Residents don't make that much, it's basically an extended internship. When she finishes her residency, she will likely be making much _much_ more than that.

The platinum card is for bragging rights, but it has no practicality. You can do whatever with your money, but I just have to share how that choice has little benefit. That $800+ is cash flow that can be saved to put in retirement accounts that most likely beat inflation and trust me you want to start this early. You should at least try to save 20%. Of course, when you become a full on surgeon your wants can increase a little bit, but you still have to be aware of lifestyle inflation. Love yourself enough today, so future you will be proud.

Fixed costs tend to be the necessities. They are costs that you don't really have much control over; you may be able to reduce them somewhat, but they can't really be eliminated. Subscriptions and memberships are _regular_ costs but they aren't really fixed, since you can quit them anytime without losing your life, your home, or your job. Budgeting is about prioritizing where your money goes, so identifying the spending that has the greatest consequences to reducing or cutting entirely is helpful for focusing your saving efforts.
You should be putting money into a Roth IRA not extra towards student loans. you will be able to pay off the loans once you’re a full doctor easily. Start saving for retirement now.
