After being a CRT since 2007 and now challenging the boards for my RRT, you have made all the lights in my brain click on! I have taken Kettering and I'm not bashing it. I just think for me..it told me things I needed to know but not why. Your breakdowns are awesome and I have so much confidence studying with your videos. The best money I ever spent was on the TMC Boot Camp! Where have you been all my life?! Lol. Thanks so much😊

Hello Joe! After graduation I was too upset about my best friends tragic passing, to do what was smart at the time… and take both exams back to back. I simply gave up all together and never pursued it. Then when I tried I kept failing miserably. But with the help of an old preceptor. I started studying hard recently again, and during a study session I ran into your hemodynamics video, and kept watching. I even reached out to you on Twitter and you guided me on which videos of yours to use for my weak points. Five years after finishing school (and giving up on myself), thanks to your videos and the help of fellow RT’s, I finally redeemed myself! and I’m officially and RRT!!! Thank you so so much for all you do! It made a huge difference for me, to refresh my brain and encourage me not to give up. I’m sure it will for others trying to succeed in their career too!!! 🥹❤️ you’re truly amazing! A million times thank you!

Great tutorials, as a NICU physician I cant get enough of your videos, and I appreciate what you´re doing. Id like to ask about an occurance that I encountered on Avea ventilator. The other day, the base of volume waveform kept going below the X axis, then coming back up. Which is kind of weird right? Does it indicate something specific, or is it just a malfunction, what do you think? Thanks again.

15yrs ago I took my TMC twice and failed it by 12 pts the first time and 2nd time by 2pts. I was crushed. During that time I worked at a hospital here in Texas under my temp license for one year and then continued on to work under a physician license and Corporate license as a Therapist. As RT are so high in demand at this time and Running into your Tik Tok videos OMG I am so stoked as so much has come to light and I know for a fact if I continue reviewing your video I have the confidence to pass the TMC.. I have e scheduled my test for the end of September Any videos you recommend to focus on And do you have a video that Covers basic Fundamentals and equations. Great stuff coach! Quick question. If you suspect a leak on a ventilator, what’s the easiest explanation why the volume waveform does not return to baseline, but the flow waveform returns to baseline?

Great question! Because the expiratory flow ceases, which takes your flow waveform back to baseline, but becuase some of the volume is lost in the leak, your volume waveform doesn't come back to baseline.

Hi coach, great video! Why does flow come back to baseline in a leak but not air trapping?

Because with air trapping the patient is still exhaling when the next breath occurs. With a leak there is a cessation of flow before the next breath despite not all the volume returning, so the flow waveform returns to baseline. Does that help?

I have a question . If a patient is breathing over the set tidal volume (ex: Vt 400 , patient going 800 or 900 )what does that mean ? What does it mean with the increase in RR too ? And what do you do ?

If you have a PEEP does the volume wave form return back to baseline?

Yes! Remember that tidal volume happens on top of PEEP, therefore all volume delivered during inspiration SHOULD be exhaled. If not, you're patient is air trapping.
