I will admit, THIS SKILL is one that I passed over and was almost the reason I would've failed the exam smfh. Yall please pay attention to the videos! You are supporting the arm at the wrist and elbow!! She specifically asked me that question after I performed the skill, I think the examiner was trying to trip me up but miraculously I answered correctly! I am now a CERTIFIED NURSE ASSISTANT!

Always raise the bed to waist level,locked the bed, lower the rail, and do toe plee with perfect alignment

Actually, for Prometric testing, you are not required to raise the bed to a comfortable height (it is not a tested checkpoint). Siderails are not used on the exam either. This skill does not do ANYTHING with toes...so...??? Not sure what you mean about a "toe plee" related to this skill. If you are talking about loosening the sheet over the toes when you make an occupied bed (that might be considered a toe pleat) it does NOT have to be perfectly aligned for the exam. Hope this helps!

Nice video :) I would just make sure that you have the patients palm up when doing shoulder abduction, so that there is less chance for an impingement to occur.

how come rotation (cookie dough rollout) is not performed? In the intro video it is explained as one of the exercises. I am taking my test in Florida and just want to make sure I am not missing anything. I bought the Clinical Manual from 4yourcna and it also does not include rotation.

Hi! Sorry for the delay in responding, this comment was routed to my spam folder. For the Florida exam, the care plan specifically states "Perform flexion/extension and abduction/adduction three times each". Since it does not instruct us to perform rotation, we cannot do that motion. The nurse has a specific reason for this - and we don't need to know why. CNAs always follow the care plan, the whole care plan and nothing but the care plan. Since the care plan does not instruct us to perform rotation ON THIS PATIENT, doing so would be an error and could possibly injure the patient (depending on their level of ability). The intro video shows all exercises that COULD be performed but the care plan will tell you which ones you actually WILL BE performing on THIS patient. Since all patients are different, each care plan will be different too. This care plan is from the testing scenario, so this is what you will be asked to do for the exam. I hope this helps! Hello!! Awaiting my exam date as for now but I’ve been wondering. When placing the call bell at the end of the exercise which side do I put the call bell on? Since we did passive rom do I assume that side is the affected side? Thank you so very much

When performing Range of Motion, you should provide a COMPLETE range of motion (up to the point of resistance) for each joint you are assigned to perform this task on. This is not OVER extending, this is proper range of motion. Over extending means to push a joint past it's current level of function (which induces pain). Remember, that as a CNA, we are not doing exercises to REGAIN motion or function, such as after an illness or injury or surgery - there is a whole department for that (Physical Therapy) and those individuals must obtain a Master's degree (this type of activity is WAY beyond the scope and practice of a CNA). In contrast, CNAs do exercises on unaffected (relatively healthy) extremities to RETAIN function. Since our goal is to prevent loss of muscle mass, strength or mobility, we need to provide a full range of motion to work the muscles adequately (but never to the point of pain). By only moving the arm level with the shoulder, you are not providing a full range of motion which could potentially lead to loss of function and mobility in that arm during the patient's recovery. If assigned to do this task, make sure you move the arm up to the point of resistance to ensure continuing function. Thank you and best of luck!

+Nicole Galbert Yes, that's what I meant. I'm guessing it doesn't matter, but you would think the last thing you do before leaving would be to wash your hands. Maybe my instructor is wrong.

My book says to wash hands first then hand them the call bell. Can I give them the call bell before I wash my hands for all of the skills? Or should I follow what my book is saying

The whole purpose of washing your hands is to remove pathogens that you encountered in the patient environment while performing your skill. You don't want to pass those pathogens on to other patients or yourself by touching your face. If you wash your hands, then touch the call light, you have re-contaminated your hands and they are no longer clean. The very last thing you do before leaving the patient environment is wash your hands - only AFTER you have done everything else. Otherwise you can spread pathogens around your facility. Best of luck!

You should wash your hands immediately before leaving the patient environment. Washing your hands and then returning to the bed to hand the patient his call light will contaminate your hands before leaving. You should leave a room with clean hands because you never know what is waiting for you in the hallway: a box of cookies from dietary department for nurse's week or a fallen patient - you always need clean hands to handle each of those! After everything is done, THEN you say you are finished. You cannot take any action on a skill once you say you are finished. Hope this helps!

When doing shoulder abduction. I've been told not to move above level with the shoulder. Do I do it as in the video, all the way up. Or stop at abducted level with the shoulder?

If you refer to the Clinical Skills Checklist for ROM Shoulder, Checkpoint 6 states: Move resident’s shoulder through ROM taking arm away from side and back to side (abduction/adduction) with no requirement of remaining level with the shoulder. ROM should be done to the point of resistance (or pain) to ensure a completion motion and maximum benefit. Please refer to the Clinical Skills Checklist on Prometric's website for your state to view the actual graded checkpoints that you are being graded on. These are the required steps: No more, no less. I'm in California & I'm wondering if there are discrepancies between the way that the test is administered from state to state or if I have extra steps in my mind for no reason. Example: In my classes shoulder PROM consisted of 4 exercises - you're only showing 2... do I have needless info or does it vary from state to state (and you're just to in CA)?

Thank you for your comment! There are several different exercises that CAN be performed on each joint, but every patient is different and will require different care. That is where the care plan comes in - the care plan tells you exactly what to do for each individual patient (see our care plan video ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-pOQO6TpabP8.html). So, you must do exactly what the care plan tells you to do for the exam. If you refer to the clinical skills checklist for Pearson Vue for California, you will notice that only flexion/extension (up/down) and abduction/adduction (side to side) motions are required for this patient. You cannot perform any other exercises (even if you know how) because THIS patient does not need them and it could injure the patient if you do something other than what is on the care plan. You will learn other motions in class because OTHER patients that you care for in a clinical setting might require those, but the exercises for the test will be specifically laid out in the care plan for you to follow. for the test i heard you dont need to touch the bed at all (to bring bed up ) so you dont have to remember bringing the bed down.

The forearm should be in supination at 90-180 degrees of abduction. 👌🏽

If you refer to the clinical skills checklist for this skill, there is no checkpoint that specifically requires this. A neutral position of the forearm is actually preferred for CNA purposes, unless the care plan specifies otherwise (and that is not generally a CNA task - since CNAs do "normal"). Tendon and muscle-isolating exercises are performed by trained Physical Therapy specialists who have 6-8 years of training and perform exercises to obtain a desired increase in strength or mobility. CNAs do exercises to maintain the mobility the patient currently possess. This is an important distinction. Thank you for your comment.
