What are some possibilities if there are suspected L4 pathologies (fasciculations in quad and calves) but this test is a negative result?

What about numbness or tingling sensation .. its also considered positive .. right?

No, because that would be indicative of radiculopathy. We are trying to provoke radiculair pain that is shooting in Nature

was there any specific reason why stabilized glut fold area rather than PSIS when you tried to extend that patient's hip ?

If you want pure hip extensions, fixation has to be on the ischial tuberosity

Why did you not perform the test in side lying position? Easier for the therapist, especially if you want to add sensitizing manoeuvres.

Probably because this position does not allow the patient to work against the extension of the hip.

It's an option, but deviates from the original description. In the few patients with upper lumbar radicular pain that we've seen knee flexion is enough to elicit symptoms and knee flexion can be better controlled in prone position.
