedure a better option for him? Is it covered by insurance? Thank you
It sounds like the occluded vessel is the superficial femoral artery or SFA. I have successfully opened up this time of blockage many times in the past. The key is the remain centrally within the vessel and not enter the wall separating 1 of the 3 vascular layers from the other (this is called a dissection). If that is done successfully, atherectomy can be performed followed by gentle balloon angioplasty to restore flow. If the vessel is dissected, probably relegated to a stent to maintain patency. @@judy9139 Very common. The SFA is very frequently affected by atherosclerotic disease. Once the vessel is blocked, it will clot back to the first major branch vessel which is often the profunda femoris. See it all the time...
Hi Doctor. After an MI in 20003, and an Out of Hospital Cardiac Arrest in 2008,,,,I am now with a CRT-D..... Which Test would tell me the status of my Coronary Arteries on a regular basis.
A definitive test is the coronary CTA or catheter angiogram. However, I wouldn't recommend either for regular screening. Much better off with lifestyle changes and yearly labs. Coronary CTA could be performed as a baseline exam which could be repeated if patient becomes symptomatic. We have started offering the Cleerly study which is basically an AI analysis of the coronary arteries using CTA data. It has a comparison feature which will allow the system to compare the prior and current studies next to each other to evaluate for progression of disease of efficacy of treatment. Again, I wouldn't repeat this test any sooner that 3-5 years, however.
@@vikrambakshi1 Unfortunately not. CT has a higher spatial resolution than MRI and can be obtained much quicker. Because of the lower spatial resolution and increased acquisition time, MRI cannot provide sufficient anatomic detail of the rapidly moving heart vessels to really evaluate the coronary arteries. In fact, I personally prefer CTA (CT angiography) over MRA (MR angiography) for just about all the vasculature in the body except the vessels of the head. MRI can evaluate the heart for areas of myocardial ischemia and infarct but, again, I wouldn't consider this a reasonable screening test.

There are two types of stents we use in the body, balloon expandable and self-expanding. Balloon expandable are almost always stainless steel, laser cut from a steel tube and provide good radial strength over a short distance. Self-expanding are usually an alloy of nickel and titanium (referred to as Nitinol). These stents enter the body compressed on a delivery catheter. As it's released off the catheter, it expands to the contour of the vessel. Radial strength is less than that of the balloon expendable but it's a better option for a long or tortuous segment of vessel as these devices tend to be more flexible and can adapt to the curvature of the artery or vein. Nitinol also has the unique property of changing configuration with temperature. Warming the device to body temperature after deployment increases the radial strength as the metal attempts to assume its maximum radial diameter. Bother materials are biocompatible.

Hi Sanju. I had bad sinusitis myself about 10 years ago and it was horrible. With the extreme discomfort and lack of appetite. I lost almost 10 pounds over the 3-4 weeks of the illness. I couldn't imagine living with that discomfort chronically. It's difficult to suggest a treatment regimen without seeing you or reviewing any of your imaging studies. There are different causes of sinusitis, most of which are bacterial requiring antibiotic therapy. However, over time, the thick mucous secretions in the sinuses can get a fungal superinfection requiring the addition of an antifungal agent. On CT, the fungal superinfection looks more dense than the standard bacterial sinusitis. If standard therapy fails, it's imperative that you have imaging studies of the paranasal sinuses to look at the anatomy and extent of disease. In some situations, patients may require sinus surgery to open blocked pathways and restore drainage channels. For myself, I was on 2 different antibiotics and I used a sinus flush (Neti pot) multiple times a day to help remove the thick secretions. I believe this was the key to my eventual cure. I know the pain is unrelenting but there are options to pursue. Please don't consider suicide. A good ENT (ear, nose and throat) doctor can tell you more about your options. I'm usually not impressed by these new technologies because, it implies that people will continue getting these blockages and machines like these will easily remove them. Teach people how NOT to get these horrible blockages is the first step, changes in diet and awareness about how to lead a healthy lifestyle is more important.
