If you are attaching things to your PFD like whistles and knives externally try using dental floss. Floss holds up to water if you replace yearly (also a good time to reassess what you are attaching), yet still breaks reasonably easily if you get hung up on something.

Flip lines: A local guide around here was carrying a flip line around his waist, ending up getting it caught in an oar, fell over the side of the raft, ended up trapped with his head submerged underwater. He was fine, but sketchy.

I keep around my waist, but tight, and tucked up under my PFD. Loose waist flip lines are like loose perimeter lines; it's bad, never do it.

I️ saw a bow like get stuck on a broken branch on a log on the river. Bow line was tight and small and the raft still got stuck. Fun watching the new guide sit in the raft getting bounced all over the place, but not fun as the section was tight and the raft was in the way of the line to run the rapid. All these choices have good and bad and as you said, we have to make that decision if it is best for us.

I definitely prefer having the knife accessible and not in a pocket. I flipped a gear boat and had a foot entrapment. I was glad I could grab my knife immediately and not have to search for it in a pocket.

I've used Scott from Sol Gear in Moab to build some specific flip lines for me. He's also built some other things I've needed. The service is top notch if you are looking for great stuff.

I've taken to doing caribeaners on the straps under the arm, clips pointed inwards. Keeps em out of the way but saves me space inside the pouch. During swiftwater, I had a rope get wrapped around my external knife, so I've taken to putting it inside the pouch, but on a string, so I can just grab the knife and it comes out of it's sheath in there. And the whistle I have on one of the straps to my shoulder straps. It has a little elastic on it, probably for tucking the strap down or something. I have a little watch on my shoulder strap, but it's just a plastic band and I don't think it'd hold my weight.

As a small guy, I understand people’s desire for a laddder, I think they are terrible. You can rig cam straps across the thort from outside d ring to outside d ring. then have 2 more going around the thort where it meets the tube to keep your 1st strap tight against the thort. This avoids entrapment hazard. (Clean line principles) Maybe a picture would explain this better, but being 5’ 4” I often struggle getting into boats but if I can get into a big tube 15’ aire that’s empty, then anyone can(with this method)

Most of the individuals who use these lines have scout bars on their high end cat frames. Those bars protect your legs from the line. I’ll take getting to boat back over faster than the potential of getting wrapped in the line in Class V water everyday of the week.

Great video. I stopped wearing my knife externally when it got caught on a strap while I was trying to self rescue after a swim on the Canyon.
