When categorizing scoliosis, the patient's age is one of the first things to consider. The same thing is considered when thinking of what kind of treatment options to provide for your patient's scoliosis.
The most common goals in scoliosis treatment are:
1. Reducing the scoliosis
2. Prevent further progression
3. Improve function and reduce pain
4. Long-term stabilization
Once a curve breaks out from 25 degrees, it is likely to continue to progress, and you need to focus on how to treat the curve to prevent progression.
The different types of scoliosis by age are:
-Infantile Scoliosis 0-2 years
-Juvenile Scoliosis 3-10 years
-Adolescent Scoliosis 11-17 years
-Adult Scoliosis 18-59 years
-Elderly Scoliosis 60 plus years
Infantile scoliosis is one of the most challenging because children can not perform any exercises on their own at this stage of their life. In this case, there are only two forms of treatment that can be used:
-Scoliosis specific therapy
-Scoliosis corrective bracing
One of the main requirements to look for on infantile scoliosis is a significant reduction once the treatment has begun. Due to the number of years to come as a child grows and how likely it is that this curve will see a progression.
For the next age group, juvenile scoliosis is the one that sees the most positive long-term results. This is due to how flexible their curve is and because their curve is also smaller (under 30 to 25 degrees).
This is usually because of their age and lack of compliance with the treatment. The normal go-to is bracing to hold the curve in place, and once it worsens, more treatment options are given. However, Dr. Tony Nalda considers the age group of juvenile scoliosis as the one with the groups with the best possibility of improvement when treated.
Methods such as exercising, self-corrective exercise, therapy, chiropractic care, and bracing can be used. Just as it is with infantile scoliosis, the goal is to get a reduction, not just to hold the scoliosis.
Adolescent scoliosis is the age group with the most rapid progression. The fastest progression that can be seen is 10 degrees in a month for the standard, but there might be some months with even faster progression in some cases, especially for girls aged 11 to 12 years old.
The danger of watching & waiting with adolescent scoliosis is that the patient can skip the treatment window between one consultation & another. And due to a rapid progression, the patient becomes a surgical case in months. Most adult cases get diagnosed at 35 to 45 years of age due to pain and other related symptoms. Due to this, most adult patients, when seeking out help for pain relief, is when they get diagnosed with scoliosis.
Initially, patients focus on symptom-based treatment, such as massages, particularly adults diagnosed from a young age. Dr. Nalda focuses adult scoliosis treatment in curve management to help reduce symptoms & pain.
The final age group is elderly scoliosis. These patients are more delicate to treat, mainly because there is a major loss of mobility for them as scoliosis progresses. There is also loss of many other abilities, such as walking & standing on their own.
Another factor is that scoliosis at this age is very quick to progress. Elderly scoliotic curves can grow 10 to 15 degrees a year, which can cause body changes, increased limitations, & spinal degeneration.
In these cases of elderly scoliosis, the main & first goal not involving surgery is to stop the progression.
