SPOILERS (potentially) it's probably inner conflict at this point (Tatariakashi-hen) for context Tataridamashi-hen plays out similar to Minagoroshi-hen except at the end Presumably Teppei comes and goes to smack keiichi around (bc satoko leads him to her house) Keiichi kills (again presumably) Teppei. In the next episode Oiishi is shown with the same bloodied bat Keiichi used and kills Mion, Shion, Rika, and then Satoko my theory is that Keiichi naturally reached L5 of hinamizawa syndrome, hallucinated Teppei, who was in actuality Oiishi, whom Satoko injected with L5 at somepoint, which explains the already bloodied clothes he's wearing when he kills everyone else at the watanagashi festival if this is right that means that his aggression towards the hinamizawa dam fighters (rebranded to "save" satoko from her abusive uncle) was bc he was already L5 I believe Satoko blackmailed Teppei to cause her harm and make herself bruised and shit so that the CPS calls would sound legit, so she could trick rika into thinking it was a fragment similar to Minagoroshi-hen, where Teppei ACTUALLY abused Satoko
